進行喉頭がん患者の治療における化学療法を伴うまたは伴わない放射線療法
喉頭を温存するための第 III 相試験: 導入化学療法および放射線療法 対 併用化学療法および放射線療法 対 放射線療法
理論的根拠: 化学療法で使用される薬剤は、さまざまな方法で腫瘍細胞の分裂を止め、増殖を停止したり死滅させたりします。 放射線療法では、高エネルギー X 線を使用して腫瘍細胞に損傷を与えます。 進行性喉頭がん患者の治療において、化学療法と放射線療法が放射線療法単独よりも有効であるかどうかはまだわかっていません。
目的: 進行喉頭癌患者の治療における化学療法ありまたはなしの放射線療法の有効性を比較するためのランダム化第 III 相試験。

目的: I. 第 III 相設定で、シスプラチン/フルオロウラシル (CDDP/5-FU) で治療したステージ III/IV の声門および声門上の扁平上皮癌患者の喉頭機能温存による全生存期間と無病生存期間を比較する。 ) に続いて、放射線療法と併用放射線療法 + CDDP と放射線療法単独の比較が続きます。 II. 化学療法の完了後、放射線療法の開始前の腫瘍反応を、放射線療法と同時化学療法の完了後の腫瘍反応と比較します。 Ⅲ. これらの治療法と再発のパターン (局所的および局所的な再発および遠隔転移) を比較します。 IV. これら 3 つのレジメンで治療された患者における二次原発腫瘍の発生率を比較します。 V. これら 3 つのレジメンの急性および慢性の副作用を比較します。 VI. これらのレジメンによる治療後に頸部解剖および/または喉頭温存手術で経験した罹患率を比較してください。 VII. 喉頭温存患者と温存喉頭切除術が必要な患者の生活の質を比較します。 Ⅷ. 放射線療法のみを受けている患者と化学療法も受けている患者の生活の質を比較します。
概要: ランダム化研究。 どの腕の患者でも、臨床的に N+ と診断された患者は、放射線治療終了後に頸部郭清を受けます。 アーム I: 2 剤併用化学療法とそれに続く放射線療法。 シスプラチン、CDDP、NSC-119875;フルオロウラシル、5-FU、NSC-19893;続いて、1.25 ~ 6 MV の光子エネルギー、8 ~ 17 MeV の電子エネルギー、または Co60 の線形加速器を使用した局所照射が行われます。 アーム II: 放射線療法と単剤化学療法/放射線増感。 アーム I と同様の装置を使用した局所照射。加えてCDDP。 アーム III: 放射線療法。 アーム I と同様の装置を使用した局所照射。
予測される獲得数: 約 3 年間で 546 人の患者 (182 人/腕) が登録される予定です。 137 人の患者が 2 年間の追跡調査を完了した後、いずれかの治療群が喉頭切除なしの生存率で明らかに劣っている場合、その治療群はさらに増加するために閉鎖されます。 410人の患者が2年間の追跡調査を完了した後、2回目の中間解析が実施される。

説明
疾患の特徴: 生検で証明された、これまで未治療の声門および声門上喉頭の扁平上皮癌で、通常は喉頭全摘出術が必要である ステージ III/IV の疾患 (T1 および M1 を除く) 入院後 4 週間以内に内視鏡による腫瘍の病期分類が必要である 腫瘍は切除可能であるとみなされる必要があるT4 疾患は以下に限定されます: 舌根部への浸潤が最大 1 cm CT 上で疑わしい軟骨浸潤 (臨床的には T3) 測定可能な疾患が必要 同時原発腫瘍がない 声門下腫瘍がない
患者の特徴: 年齢: 18 歳以上 パフォーマンスステータス: カルノフスキー 60%-100% 造血: (入国後 3 週間以内に取得) WBC 少なくとも 3,500 血小板少なくとも 100,000 肝臓: 特定されていない 腎臓: (入国後 2 週間以内に取得) クレアチニンクリアランスが少なくとも 50 mL/min (測定または計算) 血清カルシウムは正常 心血管: すべてのプロトコル治療に耐えられる適切な状態 肺: すべてのプロトコル治療に耐えられる適切な状態 その他: すべてのプロトコル治療に耐えられる適切な栄養状態 指示および指示に従うのに十分な精神状態予約を守る 非黒色腫性皮膚がん以外の二次悪性腫瘍がないこと(以前のがんの治療後、少なくとも 3 年間無病であった患者は、プロトコール委員長の裁量により適格となる場合がある) 妊娠可能な女性には妊娠検査が陰性であることが求められる 効果的な避妊が必要である妊娠可能な女性
