Description détaillée
Cette étude à site unique sera menée au Shaw Cancer Center à Edwards, Colorado, testant l'hypothèse selon laquelle le losartan diminuera le développement de la fibrose induite par les radiations chez les patientes atteintes d'un cancer du sein. Un essai de phase II randomisé en bloc, en double aveugle, contrôlé par placebo sera utilisé pour tester cette hypothèse.
Les participantes à l'étude seront bloquées par une intervention chirurgicale (chirurgie conservatrice du sein par rapport à la mastectomie), puis randomisées, 1:1, dans les bras de traitement et de contrôle pour un total de quatre bras d'étude. L'équipe de recherche et les participants à l'étude ne connaîtront pas le bras de l'étude et un placebo sera utilisé pour réduire le biais de détection dans la notification des résultats. Le biais de sélection sera minimisé grâce à la randomisation des bras d'étude.
Les participants à l'étude se verront prescrire des gélules de 12,5 mg de placebo ou du médicament à l'étude, le losartan, à prendre par voie orale deux fois par jour. La date de début du traitement sera le jour où le sujet commence la radiothérapie. La radiothérapie continuera d'être prescrite conformément aux procédures de la clinique locale. Le traitement avec l'intervention à l'étude se poursuivra pendant un an à la fin de la radiothérapie. Tous les participants seront évalués pour la fibrose, les résultats esthétiques et l'incidence de la réintervention pendant deux ans après la fin de la radiothérapie.

Critère d'intégration:
- Diagnostiqué avec un cancer du sein invasif de stade clinique ou pathologique 0-IV pour inclure le carcinome canalaire in situ (Tis), la tumeur primaire ne peut pas être évaluée (TX) et toutes les autres catégories de stade de tumeur primaire (T1-T4)
- A subi une chirurgie mammaire conservatrice ou une mastectomie avec reconstruction
- Est candidat à la radiothérapie postopératoire unilatérale conformément aux directives du National Comprehensive Cancer Network (NCCN)
- Âge ≥ 18
- Femme
Inclusion des femmes et des minorités : Les femmes de toute race et de toute origine ethnique sont éligibles pour cet essai. Les femmes enceintes ou qui allaitent sont exclues de cet essai.
Critère d'exclusion:
- Cancer du sein récurrent ou antécédents de radiothérapie mammaire antérieure
- Cancer du sein nécessitant une radiothérapie bilatérale du sein/de la paroi thoracique
- Patientes en chirurgie conservatrice du sein avec réduction mammaire controlatérale avant radiothérapie
- Antécédents d'infection post-chirurgicale
- En cours de chimiothérapie concomitante
- Risque de chute documenté
- Maladie auto-immune systémique active connue ou suspectée (à l'exception du vitiligo, de l'hypothyroïdie auto-immune résiduelle nécessitant uniquement un remplacement hormonal, du psoriasis ne nécessitant pas de traitement systémique pendant deux ans, affections dont on ne s'attend pas à ce qu'elles se reproduisent en l'absence d'un déclencheur externe) ou tout antécédent de maladie systémique arthrite inflammatoire telle que psoriasique, rhumatoïde, lupus systémique, spondylarthrite ankylosante ou arthrite réactive
Maladie intercurrente non contrôlée, y compris, mais sans s'y limiter :
- Hyperkaliémie
- Fonction rénale altérée
- Insuffisance cardiaque congestive symptomatique
- Angine de poitrine instable
- Maladie rénale
- Diabète non contrôlé
- Fibrose kystique
- Fibromyalgie selon les critères de l'American College of Rheumatology
Utilisation concomitante de :
- Autre agent du système rénine-angiotensine (RAS)
- Agents pour augmenter le potassium sérique
- Lithium
- Aliskirène pour le diabète
- Avoir une allergie connue à tout ingrédient actif ou inactif du Losartan
- Incapable de tolérer les médicaments oraux
- Enceinte ou qui allaite ou planifie une grossesse dans l'année suivant la radiothérapie
Les personnes connues pour posséder des gènes de réparation de l'ADN, notamment :
- Ataxie-Télangiectasie Mutée (ATM)
- Homologue rad21 de la protéine de réparation de rupture double brin (RAD21)
- Polymorphisme mononucléotidique C-to-T (C-509T) dans le gène du facteur de croissance transformant β-1
- Patients présentant une condition médicale, y compris les résultats des analyses de laboratoire ou des antécédents médicaux ou des évaluations de base, qui (de l'avis de l'investigateur clinique principal ou de sa personne désignée) constitue un risque ou une contre-indication à la participation à l'étude ou qui pourrait interférer avec la conduite de l'étude, l'évaluation des paramètres ou empêcher le sujet de participer pleinement à tous les aspects de l'étude

