Effecten van Riociguat op rechter VEntriculaire grootte en functie in PAH en CTEPH (RIVERII)
Een open-label, prospectief onderzoek in één centrum naar de effecten van Riociguat op de grootte en functie van de rechterventrikel bij pulmonale arteriële hypertensie en chronische trombo-embolische pulmonale hypertensie

De grootte en functie van het rechter hart zijn van het grootste prognostische belang bij PAH/CTEPH. RV-prestaties gemeten door echocardiografie en vergroot RA-gebied blijken onafhankelijke prognostische factoren te zijn bij PAH. Onlangs heeft een retrospectief onderzoek in één centrum aangetoond dat behandeling met riociguat gepaard ging met een significante vermindering van het RV- en RA-gebied na 3, 6 en 12 maanden in vergelijking met de uitgangswaarde. Het RA-gebied nam significant af na 12 maanden en de systolische functie van de RV, beoordeeld met tricuspidalis annulair vlak systolische excursie (TAPSE), verbeterde na 6 en 12 maanden behandeling met riociguat. De resultaten werden bevestigd door een recente retrospectieve multicenter studie. Het is daarom redelijk om aan te nemen dat riociguat een gunstig effect heeft op de grootte en functie van het rechter hart.
Het primaire werkzaamheidseindpunt in dit onderzoek is de verandering in RV- en RA-gebied vanaf de uitgangswaarde tot 24 weken. De behandeling wordt gestart en individueel aangepast op basis van de systolische bloeddruk en verdraagbaarheid. Patiënten die de medicatie voortijdig stopzetten, wordt gevraagd om door te gaan met onderzoeksbeoordelingen en studiebezoeken af te leggen zoals beschreven in het protocol.
Medisch onderzoek omvat anamnese, lichamelijk onderzoek, elektrocardiogram (ECG), bloedgasanalyses, longfunctietesten, laboratoriumonderzoek (inclusief NT-proBNP), echocardiografie in rust en rechterhartkatheterisatie (RHC) volgens de klinische praktijk van het PH-centrum .
De prospectieve periode van gegevensverzameling omvat een studieperiode van 24 weken en een follow-upfase van ongeveer 30 ± 7 dagen.
Het resultaat (overleving en transplantatievrije overleving) van alle patiënten zal worden beoordeeld wanneer de laatste patiënt zijn/haar observatieperiode van 24 weken heeft beëindigd.

Beschrijving
Inclusiecriteria:
- ≥18 jaar oud op het moment van opname.
- Mannelijke en vrouwelijke patiënten met symptomatische PAH met een gemiddelde pulmonale arteriële druk (mPAP) >20 mmHg en pulmonale vasculaire weerstand (PVR) ≥2 Wood Units (WU), pulmonale arteriële wigdruk (PAWP) ≤15 mmHg (Groep I / Nice Clinical Classificatie van pulmonale hypertensie) of CTEPH (Groep IV / Nice klinische classificatie van pulmonale hypertensie) gedefinieerd als inoperabel gemeten ten minste 3 maanden na het begin van volledige antistolling en mPAP >20 mmHg en PVR ≥2 WU, PAWP ≤15 mmHg; of met aanhoudende of terugkerende PH na pulmonale endarteriëctomie (mPAP >20 mmHg en PVR ≥2 WU, PAWP ≤15 mmHg gemeten ten minste 6 maanden na de operatie (acc. aan Simonneau et al. 2018).
- Niet eerder behandelde patiënten (met betrekking tot PAH-specifieke medicatie) en patiënten die zijn voorbehandeld met een endothelinereceptorantagonist of een prostacycline-analoog, maximaal 2 maanden voorbehandeld vóór screening (volgens voorafgaande combinatiebehandeling).*
- *Voorbehandelde patiënten moeten stabiel zijn op endothelinereceptorantagonisten of prostacyclinebehandeling gedurende ten minste twee weken voorafgaand aan bezoek 1. "Stabiel" wordt gedefinieerd als geen verandering in het type endothelinereceptorantagonisten of prostacycline-analoog en de respectieve dagelijkse dosis.
