Appendectomie versus conservatieve behandeling voor ongecompliceerde acute appendicitis (ACTUAA)
Een prospectieve, niet-gerandomiseerde, gecontroleerde, multicenter, multidisciplinaire studie waarin appendectomie en conservatieve behandeling voor patiënten met ongecompliceerde acute appendicitis worden vergeleken (de ACTUAA-STUDIE).
Op 15 september 2015 kwamen Italiaanse chirurgen, radiologen en pathologen met een speciale interesse en expertise in de diagnose en behandeling van acute appendicitis (AA) bijeen onder auspiciën van de Italian Society of Hospital Surgeons (ACOI) in Oristano (Italië). ) om de ACTUAA-samenwerkingswerkgroep op te richten. De belangrijkste doelstellingen van de werkgroep zijn:
Een werkbasis creëren voor het analyseren van de diagnostische kenmerken, behandelingsmodaliteiten en uitkomsten van belang van zowel de antibioticum-eerst-benadering als appendectomie voor patiënten met ongecompliceerde AA.
De klinische, laboratorium- en radiologische modaliteiten onderzoeken die voor de diagnose zijn aangenomen. De resultaten bepalen van patiënten die worden behandeld met antibiotica of appendectomie op korte en lange termijn.
Om de resultaten te vergelijken op basis van het type interventie. Het risico op recidief stratificeren voor patiënten die met antibiotica worden behandeld op basis van klinische, laboratorium- en radiologische bevindingen.
De gevoeligheid en specificiteit evalueren van klinische en laboratoriumscores voor de diagnose van ongecompliceerde AA.
Het identificeren van een subgroep van patiënten met ongecompliceerde AA voor wie een antibioticabehandeling zeer effectief kan zijn.
Algemene onderzoeksopzet Het onderzoeksprotocol is opgesteld volgens de "SPIRIT 2013 toelichting en uitwerking: leidraad voor protocollen van klinische studies".
De studieperiode wordt geschat op 12 maanden + 12 maanden follow-up (met een tweede follow-upsessie na 5 jaar), beginnend op 01/04/2017.
Deelnemers Alle volwassen patiënten (ouder dan 18 jaar) met verdenking op AA worden opgenomen op de chirurgische afdeling van de negen deelnemende Italiaanse ziekenhuizen, waar ze zorgvuldig worden bestudeerd door de dienstdoende chirurg.
Patiënten worden vervolgens geïnformeerd over het onderzoeksprotocol en uitgenodigd om schriftelijke toestemming te geven voor deelname en voor verstandige gegevensverzameling voor wetenschappelijke doeleinden.
Vervolgens kunnen alleen patiënten die diagnostische beeldvorming ondergaan (zoals later gespecificeerd) en van wie een schriftelijke geïnformeerde toestemming wordt verkregen, worden ingeschreven in de studie en door elk centrum worden geregistreerd met behulp van een uniform elektronisch registratieformulier en database.
Algemene kenmerken, medische voorgeschiedenis, klinische bevindingen, lichamelijk onderzoek en bloedonderzoek worden vermeld in het medisch dossier. Pijn zal worden gekwantificeerd door middel van een Visual Analogue Scale (VAS) scoresysteem voordat pijnstillers worden toegediend en na de behandelingen.
Om aan het onderzoek deel te nemen, moeten patiënten diagnostische beeldvorming ondergaan (US- en/of CT-scan of MRI-scan) en alleen de diagnose van ongecompliceerde AA, bevestigd door diagnostische beeldvorming, maakt inschrijving van de patiënt in het onderzoek mogelijk.
Specifieke doelstellingen van de ACTUAA-studie Het doel van deze prospectieve, niet-gerandomiseerde, gecontroleerde, multicenter, multidisciplinaire studie is het vergelijken van de antibioticatherapie en een nood-appendectomie voor de behandeling van patiënten met ongecompliceerde AA (zonder abces of vrije perforatie), bevestigd door VS en/of of CT- of MRI-scan. Voor de deelnemende centra zijn geen aanpassingen nodig in de dagelijkse praktijk op het gebied van diagnostisch, klinisch en behandeltraject. Alleen de patiënten met ongecompliceerde AA, bevestigd door een van de bovengenoemde radiologische hulpmiddelen, zullen echter in de studie worden opgenomen. De beslissing over welk van de hulpmiddelen moet worden gebruikt, is aan de lokale hoofdchirurg.
Primaire uitkomstmaat:
Aantal deelnemers met succes van de behandeling zonder complicaties, gedefinieerd als het succes van de initiële behandeling met een ongecompliceerd verloop.
Secundaire uitkomstmaten:
Duur van het ziekenhuisverblijf; Pijnevaluatie; Tijd om terug te keren naar de normale activiteit; Periode van ziekteverlof; Gecompliceerde blindedarmontsteking met peritonitis vastgesteld tijdens de chirurgische ingreep; Kwaliteit van leven zoals beoordeeld door de Short Form 12-schaal (SF-12)

