El tamaño y la función del corazón derecho son de suma importancia pronóstica en la HAP/HPTEC. Se ha demostrado que el rendimiento del VD medido por ecocardiografía y el área agrandada de la AR son factores pronósticos independientes en la PAH. Recientemente, un estudio retrospectivo de un solo centro ha demostrado que el tratamiento con riociguat se asoció con una reducción significativa del área del VD y la AR después de 3, 6 y 12 meses en comparación con el valor inicial. El área de la AR disminuyó significativamente después de 12 meses y la función sistólica del VD evaluada con la excursión sistólica del plano anular tricuspídeo (TAPSE) mejoró después de 6 y 12 meses de tratamiento con riociguat. Los resultados fueron confirmados por un estudio multicéntrico retrospectivo reciente. Por lo tanto, es razonable suponer un efecto beneficioso de riociguat sobre el tamaño y la función del corazón derecho.
El criterio principal de valoración de la eficacia en este estudio es el cambio en el área del VD y la AR desde el inicio hasta las 24 semanas. El tratamiento se iniciará y ajustará individualmente de acuerdo con la presión arterial sistólica y la tolerabilidad. A los pacientes que interrumpan prematuramente la medicación se les pedirá que continúen con las evaluaciones del estudio y realicen las visitas del estudio como se describe en el protocolo.
Los exámenes médicos comprenden la historia clínica, el examen físico, el electrocardiograma (ECG), los análisis de gases en sangre, las pruebas de función pulmonar, las pruebas de laboratorio (incluido NT-proBNP), la ecocardiografía en reposo y el cateterismo cardíaco derecho (CCD) de acuerdo con la práctica clínica del centro de HP. .
El período prospectivo de recopilación de datos comprende un período de estudio de 24 semanas y una fase de seguimiento de aproximadamente 30 ± 7 días.
El resultado (supervivencia y supervivencia sin trasplante) de todos los pacientes se evaluará cuando el último paciente haya finalizado su período de observación de 24 semanas.

Descripción
Criterios de inclusión:
- ≥18 años de edad al momento de la inclusión.
- Pacientes masculinos y femeninos con HAP sintomática con presión arterial pulmonar media (PAPm) >20 mmHg y resistencia vascular pulmonar (PVR) ≥2 Unidades Wood (WU), presión arterial pulmonar enclavada (PAWP) ≤15 mmHg (Grupo I / Nice Clinical Clasificación de Hipertensión Pulmonar) o HPTEC (Grupo IV / Clasificación Clínica de Niza de Hipertensión Pulmonar) definida como inoperable medida al menos 3 meses después del inicio de la anticoagulación completa y mPAP >20 mmHg y PVR ≥2 WU, PAWP ≤15 mmHg; o con HP persistente o recurrente después de la endarterectomía pulmonar (mPAP >20 mmHg y PVR ≥2 WU, PAWP ≤15 mmHg medidos al menos 6 meses después de la cirugía (acc. a Simonneau et al. 2018).
- Pacientes sin tratamiento previo (con respecto a la medicación específica para la HAP) y pacientes pretratados con un antagonista del receptor de la endotelina o un análogo de la prostaciclina, pretratados durante 2 meses como máximo antes de la selección (según el tratamiento combinado inicial).*
- *Los pacientes pretratados deben estar estables con el tratamiento con antagonistas de los receptores de endotelina o prostaciclina durante al menos dos semanas antes de la Visita 1. "Estable" se define como ningún cambio en el tipo de antagonistas de los receptores de endotelina o análogo de prostaciclina y la dosis diaria respectiva.
