研究期估计为 12 个月 + 12 个月的随访(5 年后进行第二次随访),从 01/04/2017 开始。
参与者 所有疑似 AA 的成年患者(年龄超过 18 岁)将被送往九家参与的意大利医院的外科,在那里他们将由值班外科医生仔细研究。

接受阑尾切除术作为一线治疗的患者
开腹或腹腔镜阑尾切除术将每位患者分配到“抗生素优先管理”组或“立即手术”组,将是非随机化的,并由随叫随到的工作人员专家外科医生在仔细评估 AIR 评分后独立决定,实验室检查结果和成像。
管理路径的决定在任何情况下都不会受到患者参与研究的影响,治疗的分配将由顾问外科医生根据意大利当前良好的手术实践和标准实践模式来决定。

采用抗生素优先策略治疗的患者
抗生素治疗。
保守治疗的患者将接受以下肠外抗生素治疗之一: 哌拉西林/他唑巴坦 (4.5 g),每天静脉注射 3 次;头孢曲松 (2 g) 每天一次或环丙沙星 (500 mg) 每天两次加甲硝唑 (500 mg) 每天三次;阿莫西林/克拉维酸(2 克),每天四次,持续时间视临床情况而定;厄他培南 (1 g) 每天一次,连续三天。
患者出院时口服抗生素(阿莫西林/克拉维酸或环丙沙星)至少四天。
