상세 설명
PD-L1 low(PD-L1Lo), EGFR wt, EML4/ALK 융합 음성 NSCLC가 있는 약 65명의 피험자가 Pembrolizumab의 효능 및 안전성과 관련된 생물학적 특성을 조사하기 위해 이 시험에 등록할 것입니다. 피험자는 3주마다 200mg의 용량으로 Pembrolizumab iv를 받습니다. 피험자는 치료에 대한 반응을 평가하기 위해 방사선 촬영으로 9주마다(63 +/- 3일) 평가됩니다. 피험자는 RECIST에서 정의한 질병의 진행, 허용할 수 없는 독성 또는 동의 철회까지 할당된 연구 치료를 계속합니다.
펨브롤리주맙을 사용한 치료는 2년의 요법이 시행되고, 문서화된 질병 진행, 허용할 수 없는 부작용(들), 치료의 추가 투여를 방해하는 병발 질환, 피험자 철회에 대한 조사자의 결정, 피험자가 동의 철회, 피험자의 임신, 시험 치료 또는 절차 요구 사항 또는 행정적 이유를 준수하지 않는 경우. RECIST 1.1에 따라 확인된 완전 반응(CR)을 얻은 펨브롤리주맙 치료 피험자는 시험 치료 중단을 고려할 수 있습니다. 이러한 피험자는 조사자의 재량에 따라 방사선학적 질병 진행을 경험한 후 Pembrolizumab으로 재치료할 수 있으며, 이 재치료는 2차 과정 단계가 될 것입니다.

포함 기준:
- IIIB/IV기에서 NSCLC 진단이 확인되었습니다. EGFR 민감화(활성화) 돌연변이 또는 ALK 전좌가 없고 PD-L1 "낮음"(
- 시험에 대한 서면 동의/동의를 기꺼이 제공할 수 있어야 합니다.
- 정보에 입각한 동의서에 서명한 날 기준으로 18세 이상이어야 합니다.
- RECIST 1.1에 따라 측정 가능한 질병이 있습니다.
- 보관된 조직학적 표본 또는 새로 얻은 종양 병변의 핵심 또는 절제 생검에서 조직을 기꺼이 제공합니다. 새로 얻은 표본은 치료 시작 1일 전 최대 45일 동안 얻은 표본으로 정의됩니다.
- ECOG 수행 척도에서 수행 상태가 0 또는 1이어야 합니다.
- 적절한 장기 기능 입증
- 모든 스크리닝 실험실은 치료 시작 후 10일 이내에 수행해야 합니다.
- 가임 여성 피험자는 연구 약물의 첫 용량을 받기 전 72시간 이내에 음성 소변 또는 혈청 임신을 가져야 합니다. 소변 검사가 양성이거나 음성으로 확인할 수 없는 경우 혈청 임신 검사가 필요합니다.
- 가임 여성 피험자는 2가지 피임 방법을 기꺼이 사용하거나 외과적으로 불임이거나 연구 과정 동안 연구 약물의 마지막 투여 후 120일까지 이성애 활동을 삼가야 합니다(참조 섹션 5.7.2). 가임기 대상자는 외과적으로 불임 수술을 받지 않았거나 > 1년 동안 월경이 없는 사람입니다.
- 남성 피험자는 연구 요법의 첫 번째 용량부터 연구 요법의 마지막 용량 후 120일까지 적절한 피임 방법을 사용하는 데 동의해야 합니다.
- 치료가 필요한 활동성 악성 종양의 병력 없음
제외 기준:
피험자는 다음과 같은 경우 시험 참여에서 제외되어야 합니다.
- EGFR 민감화 돌연변이 및/또는 ALK 전좌가 있습니다.
- 중앙 실험실에서 "높음"으로 평가된 PD-L1 발현이 있음
- 면역결핍 진단을 받았거나 시험 치료의 첫 투여 전 7일 이내에 전신 스테로이드 요법 또는 다른 형태의 면역억제 요법을 받고 있습니다.
- 활동성 결핵(Bacillus Tuberculosis)의 알려진 병력이 있습니다.
- Pembrolizumab 또는 그 부형제에 대한 과민증.
- 연구 1일 전 4주 이내에 이전에 항암 단클론 항체(mAb)를 앓았거나 4주보다 일찍 투여된 제제로 인해 부작용으로부터 회복되지 않은(즉, ≤ 1 등급 또는 기준선에서) 자.
연구 1일 전 2주 이내에 이전 화학 요법, 표적 소분자 요법 또는 방사선 요법을 받았거나 이전에 투여된 제제로 인한 부작용으로부터 회복되지 않은(즉, ≤ 1 등급 또는 기준선에서) 자.
- 참고: 등급 2 이하의 신경병증이 있는 피험자는 이 기준에 대한 예외이며 연구 대상이 될 수 있습니다.
- 참고: 피험자가 대수술을 받은 경우 치료를 시작하기 전에 개입으로 인한 독성 및/또는 합병증에서 적절하게 회복해야 합니다.
- 진행 중이거나 적극적인 치료가 필요한 알려진 추가 악성 종양이 있습니다. 잠재적인 치유 요법을 받은 피부의 기저 세포 암종 또는 피부의 편평 세포 암종 또는 제자리 자궁경부암은 예외입니다.
- 지난 2년 동안 전신 치료(즉, 질병 조절제, 코르티코스테로이드 또는 면역억제제 사용)가 필요한 활동성 자가면역 질환이 있습니다. 대체 요법(예: 티록신, 인슐린 또는 부신 또는 뇌하수체 기능 부전을 위한 생리학적 코르티코스테로이드 대체 요법 등)은 전신 치료의 한 형태로 간주되지 않습니다.
- 스테로이드가 필요한 비감염성 폐렴의 병력이 있거나 현재 폐렴이 있는 경우.

- 임상시험의 요구 사항에 협조하는 데 방해가 되는 정신과적 또는 약물 남용 장애가 있음을 알고 있습니다.
- 사전 스크리닝 또는 스크리닝 방문부터 시작하여 시험 치료의 마지막 투여 후 120일까지 시험의 예상 기간 내에 임신 또는 모유 수유 중이거나 임신 또는 아이의 아버지가 될 것으로 예상되는 경우.
- 이전에 항 PD-1, 항 PD-L1 또는 항 PD-L2 제제로 치료를 받은 적이 있습니다.
