Tillfredsställelse: En fas I/II, öppen, multicenterstudie av [225Ac]Ac-PSMA-R2 hos män med kraftigt förbehandlad PSMA positiv metastatisk kastrationsresistent prostatacancer (mCRPC) med eller utan tidigare 177Lu-märkt PSMA -riktad radioligandterapi.

Detaljerad beskrivning
Detta är en öppen fas I/II, multicenterstudie som innehåller två behandlingsgrupper (Grupp 1 och Grupp 2). Varje grupp har en dosökningsdel, när den maximala tolererade dosen/rekommenderad dos för expansion (MTD/RDE) har bestämts i var och en av dosökningsdelarna kommer studien att fortsätta med en expansionsdel i respektive grupp.
Dosökningsdelarna kommer att fastställa MTD/RDE för 225Ac-PSMA-R2 styrd av den väletablerade Bayesian Logistic Regression Model (BLRM)-metoden. Den adaptiva BLRM kommer att styras av principen Escalation with Overdose Control (EWOC) för att kontrollera risken för DLT hos framtida deltagare i studien. Doseskaleringsbeslut kommer att tas av utredarna och Novartis under dosupptrappningsmöten (DEM) baserat på säkerhets- och tolerabilitetsinformation (BLRM-sammanfattningar av DLT-risk) tillsammans med farmakokinetik och preliminär effektinformation.
Dosexpansionsdelarna kommer att bedöma antitumöraktiviteten (Overall Response Rate (ORR) av Prostate Cancer Working Group 3 (PCWG3) modifierad RECIST 1.1 och Prostate Specific Antigen 50 (PSA50) svarsfrekvens) samt ytterligare bedöma säkerheten, tolerabiliteten och PK av 225Ac-PSMA-R2.

Viktiga inkluderingskriterier:
- Bevis på PSMA-positiv sjukdom genom 68Ga-PSMA-R2 PET/CT och kvalificerad enligt central avläsning
- Dokumenterad progressiv mCRPC
- Tillräcklig organfunktion (benmärgsreserv, lever, njure)
- Tidigare orkiektomi och/eller pågående ARPI och taxanbaserad kemoterapi och borde ha fått tidigare 177Lu-PSMA-RLT (Group1 dos ecalation & expansion) eller aldrig fått 177Lu-PSMA-RLT (Group 2 do eskalation & expansion).
Viktiga uteslutningskriterier:
- Alla andra prövningsmedel inom 28 dagar efter den förväntade C1D1 av 225Ac-PSMA-R2-behandlingen
- All systemisk anti-cancerterapi inom 28 dagar efter den förväntade C1D1 av 225Ac-PSMA-R2-terapin
- Okontrollerad smärta eller inkompatibilitet som kan resultera i deltagarens bristande förmåga att följa avbildningsprocedurer
- Anamnes med CNS-metastaser och symtomatisk kompression av navelsträngen, eller kliniska eller röntgenologiska fynd som tyder på förestående kompression
- Okontrollerad kardiovaskulär historia
- Diagnos av andra maligniteter som förväntas förändra livslängden eller kan störa sjukdomsbedömningen
Andra protokolldefinierade inklusions-/exkluderingskriterier kan gälla.