- También se puede inscribir a un paciente si se va a cambiar a un tratamiento persistente con un inhibidor de la fosfodiesterasa tipo 5 (PDE-5) (pretratado durante 2 meses antes de la selección como máximo) con o sin tratamiento combinado con un antagonista del receptor de la endotelina o un análogo de la prostaciclina. riociguat por indicación clínica, particularmente cuando el perfil de riesgo del paciente permanecía en el grupo de riesgo intermedio a pesar de un tratamiento inicial adecuado que incluyera PDE5i (definido como al menos 3 de los siguientes parámetros: signos clínicos de progresión, WHO-FC III persistente, 6MWD entre 165 -440m, VO2 pico 11-15ml/min/kg (35-65% del teórico), NTproBNP 300-1400 ng/l, área RA 18-26cm2,RAP 8-14mmHg, IC 2,0-2,4 l/ min) o en caso de intolerancia a PDE5i. Cualquier decisión de cambiar será tomada por los médicos en una visita de seguimiento clínico regular.
- Se permiten tratamientos inespecíficos que también pueden usarse para el tratamiento de la HP, como anticoagulantes orales, diuréticos, digitálicos, bloqueadores de los canales de calcio o suplementos de oxígeno. Sin embargo, el tratamiento con anticoagulantes (si está indicado) debe haberse iniciado al menos 1 mes antes de la visita en pacientes con HAP 1.
- Los resultados del RHC no deben tener más de 6 meses en el momento de la selección (se considerarán como valores de referencia) y deben haberse medido en el centro participante en condiciones estandarizadas (consulte el manual de cateterismo Swan Ganz específico del estudio). Si las mediciones respectivas no se han realizado en el contexto de la evaluación diagnóstica habitual del paciente, deben realizarse como parte del estudio durante la fase previa al estudio (después de que el paciente haya firmado el consentimiento informado).
- Pueden incluirse en el estudio mujeres sin potencial fértil definidas como mujeres posmenopáusicas de 50 años o más, mujeres con ligadura de trompas bilateral, mujeres con ovariectomía bilateral y mujeres con histerectomía.
- Las mujeres en edad fértil solo pueden incluirse en el estudio si se aplica todo lo siguiente (enumerado a continuación): a. Prueba de embarazo en suero negativa en la selección y prueba de embarazo en orina negativa al inicio del estudio (visita 1). b. Acuerdo para realizar pruebas de embarazo en orina mensuales durante el estudio y hasta al menos 30 días después de la interrupción del tratamiento del estudio. Estas pruebas deben ser realizadas por el paciente en su domicilio. C. Acuerdo para seguir el esquema de anticoncepción como se especifica desde la selección hasta al menos 30 días después de la interrupción del tratamiento del estudio.
- Pacientes que puedan comprender y seguir las instrucciones y que puedan participar en el estudio durante todo el período.
- Los pacientes deben haber dado su consentimiento informado por escrito para participar en el estudio después de haber recibido información previa adecuada y antes de cualquier procedimiento específico del estudio.
Criterio de exclusión:
- Mujeres embarazadas, mujeres en período de lactancia o mujeres en edad fértil que no pueden o no desean cumplir con los métodos anticonceptivos obligatorios del estudio especificados anteriormente.
- Pacientes con tratamiento específico para HP <2 meses antes de la selección.
- Pacientes con un trastorno médico, afección o antecedentes que, en opinión del investigador, perjudiquen la capacidad del paciente para participar o completar este estudio.
- Pacientes con trastornos médicos subyacentes con una esperanza de vida prevista inferior a 2 años (p. enfermedad oncológica activa con masa tumoral localizada y/o metastatizada).
- Pacientes con antecedentes de alergias graves o múltiples a medicamentos
- Pacientes con hipersensibilidad al fármaco en investigación o a alguno de los excipientes.
- Pacientes que no pueden realizar una prueba válida de 6MWD (p. enfermedad ortopédica, enfermedad oclusiva de la arteria periférica, que afecta la capacidad del paciente para caminar).
