1 배경
VOLITION 개입은 일반적인 진료 상담 중 임상 의사 결정에 복합상병 노인이 쉽게 참여하도록 개발되었습니다. 가장 중요한 프로젝트는 개입 개발의 각 단계에 기존 문헌 및 관련 이론을 체계적으로 적용하는 수단으로서 개입 매핑 프레임워크를 따르고 평가.
두 가지 핵심 개입 구성 요소가 있습니다.

4 연구 설계
영국 일반 진료에서 수행된 혼합 방법 순차적 설명 연구. 양적 설문지 데이터가 먼저 수집됩니다. 그런 다음 정성적 인터뷰 데이터를 사용하여 참가자의 관점을 더 깊이 탐색하여 정량적 결과를 설명, 구축 및 정교화합니다. 설문지 데이터는 환자 참가자의 의도적인 선택을 알려줍니다. 따라서 양적 및 질적 단계는 결합된 결과를 해석하기 전에 연구의 중간 단계에서 연결됩니다.
5 연구 설정
영국 Devon에서 5개의 GP 실습을 모집합니다. 연구팀의 여행경비 절감과 모집 기간 단축을 위해 현지 실무자를 우선 모집한다. 그러나 참가자 샘플에서 사회인구학적 다양성을 얻기 위해 시골과 도시 지역 모두에서 관행을 모집할 것입니다.
3,000명의 환자의 최소 진료 규모는 충분한 수의 적격 참가자를 보장합니다. (평균 8,000명의 환자 진료에 약 800명의 복합상병 노인 환자가 있을 것으로 예상됩니다.
7 연구 절차
7.1 샘플링
진료당 평균 3명(총 = 15 GP), 최소 2명의 GP가 모집됩니다. 2개 이상의 장기 질환이 존재하는 것으로 정의되는 알려진 복합상병이 있는 65세 이상의 환자에게 접근할 것입니다.
환자 참가자는 연구팀이 설계한 자동화된 실습 데이터베이스 검색 전략을 사용하여 식별됩니다. 채용된 GP는 이 '플래그'를 사용하여 작업 일정에서 10명의 연속 잠재적 참가자를 식별합니다. 환자의 무작위 샘플링은 임상 프레젠테이션으로 요구되지 않으므로 연속 상담 사이의 의사 결정의 특성은 무작위로 예상됩니다. 총 150명(최소 100명)의 잠재적 환자가 5가지 진료를 통해 식별되며, 이 환자의 절반 이상으로부터 데이터를 수집하는 것을 목표로 합니다.
샘플 크기는 테스트 중인 임상 결과 측정에 대한 표준 편차의 추정을 허용하고 상담 방식(예: 대면 대 원격). 또한 질적 인터뷰를 위해 참가자의 의도적으로 선택된 하위 샘플을 얻는 것으로도 충분합니다.
7.2 모집
탈락 및 후속 조치에 대한 손실은 초과 모집에 의해 환자, GP 또는 실습 수준에서 고려됩니다. 실무자들은 참여를 권유하는 전화 서신에 이어 이메일을 받게 됩니다. 실습 모집은 GP 중 최소 2명의 관심 표명에 따라 결정됩니다. 관행은 지역 조정, 설정, GP의 시간 및 작업량에 대한 지불을 통해 인센티브를 받게 됩니다. 지불은 South West Clinical Research Network의 고문이 결정했습니다.
모집된 각 GP에 대해, 모든 상담 모드(전화, 비디오, eConsult 또는 대면)에 대해 GP와 연속으로 연락하는 10명의 잠재적 환자 참여자는 미리 지정된 자격 기준(자격 양식 참조)을 사용하여 GP에 의해 선별됩니다. 상담시간. 자격 데이터는 익명으로 수집됩니다. 자격이 있는 경우 GP는 상담 후 우편으로 보낼 환자 팩에 라벨을 붙입니다. 자격이 있는 환자는 커버 레터, 참가자 정보 시트* 및 연구팀에 반환할 선불 봉투와 함께 상담 후 설문지를 받게 됩니다. 설문지를 사용하여 환자는 연구원과의 후속 전화 인터뷰를 위해 연락하는 데 동의하고 연락처 정보를 제공하도록 요청받을 것입니다. 참여를 원하지 않는 환자는 설문지를 반환하지 않습니다. 모집된 모든 GP는 또한 상담 후 설문지를 작성하고 전화 인터뷰에 참여해야 합니다.
7.3 동의
진료 모집 시 GP의 서면 동의를 받습니다. 잠재적인 환자 참가자를 식별하고 접근하는 것에 대한 동의는 진료 관리자로부터 얻습니다. 설문지 데이터 사용에 대한 환자의 동의는 설문지 반환으로 확인되며 참가자 정보 시트를 사용하여 환자에게 이를 알립니다. 서면 동의는 전화 인터뷰 전에 얻어지며, 절차를 명확히 하고 참가자의 질문에 답하기 위해 중간 전화 통화로 통지되며, 동의서는 선불 봉투에 반환됩니다. 인터뷰를 시작하기 전에 구두 동의도 확인됩니다. 인터뷰 불참 사유를 명시한 경우 익명으로 기록됩니다. 식별 가능한 데이터는 사전 환자 동의 없이 액세스되지 않습니다.
