Estudio de reducción de analgésicos opioides (OARS): manejo del dolor posoperatorio agudo
El objetivo es proporcionar a los profesionales de la salud, incluidos los dentistas, la mejor evidencia posible para la toma de decisiones clínicas al decidir sobre analgésicos para el manejo del dolor posquirúrgico agudo, se realizará un ensayo clínico aleatorizado estratificado, doble ciego para probar la hipótesis de que una combinación de analgésicos de venta libre que no contienen opioides es al menos tan eficaz (no inferior) como el analgésico opioide recetado con más frecuencia, si no más. Se utilizará el modelo de extracción del tercer molar impactado debido a la severidad predecible del dolor postoperatorio y la generalización de los resultados.
Este ensayo clínico pragmático, aleatorizado, prospectivo, estratificado y doble ciego utilizará el modelo de dolor por extracción del tercer molar impactado.

Descripción
Criterios de inclusión:
Una persona debe cumplir con todos los siguientes criterios para ser elegible para participar en el estudio:
- Ser capaz de comprender el consentimiento informado.
- Proporcionar formulario de consentimiento informado firmado y fechado
- Ser capaz de comprender todas las instrucciones de los instrumentos de recopilación de datos en inglés
- Estar dispuesto y ser capaz de cumplir con todos los procedimientos del estudio y estar disponible durante la duración del estudio.
- Planificación para someterse a la extracción de uno o más terceros molares mandibulares parcial o totalmente retenidos
- Ser mayor de 18 años
- Estar en buen estado de salud general como lo demuestra el historial médico.
Las mujeres deben aceptar usar uno de los siguientes métodos anticonceptivos mientras participan en este estudio:

Criterio de exclusión:
Quedarán excluidos de participar los participantes que autorreporten el siguiente historial:
- Historia de sangrado gastrointestinal y/o úlcera péptica
- Antecedentes de enfermedad renal (excluyendo cálculos renales)
- Historia de enfermedad hepática
- Antecedentes de trastorno hemorrágico
- Antecedentes de depresión respiratoria.
- Cualquier efecto respiratorio previo de un opiáceo u otros fármacos anestésicos que hayan requerido asistencia respiratoria en el posoperatorio
- Asma activa o no tratada
- Antecedentes de reacción alegórica conocida al ibuprofeno, paracetamol, hidrocodona y/o anestesia
Actualmente tomando alguno de los siguientes medicamentos:
- Inhibidor de CYP3A4, como antibióticos macrólidos (p. ej., eritromicina), agentes antifúngicos azólicos (p. ej., ketoconazol) e inhibidores de la proteasa (p. ej., ritonavir), que pueden aumentar las concentraciones plasmáticas de bitartrato de hidrocodona y paracetamol y prolongar las reacciones adversas de los opioides, y que puede causar depresión respiratoria potencialmente fatal
- depresores del SNC.
- Consume 3 o más bebidas alcohólicas al día y/o tiene antecedentes de alcoholismo
- Antecedentes de abuso de drogas o alcohol
- Antecedentes familiares de abuso de drogas o alcohol en un pariente de primer grado
- No ha recibido más de una receta de opioides en los últimos 12 meses
- Actualmente embarazada o lactando
Los participantes también serían excluidos debido a cualquier criterio adicional que pondría al individuo en mayor riesgo o impediría el cumplimiento total del individuo o la finalización del estudio, que incluye:
- Participación previa en este estudio
- Incapacidad o negativa a proporcionar el consentimiento informado

NIH Required Analysis - Description Before going to sleep participants are asked to rate their "worst pain experienced", "least pain experienced" and "average pain experienced" during the day along with "pain now."
Upon waking participants were asked to rate their "worst pain experienced", "least pain experienced" and "average pain experienced" during the night along with "pain now."
A NRS scale of 0 to 10 where 0=no pain and 10=pain as bad as you can imagine was used.