No se permiten los siguientes medicamentos específicos para el tratamiento concomitante de HP o medicamentos que puedan ejercer una interacción farmacodinámica con el fármaco del estudio:
- Análogos de prostaciclina parenterales
- Inhibidores específicos de la fosfodiesterasa (p. sildenafil o tadalafil): puede cambiarse a riociguat pero no administrarse además del fármaco del estudio
- o inhibidores de la fosfodiesterasa inespecíficos (p. dipiridamol, teofilina)
- NO donantes (por ejemplo, nitratos)
Exclusiones de enfermedades pulmonares
- Asma bronquial o EPOC de moderada a grave (volumen espiratorio forzado <60 % del valor previsto) o enfermedad pulmonar restrictiva grave (capacidad pulmonar total <70 % del valor previsto) y/o definida como si la tomografía computarizada de alta resolución mostrara <20 % de enfermedad pulmonar parenquimatosa.
- Anomalías congénitas graves de los pulmones, el tórax y el diafragma.
- Evidencia clínica o radiológica de enfermedad pulmonar-veno-oclusiva (PVOD) o hemangiomatosis capilar pulmonar (PCH) o HP y neumonía intersticial idiopática (PH-IIP)
Exclusiones cardiovasculares:
- Hipertensión arterial no controlada (presión arterial sistólica >180 mmHg y/o presión arterial diastólica >110 mmHg).
- Presión arterial sistólica < 95 mmHg.
- Insuficiencia cardiaca izquierda con fracción de eyección inferior al 40%.
- Hipertensión venosa pulmonar con presión de enclavamiento arterial pulmonar >15 mmHg.
- Miocardiopatía hipertrófica obstructiva.
- Enfermedad arterial coronaria grave comprobada o sospechada según la opinión de los investigadores (pacientes con clasificación de angina de la Canadian Cardiovascular Society clase 2-4, y/o que requieren nitratos, y/o infarto de miocardio en los últimos 3 meses antes de la Visita 1).
- Evidencia clínica de enfermedad aterosclerótica sintomática (p. arteriopatía periférica con distancia recorrida reducida, antecedentes de ictus con déficit neurológico persistente, etc.).
Exclusiones relacionadas con trastornos en la función de los órganos:
a) Disfunción hepática clínicamente relevante indicada por: i. bilirrubina >2 veces el límite superior normal ii. y/o transaminasas hepáticas >3 veces el límite superior normal iii. y/o signos de insuficiencia hepática grave (p. alteración de la síntesis de albúmina con una albúmina < 32 g/l, encefalopatía hepática > grado 1a: Criterios de West Haven de alteración del estado mental en la encefalopatía hepática) b) Insuficiencia renal (tasa de filtración glomerular <30 ml/min, p. calculado en base a la fórmula de Cockcroft).

Otro: Riociguat
Riociguat (1 mg, 1,5 mg, 2 mg y 2,5 mg tres veces al día) comenzando con 1,0 mg tres veces al día al inicio del estudio.
La dosis se aumentará individualmente hasta una dosis máxima de 2,5 mg tres veces al día después de 8 semanas.
La medicación del estudio se proporcionará por vía oral con o sin alimentos.
Los comprimidos deben tomarse tres veces al día con un intervalo aproximado de 6 a 8 horas.

El tratamiento se iniciará y ajustará individualmente de acuerdo con la presión arterial sistólica y la tolerabilidad.
Durante la fase de titulación, se le pedirá a cada paciente que mida su presión arterial sistólica periférica y la frecuencia cardíaca en casa tres veces al día y que registre los valores en el diario del paciente.
Los resultados serán examinados por el investigador durante cada visita/visita telefónica.
Siempre que la presión arterial sistólica sea ≥ 95 mmHg medida en el valle antes de la ingesta de cada dosis y el paciente no presente signos o síntomas de hipotensión, la dosis del medicamento del estudio se ajustará en +0,5 mg tres veces al día cada 2 semanas hasta la dosis máxima tolerada ( dosis máxima permitida: de 2,5 mg tid).
Después del período de titulación, la presión arterial debe medirse ante signos o síntomas de hipotensión.
Dosis de mantenimiento: Se debe mantener la dosis individual establecida a menos que se presenten signos y síntomas de hipotensión.
