1. Fő célkitűzés: A szénhidrátbevitel változásának a glükóz kontrollra gyakorolt hatásának értékelése 1-es típusú cukorbetegségben szenvedő betegeknél.
- Elsődleges végpont: a 2 csoport közötti időbeli különbség (TIR).
- Másodlagos végpont:
1) a variációs koefficiens különbsége (CV), a glikémiás eltérések átlagos amplitúdója (MAGE), a glikémiás eltérések nagy amplitúdója (LAGE) a 2 csoport között; 2) a HbA1c,GA,1,5-anhidroglucit (1,5-AG) változásának különbsége az alapvonalhoz képest a két csoport között; 3) a hipoglikémiás események (%), a súlyos hipoglikémia és az éjszakai hipoglikémiás események előfordulási gyakoriságának változása a kiindulási értékhez képest a két csoport között; 4) az inzulinadag változásának különbsége (NE/kg/nap) a kiindulási értékhez képest a két csoport között.
2. Másodlagos célkitűzés: Az 1-es típusú cukorbetegségben szenvedő betegek vércukorszintjének javítását célzó diétás beavatkozás lehetséges mechanizmusának feltárása.
- Az étrendi beavatkozás hatásai az 1-es típusú cukorbetegek bél mikrokörnyezetére és mikroflórájára;
- Az étrendi beavatkozás hatása az 1-es típusú cukorbetegek immunfunkciójára;
- Az étrendi beavatkozás hatásai az 1-es típusú cukorbetegek metabolomikájára.

Bevételi kritériumok:
- Azok, akik vállalják, hogy részt vesznek a vizsgálatban, és aláírják a tájékozott beleegyezésüket;
- 1-es típusú diabetes mellitus diagnózisa (ADA2024);
- Életkor 18-65 év;
- Az exogén inzulinterápia (CSII) függvényében a kezelési terv 2 hónapon belül változatlan marad (az inzulin típusa nem változtatható, az adag a plazma glükózszintjének megfelelően módosítható);
- Testtömegindex (BMI) 18-24 kg/m2;
- HbA1c ≤9,5%;
- Véletlenszerű C-peptid ≥200 pmol/L.
Kizárási kritériumok:
- 1-es típusú diabetes mellitusban szenvedő nászutasok;
- terhes nők vagy terhességet tervező nők;
- Vegetáriánus betegek;
- Orális hipoglikémiás gyógyszereket (alfa-glükozidáz inhibitorok, DPP-IV gátlók stb.) használó betegek;
- Olyan betegek, akik 30 napon belül glükokortikoidokat szednek;
- Súlyos ételallergia anamnézisében;
- Akut szövődményekben szenvedő betegek, például DKA;
- Gastroparesisben, gyulladásos bélbetegségben és egyéb szövődményekben szenvedő betegek;
- Nagy albuminuriában és veseelégtelenségben szenvedő betegek;
- Nem kontrollált hyperthyreosisban és hypothyreosisban szenvedő betegek;
- Szívbetegség, szívkoszorúér-betegség és aritmia anamnézisében;
- Súlyos májműködési zavar (ALT vagy AST a normálérték felső határának 1,5-szerese);
- Rosszindulatú daganatok a kórelőzményében, kontrollálatlan egyéb immunrendszeri betegségek, kontrollálatlan fertőzések;
- Alkohollal való visszaélés, mentális zavarok vagy egyéb olyan állapotok, amelyek alkalmatlanok arra, hogy megfigyelőként szerepeljenek a drogteszteken;
- Bármilyen olyan betegségben szenvedő betegek, akik valószínűleg zavarják a vizsgálatban való részvételt vagy az értékelést.