7.4 기본 데이터
진료 수준 데이터(목록 크기, 위치 및 박탈, 진료 규모, 직원 배치, 농촌/도시) 및 GP 참가자 데이터(연령, 성별, 민족 및 자격 취득 이후 시간)는 채용 시 수집됩니다. 관심 있는 환자 그룹에 대한 원격 상담 경험의 불평등 가능성을 식별하거나 해결했는지 여부와 방법에 관한 정보를 실무에서 요청합니다.
데이터는 복합상병이 있는 고령 환자와의 상담 모드(전화, 비디오, eConsult 또는 대면) 및 상대적 빈도와 관련하여 실무 수준에서 수집됩니다. 각 진료의 해석 및 원격 대 대면 컨설팅 제공과 관련하여 GP의 업무량을 관리하는 방법에 관한 데이터를 수집하고 비교하기 위해 템플릿이 GP 수술에 제공됩니다. 데이터는 모집 시 GP로부터 복합상병이 있는 고령 환자와 상담할 때 다양한 상담 방식에 대한 선호도에 대해 얻을 수 있습니다.
환자의 인구 통계(연령, 성별 및 민족) 및 건강 상태 데이터는 상담 후 설문지를 사용하여 자가 보고를 통해 수집됩니다. 적용 가능한 경우 각 환자에 대한 전자 노쇠 지수 점수는 상담 후 설문지에서 GP로부터 얻을 수 있습니다.
7.5 연구 평가
정량적 결과 측정 데이터
GPPS(General Practice Patient Survey)는 5점 리커트 척도를 사용합니다. 이들 중 일부는 상담 후 환자 설문지에서 PROM(Patient Reported Outcome Measures)을 평가하는 데 적합합니다. PROM은 의사 결정 참여에 대한 환자의 선호도, 환자가 의사 결정에 참여했다고 느끼는 정도, 환자가 선호하는 상담 방식 및 이를 얻었는지 여부, 받은 치료에 대한 만족도, 그들은 충분한 시간과 어느 정도의 환자 지원이 주어졌다고 느꼈습니다.
GP를 위한 상담 후 설문지는 상담 방식 및 대략적인 기간(척도 사용)과 의사 결정에 환자가 관여했다고 느끼는 정도에 대한 GP의 평가에 관한 데이터를 수집합니다.
질적 인터뷰 데이터
상담 후 참가자 설문지의 수신 및 초기 분석에 따라 20명의 환자가 샘플링되고 이 숫자의 두 배가 접근됩니다. 연령, 사회인구학적 특성, 건강 상태, 상담 방식, 공유된 의사 결정에 대한 선호도 및 경험에 따라 이질성을 보장하기 위해 설문지 데이터를 사용하여 환자를 의도적이고 반복적으로 선택합니다. 인터뷰를 준비하기 위해 환자에게 전화로 연락을 드릴 것입니다. 모든 참여 GP(n = 10-15)는 이메일로 초대됩니다. 샘플 크기는 주제별 포화도를 평가할 때 검토됩니다.
반구조화된 전화 인터뷰는 주제 가이드의 도움을 받을 것이며 디지털 방식으로 오디오 녹음, 필사 및 현장 메모로 지원됩니다. 초기 공개 토론 후 주제 가이드는 참가자가 설문지 응답의 이유에 대해 토론하도록 권장합니다. 예를 들어 참가자는 공유된 의사 결정에 대한 평가 또는 특정 상담 양식에 대한 선호도에 대해 더 심도 있게 토론하라는 메시지를 받을 수 있습니다. 개입은 VOLITION의 구현에 잠재적으로 영향을 미칠 수 있는 요인에 대한 토론을 자극하기 위해 전화 인터뷰 중 적절한 중간 지점에서 참가자에게 설명됩니다(부록의 주제 가이드 참조). 참가자들은 영국에서 진행 중인 사회적 거리두기 조치에 비추어 다양한 상담 양식의 맥락에서 이러한 요소에 대해 논의하게 됩니다. 중재 자료는 이전 인터뷰 대화에 대한 자료의 영향을 피하기 위해 인터뷰 전에 환자에게 게시되지 않습니다. 참가자는 연구자의 영향 없이 새로운 데이터를 수집하기 위해 자신의 아이디어를 토론하도록 권장됩니다. 참가자 인터뷰의 오디오 녹음은 Nvivo 컴퓨터 소프트웨어를 사용하여 코딩됩니다.
혼합 방법
데이터 분석은 순차적-설명적 접근 방식을 취합니다. 정량적 설문 데이터 및 후속 분석(아래 세부 정보 참조)은 연구 문제에 대한 참가자의 관점에 대한 일반적인 이해를 제공합니다. 질적 데이터와 그 분석은 의도적으로 선택된 참가자의 관점을 더 깊이 탐구함으로써 통계 결과를 다듬고 설명할 것입니다. 인터뷰 해석은 연역적 방법과 귀납적 방법을 결합하여 프로젝트 초기에 개발된 이론이 유효한지 여부를 조사할 뿐만 아니라 새로운 데이터와 새로운 주제에 개방적입니다. 주제 포화도 개념을 적용하고 부정적인 사례를 적극 모색한다. 코딩이 적용되는 일관성은 데이터의 1/5에서 평가됩니다.
