Environ 11 millions d'Américains ont une myopie qui peut être corrigée avec des lunettes ou des lentilles de contact. Certaines de ces personnes peuvent également être candidates à la kératotomie radiaire (RK), une procédure qui vise à corriger ou à réduire la myopie par une intervention chirurgicale qui aplatit la courbure cornéenne.
La kératotomie a été réalisée pour la première fois par des chirurgiens en Europe et aux États-Unis à la fin des années 1800, et les principes optiques et mécaniques de base de l'opération ont été définis dans les années 1940 et 1950 par les médecins japonais T. Sato et K. Akiyama, qui utilisaient des incisions cornéennes postérieures. Les incisions postérieures ont endommagé la cornée et la procédure a été modifiée en Union soviétique par les médecins Fyodorov et V. Durnev pour inclure des incisions uniquement dans la cornée antérieure. Depuis son introduction aux États-Unis en 1978, de nombreux ophtalmologistes ont modifié la procédure en introduisant des améliorations techniques et chirurgicales telles que des méthodes ultrasonores pour mesurer l'épaisseur de la cornée et l'utilisation de couteaux micrométriques à lame de diamant pour faire les incisions.
Cependant, l'évaluation scientifique de la RK a pris du retard par rapport à l'intérêt croissant du public et des professionnels pour la procédure. En 1980, en réponse aux inquiétudes généralisées concernant l'innocuité et l'efficacité à long terme de la RK, un groupe de chirurgiens ophtalmologistes a approché le National Eye Institute avec une proposition d'essai clinique multicentrique qui évaluerait les avantages et les risques potentiels de cette procédure.
L'étude d'évaluation prospective de la kératotomie radiale, impliquant 435 patients et 99 patients pilotes, était un essai clinique conçu pour évaluer l'innocuité et l'efficacité à court et à long terme d'une technique de kératotomie radiale. La procédure a été évaluée en comparant l'erreur de réfraction et la vision non corrigée d'un patient avant et après la chirurgie. L'œil le plus myope a été opéré en premier. Les patients devaient attendre 1 an avant d'être opérés du deuxième œil.
La technique chirurgicale était standardisée, consistant en huit incisions radiales centrifuges faites manuellement avec un bistouri diamanté micrométrique. Le diamètre de la zone centrale, non coupée, claire a été déterminé par la réfraction cycloplégique équivalente sphérique préopératoire (-2,00 à -3,12 D = 4,0 mm ; -3,25 à -4,3 D = 3,5 mm ; -4,50 à -8,00 D = 3,0 mm) . La longueur de la lame, qui a déterminé la profondeur de l'incision, a été fixée à 100 % de la plus fine des quatre lectures peropératoires de l'épaisseur de la cornée par ultrasons prises de manière paracentrale aux méridiens 3, 6, 9 et 12 heures juste à l'extérieur du marque délimitant la zone dégagée. Les incisions ont été faites du bord de la marque de trépan à l'arcade vasculaire limbique et ont été espacées de manière équidistante autour de la cornée.
Les patients ont été examinés en préopératoire et après la chirurgie à 2 semaines, 3 mois, 6 mois, annuellement pendant 5 ans et à 10 ans. Des examens le matin et le soir du même jour ont été effectués à 3 mois, 1 an, 3 ans et 11 ans chez un sous-ensemble de patients pour tester les fluctuations diurnes de la vision et de la réfraction.
Les principales variables de résultat mesurées à chaque visite étaient l'acuité visuelle non corrigée et corrigée par lunettes et l'erreur de réfraction avec la pupille dilatée et non dilatée. La forme cornéenne a été mesurée par kératométrie centrale et photokératoscopie. La fonction endothéliale a été évaluée par microscopie spéculaire. Un examen au microscope à la lampe à fente a été effectué pour vérifier les complications des incisions. La sensibilité au contraste a été testée dans un sous-ensemble de patients. La motivation et la satisfaction des patients ont été étudiées avec des questionnaires psychométriques au départ, 1 an, 5-6 ans et 10 ans.
