RBBB 환자의 적응형 심장 재동기화 요법

연구 개요
상세 설명
현재 지침에서는 CRT 이식이 LBBB 환자에 비해 RBBB 환자에게 덜 유익하다고 명시하고 있습니다(2). 현재 증거에 따르면 좁은 QRS 및 좌심실 기능 장애가 있는 환자의 빈번하거나 지속적인 RV 전용 페이싱은 RV 정점 페이싱으로 인한 심실 비동기화로 인해 해로울 수 있습니다(3-5).
연구에 따르면 좌심실 기능 장애와 LBBB 및 ICD 백업이 있는 동율동 환자에서 좌심실 전용 페이싱이 BIV 페이싱보다 열등하지 않습니다(6,7). 심각한 좌심실 수축 기능 장애 및 LBBB(적응형 CRT)가 있는 환자에서 고유 전도와 동기화된 좌심실 조율을 제공하는 CRT의 새로운 알고리즘은 적어도 프로토콜 기반 심초음파 최적화만큼 효과적이라는 것을 보여주었습니다. 또한 HF 및 모든 원인으로 인한 입원 모두에 대해 30일 재입원 확률이 크게 감소하고 기존 CRT에 비해 AF 발생률이 46% 감소하며 CRT 장치 배터리 수명이 연장되는 것으로 나타났습니다(8-10). > 50% 동기화된 좌심실 조율을 제공하는 적응형 CRT 알고리즘을 가졌거나 적응형 CRT 알고리즘으로 정상적인 방실 전도를 가진 환자는 <50% 동기화된 좌심실 조율 또는 심초음파에 최적화된 BIV를 가진 환자와 비교할 때 사망 또는 심부전 입원 위험이 감소했습니다. 각각 페이싱(11). 또 다른 급성 연구에서 LV dP/dtmax는 LV 페이싱이 고유 활성화로 인한 심실 융합과 연관되었을 때 BIV 페이싱보다 LV에서 더 높았습니다(12).
여러 연구에서 프로그래밍된 방실 지연(AVD) 및 심실간 지연(VVD) 지연의 최적화가 BIV 페이싱의 장기 결과를 점진적으로 개선할 수 있다고 제안했습니다(13-15).
측면 좌심실 벽은 LBBB와 비교할 때 RBBB 환자에서 조기에 수축하므로 CRT 장치의 좌심실 조율이 덜 도움이 될 수 있습니다. . 빈맥으로 유발된 심근병증과 RBBB(6마리) 또는 LBBB(6마리)(16)가 있는 12마리의 개를 대상으로 한 실험 연구에서 RV 전용 페이싱은 좌심실에 배치된 카테터 팁으로 측정한 dP/dtmax로 볼 때 좌심실 기능과 동시성을 향상시켰습니다. RBBB 그룹에서 심장 MRI에 의한 좌심실 및 동시성 평가는 BIV 조율보다 우수합니다. 좌심실 전용 조율은 RBBB 그룹에서 기능을 악화시켰습니다.
우심실만 조율은 7명의 RBBB 및 우심실 기능장애 환자의 전향적 연구에서 평가되었으며 대부분의 환자는 선천성 심장병을 앓았습니다. 좌심실 글로벌 기능은 기준선에서 온전했습니다(17). PR 간격의 90%의 방실 지연을 갖는 순차 방실 RV 조율은 심도자술에서 볼 수 있는 RV dP/dtmax 및 좌심실 심장 지수의 개선 모두에서 심방 단독 조율보다 우수했습니다. RV 페이싱이 있는 RBBB 환자에서 RV dP/dtmax는 22% 증가했고 QRS는 163 +/- 39에서 126 +/- 31 ms로 감소했습니다. 두 연구는 함께 RBBB 및 HF 환자에서 BIV 페이싱의 이점의 대부분이 RV 페이싱 구성 요소에 기인하며 LV 페이싱 구성 요소로 인해 이점이 감소할 수 있음을 시사합니다. RBBB 환자의 RV 페이싱 타이밍을 개선하면 LBBB 및 좌심실 수축 기능 장애가 있는 환자의 적응형 LV 페이싱 CRT에서 볼 수 있듯이 RV 페이싱의 비동기화를 크게 줄이고 RV 페이싱의 이점을 최대화할 수 있습니다.

- 부비동 리듬 또는 고립된 심방 조율
- 좌심실 수축 기능 장애(장치 이식 시 EF <35%)
- AV 전도 <250ms, RBBB 및
- ACC/AHA/HRS 지침(2, 18)에 따라 CRT 장치를 이식했거나 ACC/AHA/HRS 지침(2)에 따라 연구 등록 중에 CRT 장치를 이식할 것입니다.
제외 기준:
- 심방세동
- 심방조동
- 심방 빈맥
- AV 지연 > 250ms
- 연구 당시 안정시 심박수가 100bpm인 부비동 빈맥
- 빈번한 APC(총 박동/분의 > 25%) 또는 PVC(총 박동/분의 > 20%), 또는
- 등록 시 EF > 40%인 환자(LV 수축기 기능이 이식 시부터 개선된 것으로 확인된 경우).
