BAGGRUND:
Hypertension er blevet klart identificeret som en væsentlig risikofaktor for koronar hjertesygdom og slagtilfælde. Hypertensionsscreening og interventionsprogrammer for børn er baseret på konceptet om, at forhøjet normalt blodtryk kan være barndomsforløbet til voksenhypertension, eller at blodtrykket 'sporer' gennem barndommen, ungdommen og voksenalderen. Blodtryksmåling er veletableret for voksne og unge voksne, men resultater fra forskellige undersøgelser af børn er kontroversielle. Ved et symposium om forhøjet blodtryk hos de unge i 1979 blev det konkluderet, at der ikke var tilstrækkelig information til at anbefale specifikke interventioner til præpubertale børn med blodtryksniveauer i de øvre percentiler af blodtryksfordelingen. Undersøgelser af længere varighed med store populationsstørrelser og gentagne blodtryksmålinger over tid var nødvendige for at bestemme, om blodtryksniveauer i disse unge aldre var prædiktive for voksenhypertension.
DESIGN FORTÆLLING:
I foråret 1978 blev i alt 10.423 børn i alderen 6-8 år screenet. En delprøve blev inviteret til at deltage i undersøgelsen af blodtryksmåling. Prøven blev udvalgt som følger: alle børn i de øverste fem procent af den indledende systoliske blodtryksfordeling; alle børn i de nederste fem procent af den indledende systoliske blodtryksfordeling; en tilfældig stikprøve af en ud af to af de resterende sorte børn; en tilfældig prøve på hver ni af de resterende hvide børn; og alle de resterende børn af andre racer. Inkluderet var familier med omkring 200 indianske børn. Ud af 2.641 udvalgte børn nægtede 878 at forlade en prøve på 1.753. Baselinedata om alder, race, køn, triceps hudfoldtykkelse, kosthistorie, urinudskillelse af natrium og kalium, fysiologiske mål, ændringer i livsbegivenheder, sygehistorie, demografiske data og kropsstørrelsesmålinger af børn og deres forældre blev indsamlet ved det første hjemmeinterview udført mellem 1978 og 1979. Børnene blev fulgt to gange om året i 1978 til 1980 og 1982 til 1985 for blodtryk og kropsstørrelse. Der blev foretaget årlige målinger af blodtryk, højde, vægt, hudfoldtykkelse og kropsomfang. Screeninger fortsatte gennem begyndelsen af 1991, da kohorten var 19-21 år gammel.
Adskillige delundersøgelser blev udført, herunder forholdet mellem kationtransport af røde blodlegemer og essentiel hypertension, type A adfærdsvurderinger og familiær aggregering af koronar-tilbøjelig adfærd.