- Een patiënt kan ook worden ingeschreven als een aanhoudende behandeling met fosfodiësterase type 5 (PDE-5)-remmers (voorbehandeld gedurende maximaal 2 maanden voorafgaand aan de screening) met of zonder combinatiebehandeling met een endothelinereceptorantagonist of prostacycline-analoog moet worden overgezet riociguat volgens klinische indicatie, met name wanneer het risicoprofiel van de patiënt in de middelmatige risicogroep bleef ondanks adequate initiële behandeling inclusief PDE5i (gedefinieerd als ten minste 3 van de volgende parameters: klinische tekenen van progressie, aanhoudende WHO-FC III, 6MWD tussen 165 -440m, piek V02 11-15ml/min/kg (35-65% voorspeld), NTproBNP 300-1400 ng/l, RA-gebied 18-26cm2,RAP 8-14mmHg, CI 2,0-2,4 l/ min) of in geval van PDE5i-intolerantie. Elke beslissing om over te stappen wordt genomen door de clinici tijdens een regelmatig klinisch vervolgbezoek.
- Niet-specifieke behandelingen die ook voor de behandeling van PH kunnen worden gebruikt, zoals orale antistollingsmiddelen, diuretica, digitalis, calciumantagonisten of zuurstofsuppletie zijn toegestaan. Behandeling met antistollingsmiddelen (indien geïndiceerd) moet echter minimaal 1 maand voor bezoek zijn gestart bij patiënten met PAH 1.
- RHC-resultaten mogen bij de screening niet ouder zijn dan 6 maanden (worden beschouwd als basiswaarden) en moeten in het deelnemende centrum onder gestandaardiseerde omstandigheden zijn gemeten (raadpleeg de studiespecifieke Swan Ganz-katheterisatiehandleiding). Als de respectieve metingen niet zijn uitgevoerd in de context van het reguliere diagnostische werk van de patiënt, moeten ze worden uitgevoerd als onderdeel van het onderzoek tijdens de pre-studiefase (nadat de patiënt de geïnformeerde toestemming heeft ondertekend).
- Vrouwen zonder vruchtbare leeftijd, gedefinieerd als postmenopauzale vrouwen van 50 jaar of ouder, vrouwen met bilaterale afbinding van de eileiders, vrouwen met bilaterale ovariëctomie en vrouwen met hysterectomie kunnen in het onderzoek worden opgenomen.
- Vrouwen in de vruchtbare leeftijd kunnen alleen in het onderzoek worden opgenomen als al het volgende van toepassing is (zie hieronder): Negatieve serumzwangerschapstest bij screening en een negatieve urinezwangerschapstest bij aanvang van de studie (bezoek 1). B. Overeenkomst om maandelijkse urine-zwangerschapstesten uit te voeren tijdens de studie en tot ten minste 30 dagen na stopzetting van de studiebehandeling. Deze tests moeten door de patiënt thuis worden uitgevoerd. C. Overeenkomst om het anticonceptieschema te volgen zoals gespecificeerd vanaf de screening tot ten minste 30 dagen na stopzetting van de studiebehandeling.
- Patiënten die instructies kunnen begrijpen en opvolgen en die de gehele periode aan het onderzoek kunnen deelnemen.
- Patiënten moeten hun schriftelijke geïnformeerde toestemming hebben gegeven om aan het onderzoek deel te nemen nadat ze vooraf voldoende informatie hebben ontvangen en voorafgaand aan eventuele onderzoeksspecifieke procedures.
Uitsluitingscriteria:
- Zwangere vrouwen, of vrouwen die borstvoeding geven, of vrouwen die zwanger kunnen worden en niet in staat of bereid zijn om te voldoen aan de hierboven beschreven voorgeschreven anticonceptiemethodes.
- Patiënten met PH-specifieke behandeling <2 maanden voor screening.
- Patiënten met een medische stoornis, aandoening of een dergelijke voorgeschiedenis die naar de mening van de onderzoeker het vermogen van de patiënt om deel te nemen aan of het voltooien van dit onderzoek zou belemmeren.
- Patiënten met onderliggende medische aandoeningen met een verwachte levensverwachting van minder dan 2 jaar (bijv. actieve kankerziekte met gelokaliseerde en/of gemetastaseerde tumormassa).
- Patiënten met een voorgeschiedenis van ernstige of meerdere geneesmiddelenallergieën
- Patiënten met overgevoeligheid voor het onderzoeksgeneesmiddel of voor één van de hulpstoffen.
- Patiënten die geen geldige 6MWD-test kunnen uitvoeren (bijv. orthopedische ziekte, perifere arteriële occlusieve ziekte, die het vermogen van de patiënt om te lopen aantast).
