Impatto delle manifestazioni/comorbilità sistemiche sullo stato clinico, sulla prognosi e sull'utilizzo delle risorse sanitarie nei pazienti con BPCO
Lo scopo di questo studio è determinare la prevalenza e la gravità dei disturbi extrapolmonari e quantificare l'impatto delle manifestazioni d'organo extrapolmonari sulla morbilità e sulla mortalità dei pazienti con BPCO.
Inoltre i ricercatori valutano la relazione tra infiammazione sistemica e coinvolgimento degli organi.

Descrizione dettagliata
Le malattie croniche e le comorbilità hanno un impatto crescente sulla vita individuale e sull'uso dell'assistenza sanitaria, come esemplificato nella broncopneumopatia cronica ostruttiva (BPCO).
Attualmente mancano database indipendenti e completi che consentano una visione integrata.
Lo studio stabilirà per la prima volta un database longitudinale ampio e completo che copre un ampio pannello di comorbidità e marcatori di alterazioni sistemiche.
Questi dati avranno un valore unico nel rivelare fenotipi specifici della BPCO attraverso modelli e profili di rischio di comorbidità, consentendo una diagnosi più precisa e mirando alla terapia e un'efficiente allocazione delle risorse.

Criterio di inclusione:
- dai 40 anni in su
- diagnosi di BPCO (Criteri GOLD) o bronchite cronica
- consenso informato
- disponibile per visite di studio ripetute per 18 mesi
Criteri di esclusione:
- aver subito un intervento chirurgico al grande polmone (ad es. riduzione polmonare, trapianto polmonare)
- Riacutizzazione moderata o grave nelle ultime 4 settimane
- avere un tumore al polmone
- Incapace di camminare o di capire l'intenzione del progetto

Il BODE-Score comprende BMI, ostruzione delle vie aeree, dispnea e capacità di esercizio ed è un buon predittore di morte nei pazienti con BPCO.
La progressione è definita come una variazione del punteggio BODE di >=1 punto durante il periodo di osservazione.
