詳細な説明
この単一施設研究は、コロラド州エドワーズにあるショーがんセンターで実施され、ロサルタンが乳がん患者の放射線誘発性線維症の発症を減少させるという仮説を検証します。 ブロック、二重盲検、プラセボ対照、無作為化第II相試験を利用して、この仮説を検証します。
研究参加者は、外科的介入(乳房温存手術対乳房切除術)によってブロックされ、その後、合計4つの研究群の治療群と対照群に1:1で無作為化されます。 研究チームと研究参加者は研究部門を知らされず、結果の報告における検出バイアスを減らすためにプラセボが使用されます。 選択バイアスは、研究群の無作為化によって最小限に抑えられます。
試験参加者は、12.5mg のプラセボカプセルまたは治験薬ロサルタンを 1 日 2 回経口摂取するように処方されます。 治療開始日は、対象が放射線治療を開始する日になります。 放射線療法は、地元の診療所の手順に従って引き続き処方されます。 研究介入による治療は、放射線療法の完了後1年間継続されます。 すべての参加者は、放射線療法の完了後2年間、線維症、美容転帰、および再手術の発生率について評価されます。

- -臨床的または病理学的ステージ0〜IVの浸潤性乳がんと診断され、非浸潤性乳管がん(Tis)、原発腫瘍を評価できない(TX)、および他のすべての原発腫瘍ステージカテゴリ(T1〜T4)
- 乳房温存手術または再建を伴う乳房切除術の治療を受けている
- -National Comprehensive Cancer Network(NCCN)のガイドラインに従って、片側手術後の放射線療法の候補です
- 18歳以上
- 女性
女性とマイノリティの参加: あらゆる人種や民族の女性がこの試験に参加できます。 妊娠中または授乳中の女性は、この試験から除外されます。
除外基準:
- 乳がんの再発または以前の乳房放射線療法の既往
- 両側乳房/胸壁放射線療法を必要とする乳がん
- 放射線療法前に対側乳房縮小術を受けた乳房温存手術患者
- -術後感染の病歴
- -同時化学療法を受けている
- 文書化された転倒リスク
- -アクティブな既知または疑われる全身性自己免疫疾患(白斑、ホルモン補充のみを必要とする残存自己免疫性甲状腺機能低下症、2年間の全身治療を必要としない乾癬、外部トリガーがなければ再発しないと予想される状態を除く)または全身性の病歴乾癬、リウマチ、全身性狼瘡、強直性脊椎炎または反応性関節炎などの炎症性関節炎
以下を含むがこれらに限定されない制御されていない併発疾患:
- 高カリウム血症
- 腎機能障害
- 症候性うっ血性心不全
- 不安定狭心症
- 腎臓病
- コントロール不良の糖尿病
- 嚢胞性線維症
- 米国リウマチ学会の基準に基づく線維筋痛症
以下の併用:
- その他のレニン-アンジオテンシン系 (RAS) 剤
- 血清カリウムを増加させる薬剤
- リチウム
- 糖尿病に対するアリスキレン
- ロサルタンの有効成分または不活性成分に対する既知のアレルギーがある
- 経口薬に耐えられない
- 妊娠中、授乳中、または放射線照射の翌年に妊娠を計画している
以下のような DNA 修復遺伝子を持っていることが知られている個人:
- 毛細血管拡張性運動失調症 (ATM)
- 二本鎖切断修復タンパク質 rad21 ホモログ (RAD21)
- トランスフォーミング増殖因子 β-1 遺伝子の C-to-T 一塩基多型 (C-509T)

乳房のボリュームの変化

乳房容積の両側マンモグラフィー測定は、定期的なフォローアップ間隔で計算されます。
放射線誘発性線維症に伴う乳房の縮小は、ベースラインから放射線療法終了後 18 か月までの、治療を受けた乳房の乳房容積の変化をモニタリングすることによって評価されます。
両方の乳房の乳房容積の測定には、頭尾方向の投影からの乳房の高さ (H) センチメートル (cm)、乳房の幅 (W) 単位、圧迫厚さ cm (C) が使用されます。
ミリリットル単位の体積 (mL) = (π/4) x 高さ x 幅 x センチメートル。マンモグラムでは、乳頭から胸筋までの乳頭線上の距離測定値 (センチメートル単位) と、乳頭から胸筋までの長さ測定値 (センチメートル単位) も提供されます。乳頭後線(PNL)を90°の角度で二等分する下縁まで。

美容は臨床医の評価であるハーバード美容スケールを使用して評価されます。 1 = 優れています (治療された乳房は未治療の乳房とほぼ同じ)。 2=良好(処理された乳房は未処理の乳房とはわずかに異なる)。 3=普通(処理された乳房は未処理の乳房とは明らかに異なるが、歪んでいない)。 4=不良 治療された乳房が著しく歪んでいる。

自己申告された参加者の生活の質は、Breast-Q 再構成モジュールによって評価されます。 Breast-Q バージョン 2.0 ツールは、心理領域と満足度領域の両方で臨床結果に対する参加者の認識を評価するために開発されました。
