ÉTUDE DE PHASE III DE LA RADIOTHÉRAPIE EXTERNE POST-OPÉRATOIRE DANS LE STADE PATHOLOGIQUE T3 N0 CARCINOME DE LA PROSTATE
JUSTIFICATION : La radiothérapie utilise des rayons X à haute énergie pour endommager les cellules tumorales et peut être un traitement efficace du cancer de la prostate.
OBJECTIF : Essai de phase III randomisé pour comparer la radiothérapie à l'absence de traitement supplémentaire dans le traitement des patients atteints d'un cancer de la prostate de stade III après une prostatectomie radicale.

Description détaillée
OBJECTIFS:
- Comparer les taux de récidive locale, la morbidité aiguë et tardive, la survie globale, la survie sans maladie et la survie liée au cancer des patients atteints d'un adénocarcinome de la prostate pT3 pN0 randomisés après une prostatectomie radicale à une irradiation pelvienne conventionnelle postopératoire (60 Gy) par rapport à l'absence de traitement jusqu'à rechute.
- Mieux définir les indications pathologiques sélectives de la radiothérapie chez les patients atteints de la maladie pT3 pN0.
APERÇU : Il s'agit d'une étude randomisée.
- Bras I : Les patients subissent une radiothérapie quotidienne, 5 jours par semaine, pendant 5 semaines, suivie d'une radiothérapie de rappel pendant 1 à 1,4 semaines.
- Bras II : Les patients sont observés. La rechute locale est traitée par radiothérapie pelvienne conventionnelle.
Les patients sont suivis tous les 3 mois pendant la première année postopératoire, tous les 6 mois jusqu'à la cinquième année, puis annuellement par la suite.
RECUL PROJETÉ : Un total de 1 000 patients seront comptabilisés pour cette étude dans un délai de 7,5 ans.

CARACTÉRISTIQUES DE LA MALADIE :
Adénocarcinome invasif de la prostate histologiquement documenté au stade pT3 pN0 après prostatectomie radicale
- Le stade préopératoire doit avoir été T0-3 N0 M0 sur la base d'un examen physique, d'une radiographie pulmonaire, d'une scintigraphie osseuse, d'une tomodensitométrie ou d'une IRM de l'ensemble du bassin et de l'abdomen et du PSA sérique
Au moins 1 des fonctionnalités suivantes doit être présente :
- Envahissement complet de la capsule (c.-à-d. perforation)
- Marges chirurgicales positives (microscopiques ou brutes)
- Invasion des vésicules séminales
- La radiothérapie doit débuter dans les 16 semaines suivant la chirurgie, après récupération de la fonction urinaire
