術中の肺吸引は、死に至る可能性があり、病的状態につながる可能性があります。 さらに、誤嚥率の信頼できる推定値は不確実です。 一部には、このあいまいさは、将来のデータの欠如に関連しています。 関連する研究は、遡及的なチャートのレビューまたは自発的な報告データベースからの結果です。 さらに、誤嚥の診断は不正確になる可能性があります。 気管気管支樹からの胆汁または粒子状物質の吸引がある場合、または内視鏡による視覚化がある場合、所見は確実です。 しかし、術中または術後に新たな胸部 X 線浸潤像が発生し、それに伴う頻呼吸、低酸素症、喘鳴、または人工呼吸器の気道圧の変化がある場合、診断は推定に基づくものです。
全身気管内麻酔を受けるほとんどの患者は仰臥位または水平位です。 しかし、文献からの証拠は、人工呼吸器を使用している患者の仰臥位は、誤嚥および人工呼吸器関連肺炎 (VAP) のリスクであることを示しています。 集中治療室 (ICU) の人工呼吸中、Institute for Healthcare Improvement は、肺吸引と VAP を防ぐためにベッドの頭を高くすることを推奨しています。 他の調査では、水平位置と術中誤嚥との間に深い関係があることが示されています。 カフ付き気管内チューブの存在にもかかわらず誤嚥が発生するという実質的な手術室、ICU、および動物調査の証拠があります。 同様に、このグループによる以前の研究では、30% の周術期低酸素血症率が示されており、これは水平姿勢と有意に関連していました。 術後の入院期間は、低酸素血症がない場合と比較して、低酸素血症がある場合は 2 日長くなり (p < 0.0001)、これは 2 か月の研究で合計 300 日追加されたことを表しています。
このレトロスペクティブ研究の目的は、麻酔科または手術室の看護スタッフのいずれかによって不適切であると見なされない限り、手術患者のルーチンとして 10 度の逆トレンデレンブルグ体位という最近の方針変更を採用した後に調査を繰り返すことです。

- 18歳以上
- Glasgow Coma Scaleスコア≧13(気管挿管前)
- 米国麻酔学会分類 I-IV
- 術前の肺の安定性
除外基準:
- 救急外来到着前の気管挿管
- 心臓および胸部外科患者
