Estudo de Redução de Analgésicos Opioides (OARS): Manejo da Dor Cirúrgica Pós-Operatória Aguda
O objetivo é fornecer aos profissionais de saúde, incluindo dentistas, a melhor evidência possível para a tomada de decisão clínica ao decidir sobre analgésicos para o controle da dor aguda pós-cirúrgica, um ensaio clínico randomizado estratificado duplo-cego será conduzido para testar a hipótese de que uma combinação de analgésicos não opiáceos de venda livre é pelo menos tão, senão mais, eficaz (não inferior) do que o analgésico opióide mais comumente prescrito. O modelo de extração de 3º molar impactado será usado devido à severidade previsível da dor pós-operatória e generalização dos resultados.
Este ensaio clínico pragmático duplo-cego, prospectivo, estratificado e randomizado usará o modelo de dor de extração de 3º molar impactado.

Critério de inclusão:
Um indivíduo deve atender a todos os seguintes critérios para ser elegível para participar do estudo:
- Ser capaz de compreender o consentimento informado.
- Fornecer formulário de consentimento informado assinado e datado
- Ser capaz de entender todas as instruções para instrumentos de coleta de dados em inglês
- Estar disposto e capaz de cumprir todos os procedimentos do estudo e estar disponível durante o estudo
- Planejando a extração de um ou mais terceiros molares inferiores parcial ou totalmente impactados
- Ter 18 anos ou mais
- Estar em boa saúde geral, conforme evidenciado pelo histórico médico
As mulheres devem concordar em usar um dos seguintes métodos de contracepção durante a participação neste estudo:
- pílula anticoncepcional
- dispositivo intrauterino
- preservativos
- abstinência
Critério de exclusão:
Os participantes que relatarem o seguinte histórico serão excluídos da participação:
- História de sangramento gastrointestinal e/ou úlcera péptica
- História de doença renal (excluindo cálculos renais)
- Histórico de doença hepática
- História de distúrbio hemorrágico
- Histórico de depressão respiratória
- Qualquer efeito respiratório anterior de um opioide ou outro anestésico que exigisse suporte respiratório no pós-operatório
- Asma ativa ou não tratada
- História de reação alegórica conhecida ao ibuprofeno, paracetamol, hidrocodona e/ou anestesia
Atualmente tomando algum dos seguintes medicamentos:
- Inibidor do CYP3A4, como antibióticos macrólidos (por exemplo, eritromicina), agentes antifúngicos azólicos (por exemplo, cetoconazol) e inibidores de protease (por exemplo, ritonavir), que podem aumentar as concentrações plasmáticas de bitartarato de hidrocodona e acetaminofeno e prolongar as reações adversas dos opioides, e que pode causar depressão respiratória potencialmente fatal
- Depressores do SNC.
- Consome 3 ou mais bebidas alcoólicas todos os dias e/ou tem histórico de alcoolismo
- Histórico de abuso de drogas ou álcool
- História familiar de abuso de drogas ou álcool em um parente de primeiro grau
- Não teve mais do que uma receita de opioides preenchida nos últimos 12 meses
- Atualmente grávida ou amamentando
Os participantes também seriam excluídos devido a quaisquer critérios adicionais que colocariam o indivíduo em risco aumentado ou impediriam a adesão total do indivíduo ou a conclusão do estudo, que inclui:
- Participação prévia neste estudo
- Incapacidade ou recusa em fornecer consentimento informado
