パーキンソン病(PD)は、中脳の黒質緻密部におけるドーパミン作動性ニューロンの進行性喪失によって引き起こされる、一般的な神経変性疾患です。 これによりドーパミンレベルが低下し、脳が運動を制御・調整する能力が損なわれ、無動、筋強剛、振戦、姿勢不安定性などの特徴的な運動症状が生じます。
PDの世界全体のプール有病率は、1,000人あたり1.51例と推定され、男性でより高く、年齢とともに著しく増加します。人口の高齢化により、2050年までに症例数が大幅に増加すると予測されています。 PD患者の約60%が毎年転倒し、その3分の2は反復性転倒を経験し、深刻な二次的合併症、骨折、移動能力の低下、転倒への恐怖、死亡率リスクの増加につながります。
環境要因と遺伝的要因がPDのリスクに関与しており、根治的治療法は存在しませんが、薬物療法(例:レボドパ)と身体活動が症状管理の中心となります。 定期的な身体活動と運動は、PD患者の移動能力、歩行、バランス、姿勢安定性を改善することが示されており、活動レベルの長期的維持は、姿勢・歩行機能、日常生活動作、処理速度の悪化を遅らせることと関連しています。 最近のコクランレビューは、軽度から中等度のPD患者において、運動介入が転倒率を約35%、転倒者の割合を約10%減少させることを確認し、完全に監督されたプログラムがより大きな利益を示しました。
眼球運動障害(例えば、短縮性サッケード、滑動性追従運動障害、輻輳不全)はPDで十分に記録されており、バランスと歩行障害を悪化させる視覚運動障害に寄与します。 PDにおける眼球運動の現れと、一般的な運動がバランスと転倒に及ぼす影響については研究がなされていますが、標的介入としての単独の眼球(眼球運動)運動に関するエビデンスは依然として限られています。 トレッドミル歩行と組み合わせた二重課題眼球運動トレーニングからの予備的知見は、一側性および静的バランスパラメータの改善を示しており、眼球運動要素を含む前庭リハビリテーションは、小規模なPDコホートにおいて眼球運動機能とバランスに肯定的な効果を示しました。 これまでに、構造化された眼球運動がPD患者の運動能力、バランス、転倒リスクに及ぼす単独の影響を直接評価した無作為化試験はありません。
本研究は、PD患者向けに特別に設計された、8週間の同期型在宅遠隔眼球運動プログラムを評価することで、このギャップに対処します。このプログラムは、テレカンファレンスを通じて提供され、アクセシビリティ、遵守率、安全性を高めながら、クリニック内療法に関連する費用と移動の負担を最小限に抑えます。

説明
参加基準:
- 18歳以上
- 臨床的に診断された特発性パーキンソン病(Hoehn and Yahr ステージ1-3)
- 少なくとも1ヶ月間の安定した抗パーキンソン病薬療法
- 自立歩行可能、または最小限の補助で歩行可能(杖・歩行器使用可)
- 担当神経科医の許可
- 研究参加に必要なインフォームドコンセントを与える意思決定能力を有する
- スマートデバイス(スマートフォン、ノートパソコン、タブレットなど)およびインターネットへのアクセスを有する
除外基準:
- 不安定な心臓状態(狭心症、代償不全性うっ血性心不全、重度不整脈など)
- 活動性感染症または急性疾患
- 血行動態不安定
- 重度の認知障害(Mini-Mental State Examination < 24)
- 運動不可能(下肢切断(義肢なし)、安静時・最小活動での重度筋骨格痛)
- 骨折または入院を伴う最近の転倒(過去3ヶ月以内)
- 不安定なPD治療または変更中(漸増中)の薬物療法
- 過去3ヶ月以内の心筋梗塞または脳卒中
- 眼鏡で矯正不可能な重度視覚障害

実験的:Supervised In-Center Eye Exercise Group
Participants will receive a structured eye-exercise program designed to train oculomotor control, gaze stability, and eye-head coordination.
The program will be delivered face-to-face in the center during supervised sessions.
Each session will last approximately 30 to 45 minutes, 3 sessions per week for 8 weeks.

The participants in the study group will perform the eye-exercise program in a standardized seated position under supervision by trained study personnel in a rehabilitation/clinic setting.
The exercises will be administered face-to-face and will include progressive visual fixation, smooth pursuit, saccadic eye movements, convergence/divergence, gaze stabilization, and combined eye-head coordination tasks.
Progression will be based on increases in movement speed, number of repetitions, task complexity, and participant tolerance, while maintaining the standardized seated position and ensuring safety throughout the intervention.

Participants allocated to the control arm will receive standard neurological care and routine advice to maintain their usual daily activities.
They will not receive the structured eye-exercise program during the intervention period.

Change in dynamic balance

|
Dynamic balance will be assessed using the Mini-BESTest total score.
The Mini-BESTest evaluates anticipatory postural adjustments, reactive postural control, sensory orientation, and dynamic gait.
Higher scores indicate better dynamic balance and postural control.