- Ondertekende geïnformeerde toestemming
- Leeftijd ouder dan 18 jaar
- Ongecompliceerde AA bevestigd door echografie en/of CT- of MRI-scan.
Uitsluitingscriteria:
- Zwangerschap of borstvoeding
- Diffuse peritonitis bij lichamelijk onderzoek
- Ernstige systemische ziekte
- Positieve anamnese voor inflammatoire darmziekte (IBD)

Patiënten ondergingen appendectomie als eerstelijnsbehandeling
Open of laparoscopische appendectomie De toewijzing van elke patiënt aan ofwel de "antibiotica-eerste behandeling"-arm of de "onmiddellijke operatie"-arm zal niet-gerandomiseerd zijn en onafhankelijk worden beslist door de stafspecialist-chirurg van wacht, na zorgvuldige beoordeling van de AIR-score, laboratoriumbevindingen en beeldvorming.
De beslissing over het behandelingstraject zal in geen geval worden beïnvloed door de deelname van de patiënt aan het onderzoek, en de toewijzing van de behandeling zal worden bepaald door de adviserende chirurg volgens de huidige goede chirurgische praktijken en standaard praktijkpatronen in Italië.

Patiënten behandeld met antibiotica-eerst-strategie
Antibiotische therapie.maging.
Patiënten die conservatief worden behandeld, krijgen een van de volgende parenterale antibioticabehandelingen: Piperacilline/Tazobactam (4,5 g) drie intraveneuze toedieningen per dag; Ceftriaxon (2 g) eenmaal daags of Ciprofloxacine (500 mg) tweemaal daags plus Metronidazol (500 mg) driemaal daags; Amoxicilline/Clavulaanzuur (2 g) viermaal daags gedurende een periode die afhangt van de klinische toestand; Ertapenem (1 g) één toediening per dag gedurende drie dagen.
Patiënten werden ontslagen met orale antibiotica (amoxicilline/clavulaanzuur of ciprofloxacine) gedurende ten minste vier dagen.

Aantal deelnemers met succes van behandeling zonder complicaties, gedefinieerd als succes van de initiële behandeling met een ongecompliceerd beloop: geen postoperatieve complicaties, bijwerkingen of falen van de behandeling, zoals beoordeeld door de Dindo-Clavien-classificatie.
Buikabces na behandeling, darmocclusie, littekenbreuk, longembolie, cardiovasculaire complicaties, postoperatieve wondinfectie, complicaties als gevolg van anesthesie, bijwerkingen van antibiotica.
Complicaties worden geanalyseerd voor zowel patiënten die een appendectomie hebben ondergaan als voor degenen die zijn behandeld met een operatie als tweedelijnsbenadering, nadat de primaire antibioticabehandeling heeft gefaald.
Voor patiënten die met antibiotica worden behandeld, zal het falen van de behandeling (persistentie en recidiefpercentages van acute appendicitis) worden beoordeeld binnen het totale aantal complicaties.
Er zullen specifieke deelanalyses van de complicaties worden uitgevoerd.