De volgende specifieke medicijnen voor gelijktijdige behandeling van PH of medicijnen die een farmacodynamische interactie met het onderzoeksgeneesmiddel kunnen uitoefenen, zijn niet toegestaan:
- Parenterale prostacycline-analogen
- Specifieke fosfodiësteraseremmers (bijv. sildenafil of tadalafil): kan worden overgezet op riociguat, maar mag niet naast het onderzoeksgeneesmiddel worden gegeven
- of niet-specifieke fosfodiësteraseremmers (bijv. dipyridamol, theofylline)
- GEEN donoren (bijv. Nitraten)
Uitsluitingen voor longziekten
- Matige tot ernstige bronchiale astma of COPD (geforceerd expiratoir volume <60% voorspeld) of ernstige restrictieve longziekte (totale longcapaciteit < 70% voorspeld) en/of gedefinieerd alsof computertomografie met hoge resolutie <20% parenchymale longziekte vertoont.
- Ernstige aangeboren afwijkingen van de longen, thorax en middenrif.
- Klinisch of radiologisch bewijs van pulmonale-veno-occlusieve ziekte (PVOD) of pulmonale capillaire hemangiomatose (PCH) of PH en idiopathische interstitiële pneumonie (PH-IIP)
Cardiovasculaire uitsluitingen:
- Ongecontroleerde arteriële hypertensie (systolische bloeddruk >180 mmHg en/of diastolische bloeddruk >110 mmHg).
- Systolische bloeddruk <95 mmHg.
- Linkerhartfalen met een ejectiefractie van minder dan 40%.
- Pulmonale veneuze hypertensie met pulmonale arteriële wigdruk >15 mmHg.
- Hypertrofische obstructieve cardiomyopathie.
- Ernstige bewezen of vermoede coronaire hartziekte volgens de mening van de onderzoeker (patiënten met Angina Classificatie klasse 2-4 van de Canadian Cardiovascular Society, en/of die nitraten nodig hebben, en/of een myocardinfarct in de laatste 3 maanden vóór bezoek 1).
- Klinisch bewijs van symptomatische atherosclerotische ziekte (bijv. perifere arteriële ziekte met verminderde loopafstand, voorgeschiedenis van een beroerte met aanhoudende neurologische uitval enz.).
Uitsluitingen gerelateerd aan stoornissen in orgaanfunctie:
a) Klinisch relevante leverdisfunctie aangegeven door: i. bilirubine >2 maal bovengrens normaal ii. en/of levertransaminasen >3 maal bovengrens normaal iii. en/of tekenen van ernstige leverinsufficiëntie (bijv. gestoorde albuminesynthese met een albumine < 32 g/l, hepatische encefalopathie > graad 1a: West Haven Criteria of Altered Mental Status In Hepatic Encephalopathy) b) Nierinsufficiëntie (glomerulaire filtratiesnelheid < 30 ml/min b.v. berekend op basis van de Cockcroft-formule).

Ander: Riociguat
Riociguat (1 mg, 1,5 mg, 2 mg en 2,5 mg driemaal daags) beginnend met 1,0 mg driemaal daags aan het begin van het onderzoek.
De dosering zal na 8 weken individueel worden verhoogd tot een maximale dosering van 2,5 mg driemaal daags.
De onderzoeksmedicatie wordt oraal toegediend, met of zonder voedsel.
Tabletten moeten driemaal daags worden ingenomen met een tussenpoos van ongeveer 6 tot 8 uur.

De behandeling wordt gestart en individueel aangepast op basis van de systolische bloeddruk en verdraagbaarheid.
Tijdens de titratiefase wordt aan elke patiënt gevraagd om driemaal per dag thuis de perifere systolische bloeddruk en de hartslag te meten en de waarden in het patiëntendagboek te noteren.
De resultaten worden bij elk bezoek/telefonisch bezoek door de onderzoeker bekeken.
Op voorwaarde dat de systolische bloeddruk ≥ 95 mmHg is, gemeten bij de dalwaarde vóór inname van elke dosis en de patiënt geen tekenen of symptomen van hypotensie heeft, zal de dosis studiemedicatie elke 2 weken worden getitreerd met +0,5 mg driemaal daags tot de maximaal getolereerde dosering ( maximaal toegestane dosis: van 2,5 mg driemaal daags).
Na de titratieperiode moet de bloeddruk worden gemeten op tekenen of symptomen van hypotensie.
Onderhoudsdosis: De vastgestelde individuele dosis moet worden gehandhaafd, tenzij tekenen en symptomen van hypotensie optreden.
