- 患者は、計画された来院、治療スケジュール、臨床検査、および研究のその他の要件に喜んで従うことができなければなりません。
患者は次の種類の腫瘍を提示する必要があります。
- 第 I 相パートでは、あらゆる種類の腫瘍を持つ患者が対象となります。
- フェーズ II 部分では、2 つのコホートが確立されます。
私。コホート A には、プログラム細胞死タンパク質 1 (PD1)/プログラム細胞死リガンド 1 (PDL1) 阻害に対する感受性が既知の腫瘍タイプを呈する患者が含まれます (例: マイクロサテライト不安定性を示す黒色腫、腎がん、肺がん、尿路上皮がん、結腸直腸がん) MSI)、…)。 これらの患者は、PD1/PDL1 遮断の経験がない必要があります。
ii.コホートBには、以前のPD1/PDL1遮断後に進行したPD1/PDL1感受性腫瘍を有する患者(例:黒色腫、NSCLC、腎癌、膀胱癌など)が含まれる。 以前のPD1/PDL1遮断と研究への参加の間に追加の治療を投与することもできますが、直前に投与する場合は、両方の治療の間に最低4週間の休薬期間を観察する必要があります。
- 患者は治験責任医師の基準に従って標準治療を受けているか、標準治療を受ける資格がない必要があります。
- 患者は、連続した腫瘍内治療と生検を実施できる腫瘍病変を少なくとも 1 つ提示する必要があります。
- RECIST基準に従って測定可能な疾患。 測定可能な病変は、腫瘍内ウレルマブで治療された病変とは異なっていなければなりません。
- 他の対象基準が満たされている限り、以前の治療ラインに制限はありません。
- Eastern Cooperative Oncology Group (ECOG) のパフォーマンス ステータスは 0 ~ 1。
- 平均余命は12週間を超えます。
適切な臓器機能は次のように定義されます。
- 骨髄予備力: 白血球 (WBC): >=2000/mm3 絶対好中球数 (ANC) >=1500x 109/L;血小板数 >=100000/mm3 100 x 109/L;ヘモグロビン >=9.0 g/dL)。
- 肝臓: ビリルビンが正常上限値 (ULN) の 1.5 倍未満、アスパラギン酸アミノトランスフェラーゼ (AST) およびアラニン アミノトランスフェラーゼ (ALT) が 3.0 x ULN 未満 (ギルバート症候群患者の場合、BR< 3 x ULN)。
- 腎臓: クレアチニン < 1.5 x ULN、または Cokcroft-Gault 式を使用した推定クレアチニン クリアランス > 40 ml/分。
- 妊娠の可能性のある女性(WOCBP、すなわち、妊孕性があり、初経後、永久的に不妊でない限り閉経後になるまで)は、妊娠を回避するための非常に効果的な方法(すなわち、排卵阻害に関連するエストロゲンとプロゲストゲンの併用(経口、膣内、または経皮))を使用しなければなりません。 )、排卵阻害を伴うプロゲストゲンのみのホルモン避妊(経口、注射、または埋め込み型)、子宮内器具、子宮内ホルモン放出システム、両側卵管閉塞、精管切除されたパートナー、または23週間の性的禁欲(30日間にニボルマブに必要な期間を加えたもの)ウレルマブは治験薬の最後の投与後に 5 つの半減期を経ます。
- WOCBP では、治療開始前 24 時間以内に血清または尿の妊娠検査 (最低感度 25 IU/L または同等の HCG 単位) が陰性でなければなりません。
- WOCBP で性行為を行う男性は、失敗率が年間 1% 未満の避妊方法を使用する必要があります。 ニボルマブの投与を受けており、WOCBPで性的活動を行っている男性には、治験薬の最後の投与後31週間は避妊を厳守するよう指導される。 妊娠の可能性がない女性(すなわち、閉経後または外科的に不妊である女性、および無精子症の男性)は避妊を必要としません。
- 患者は18歳以上である必要があります。
除外基準:
- 治験責任医師の意見で、治験への参加や治験薬の投与に関連するリスクを高めたり、計画された治療を受ける被験者の能力を損なったり、治験結果の解釈を妨げたりする可能性がある、重篤なまたは制御されていない医学的障害。 肝臓に影響を与える症状には特別な注意が必要です。
- 活動性の自己免疫疾患、既知の自己免疫疾患、または自己免疫疾患が疑われる対象。 白斑、I型糖尿病、ホルモン補充のみを必要とする自己免疫状態による残存甲状腺機能低下症、全身治療を必要としない乾癬、または外部誘因がない場合に再発が予想されない状態を有する被験者は、登録が許可されます。
- 放射線療法を含む他の以前の治療法の休薬期間は、関連する毒性の解消に応じて研究者によって決定されます。 限定照射野緩和放射線療法では、ウォッシュアウト期間は必要ありません。 以前の治療が PD1/PDL1 阻害である場合、両方の治療の間に最低 4 週間の休薬期間を置く必要があります。
- 治験薬投与後14日以内にコルチコステロイド(プレドニゾン1日あたり10mg以上)または他の免疫抑制剤による全身治療が必要な状態にある患者は除外されるべきである。 活動性自己免疫疾患がない場合、吸入または局所ステロイドおよび副腎置換用量が1日あたりプレドニゾン相当量10 mgを超える場合は許可されます。
- 結果の解釈を妨げる可能性がある活動性脳転移。 転移が制御されている被験者は登録が許可されます。 制御された脳転移とは、放射線治療および/または外科的治療後、少なくとも 4 週間にわたって X 線検査で進行がみられないことと定義されます。
- 妊娠中または授乳中の患者。
- ヒト免疫不全ウイルス (HIV) または既知の後天性免疫不全症候群 (AIDS) の検査結果が陽性となった既知の病歴。 定期的なテストは必要ありません。
- B型肝炎ウイルス表面抗原(HBV sAg)またはC型肝炎ウイルスリボ核酸(HCV RNA)の検査結果が陽性で、活動性または慢性感染を示します。 C型肝炎抗体が陽性で、ポリメラーゼ連鎖反応(PCR)による定量的C型肝炎が陰性である被験者が対象となります。 回復したA型肝炎ウイルス感染歴は除外基準ではありません。
- 薬物成分を研究するためのアレルギーの病歴、またはモノクローナル抗体に対する重度の過敏反応の病歴。
- 非自発的に収監されている囚人または対象者、または精神疾患または身体疾患(感染症など)の治療のために強制的に拘留されている囚人または対象者。
- 研究者の意見では、研究の結果を妨げる可能性があると研究者が判断した、併発または以前の悪性腫瘍。
- 既知の薬物またはアルコール乱用。
