PAH/CTEPH では、右心の大きさと機能が予後にとって最も重要です。 心エコー検査によって測定された RV パフォーマンスと拡大した RA 領域は、PAH の独立した予後因子であることが示されています。 最近、レトロスペクティブな単一施設の研究で、リオシグアト治療が、ベースラインと比較して 3、6、および 12 か月後に RV および RA 領域の有意な減少と関連していたことが示されました。 RA 面積は 12 か月後に有意に減少し、リオシグアト療法の 6 か月および 12 か月後に、三尖弁輪面収縮期可動域 (TAPSE) で評価された RV 収縮機能が改善されました。 この結果は、最近の遡及的多施設研究によって確認されました。 したがって、リオシグアトが右心臓のサイズと機能に有益な効果をもたらすと仮定することは合理的です。
この研究の主要な有効性エンドポイントは、ベースラインから 24 週間までの RV および RA 領域の変化です。 治療が開始され、収縮期血圧と忍容性に応じて個別に調整されます。 時期尚早に投薬を中止した患者は、研究評価を継続し、プロトコルに概説されているように研究訪問を行うよう求められます。
健康診断は、病歴、身体検査、心電図(ECG)、血液ガス分析、肺機能検査、臨床検査(NT-proBNPを含む)、安静時心エコー検査、およびPHセンターの臨床実践に基づく右心カテーテル検査(RHC)で構成されます.
データ収集の予測期間は、24 週間の研究期間と約 30±7 日間のフォローアップ期間で構成されます。
最後の患者が24週間の観察期間を終了したときに、すべての患者の結果(生存および無移植生存)が評価されます。

包含基準:
- 登録時の年齢が 18 歳以上。
- 平均肺動脈圧(mPAP)が 20 mmHg を超え、肺血管抵抗(PVR)が 2 ウッド ユニット(WU)以上、肺動脈楔入圧(PAWP)が 15 mmHg 以下の症候性 PAH の男性および女性患者(グループ I / Nice Clinical -肺高血圧症の分類)またはCTEPH(グループIV /肺高血圧症のナイス臨床分類)は、完全な抗凝固療法の開始から少なくとも3か月後に測定され、手術不能と定義され、mPAP> 20 mmHgおよびPVR ≥ 2 WU、PAWP ≤ 15 mmHg;または肺動脈内膜切除術後の持続性または再発性の PH を伴う (mPAP > 20 mmHg および PVR ≥ 2 WU、PAWP ≤ 15 mmHg 手術後少なくとも 6 ヶ月で測定 (acc. - 経口抗凝固薬、利尿薬、ジギタリス、カルシウム チャネル遮断薬、酸素補給など、PH の治療にも使用できる非特異的な治療は許可されます。 ただし、抗凝固薬による治療(適応がある場合)は、PAH 1 の患者では来院の少なくとも 1 か月前に開始されている必要があります。
- RHC の結果は、スクリーニング時に 6 か月を超えてはならず (ベースライン値と見なされます)、標準化された条件下で参加センターで測定されたものでなければなりません (研究固有の Swan Ganz カテーテル法マニュアルを参照してください)。 それぞれの測定が患者の定期的な診断精密検査との関連で実行されていない場合は、研究前段階 (患者がインフォームド コンセントに署名した後) に研究の一部として実行する必要があります。
- 50歳以上の閉経後の女性として定義される出産の可能性のない女性、両側卵管結紮の女性、両側卵巣摘出術の女性、および子宮摘出術の女性を研究に含めることができます。

心血管除外:

他の:リオシグアト
リオシグアト(1mg、1.5mg、2mg、2.5mgを1日3回)は、研究の開始時に1.0mgを1日3回から開始します。
用量は、8 週間後に 1 日 3 回、最大用量 2.5 mg まで個別に増量されます。
研究薬は食事の有無にかかわらず経口的に投与されます。
錠剤は 1 日 3 回、約 6 ~ 8 時間の間隔をあけて服用する必要があります。

治療が開始され、収縮期血圧と忍容性に応じて個別に調整されます。
滴定段階では、各患者は自宅で末梢収縮期血圧と心拍数を 1 日 3 回測定し、その値を患者日誌に記録するよう求められます。
結果は、各訪問/電話訪問中に研究者によって調べられます。
各用量の摂取前にトラフで測定した収縮期血圧が 95 mmHg 以上であり、患者に低血圧の徴候や症状がない場合、治験薬の用量は 2 週間ごとに +0.5 mg tid ずつ最大耐量まで滴定されます (最大許容用量:2.5 mg tid)。
滴定期間の後、低血圧の徴候または症状に応じて血圧を測定する必要があります。
維持用量:低血圧の徴候や症状が発生しない限り、確立された個人用量を維持する必要があります。