La fibrose sera évaluée par un fournisseur de radio-oncologie à l'aide de l'échelle Late Effects Normal Tissue Task Force (LENT) - Subjective, Objective, Management, Analytic (SOMA) (LENT-SOMA).
0 = Fibrose absente, non détectable.
1=La fibrose est à peine palpable ; 2=Densité accrue certaine ; 3=Densité, rétraction et fermeté et fixation très marquées

Fibrose pulmonaire radiographique dans le champ de rayonnement de patientes atteintes d'un cancer du sein irradiées

La fibrose pulmonaire radiographique sera évaluée par des tomodensitogrammes haute résolution du thorax.
Les tomodensitogrammes thoraciques seront fusionnés au tomodensitogramme de planification du rayonnement pour confirmation du chevauchement de la fibrose avec le champ de rayonnement.

Niveaux moyens de sénescence cellulaire, de biomarqueurs sériques du facteur de croissance transformant bêta-1 (TGF-β1) et du phénotype sécrétoire associé à la sénescence (SASP)

La sénescence cellulaire et le phénotype sécrétoire associé à la sénescence (SASP), y compris le TGF-β et l'inflammation, seront quantifiés dans le groupe de traitement et le groupe témoin.
Une approche nouvelle et experte pour mesurer les cellules sénescentes dans le sérum sera utilisée.

Modification du volume des seins

La détermination mammographique bilatérale du volume mammaire sera calculée à intervalles de suivi de routine.
Le rétrécissement mammaire associé à la fibrose radio-induite sera évalué en surveillant l'évolution du volume mammaire du sein traité entre le départ et 18 mois après la fin de la radiothérapie.
La mesure du volume mammaire sur les deux seins utilisera la hauteur de la poitrine en centimètres (cm) (H), la largeur de la poitrine en cm (W) et l'épaisseur de compression en cm (C), à partir d'une projection craniocaudale.
Volume en millilitres (mL) = (π/4) x H x L x C. Les mammographies fourniront également une mesure de distance, en centimètres, sur la ligne du mamelon depuis le mamelon jusqu'au muscle pectoral et une mesure de longueur, en centimètres, depuis le haut. à la marge inférieure qui coupe la ligne postérieure au mamelon (PNL) à un angle de 90°.

Cosmesis sera évalué à l'aide d'une évaluation clinicienne L'échelle Harvard Cosmesis : 1 = Excellent (sein traité presque identique au sein non traité) ; 2=Bon (Sein traité légèrement différent du sein non traité) ; 3=Passable (Sein traité clairement différent du sein non traité mais non déformé) ; 4=Mauvais Sein traité gravement déformé.

La qualité de vie autodéclarée des participants sera évaluée par le module de reconstruction Breast-Q. L'outil Breast-Q, version 2.0 a été développé pour évaluer la perception des participants des résultats cliniques dans les domaines psychologiques et de satisfaction sera utilisé.

La décision du participant de subir une chirurgie correctrice sur l'un ou l'autre sein après la radiothérapie sera enregistrée à chaque instant.
Les patientes post-mastectomie seront considérées comme ayant été réopérées si une chirurgie corrective a eu lieu après la pose d'un implant permanent.
