Adaptieve cardiale resynchronisatietherapie bij patiënten met RBBB
Cardiale resynchronisatietherapie met gesynchroniseerde RV-stimulatie ter verbetering van de hartfunctie bij patiënten met rechterbundeltakblok en systolische LV-dysfunctie

Volgens de huidige richtlijnen is implantatie van CRT minder gunstig voor patiënten met RBBB in vergelijking met patiënten met LBBB (2). Huidig bewijs suggereert dat frequente of aanhoudende RV-only stimulatie bij patiënten met nauwe QRS en LV disfunctie schadelijk kan zijn als gevolg van ventriculaire desynchronisatie die toe te schrijven is aan RV apex stimulatie (3-5).
Studies suggereren dat LV-only stimulatie niet inferieur is aan BIV-stimulatie bij sinusritmepatiënten met LV-disfunctie en back-up van LBBB en ICD (6,7). Er is aangetoond dat een nieuw algoritme van CRT door gesynchroniseerde LV-stimulatie te leveren met de intrinsieke geleiding bij patiënten met ernstige LV systolische disfunctie en LBBB (adaptieve CRT) aantoont dat het minstens zo effectief is als protocolgestuurde echocardiografische optimalisatie. Het heeft ook een significante vermindering aangetoond van de kans op heropname na 30 dagen voor zowel HF als ziekenhuisopnames door alle oorzaken, 46% minder incidentie van AF in vergelijking met conventionele CRT, en verlenging van de levensduur van de batterij van het CRT-apparaat (8-10). Patiënten met een adaptief CRT-algoritme dat > 50% gesynchroniseerde LV-stimulatie leverde of normale AV-geleiding hadden met het adaptieve CRT-algoritme hadden een lager risico op overlijden of ziekenhuisopname voor hartfalen in vergelijking met patiënten met <50% gesynchroniseerde LV-stimulatie of voor echocardiografie geoptimaliseerde BIV respectievelijk pacing (11). In een ander acuut onderzoek was de LV dP/dtmax hoger bij LV dan bij BIV-stimulatie wanneer LV-stimulatie gepaard ging met ventriculaire fusie veroorzaakt door intrinsieke activatie (12).
Verscheidene onderzoeken hebben gesuggereerd dat optimalisatie van de geprogrammeerde atrioventriculaire vertraging (AVD) en interventriculaire vertraging (VVD) vertragingen stapsgewijs het langetermijnresultaat van BIV-stimulatie kan verbeteren (13-15).
De laterale LV-wand trekt vroeg samen bij patiënten met RBBB in vergelijking met LBBB, dus het is minder waarschijnlijk dat LV-stimulatie in de CRT-apparaten gunstig is. . In een experimenteel onderzoek bij 12 honden met tachycardie-geïnduceerde cardiomyopathie en RBBB (6 honden) of LBBB (6 honden) (16) verbeterde stimulatie met alleen RV de LV-functie en synchronie zoals gezien door dP/dtmax gemeten door kathetertip in de LV kamer- en synchronie-evaluatie door cardiale MRI in de RBBB-groep even goed of beter dan BIV-stimulatie. Alleen LV-stimulatie verslechterde de functie in de RBBB-groep.
Pacing met alleen RV werd ook geëvalueerd in een prospectieve studie van 7 patiënten met RBBB- en RV-disfunctie, waarbij de meeste patiënten een aangeboren hartaandoening hadden. LV globale functie was intact bij baseline (17). Sequentiële atrioventriculaire RV-stimulatie met een atrioventriculaire vertraging van 90% van het PR-interval was superieur aan alleen atriale stimulatie voor zowel verbetering in RV dP/dtmax als LV cardiale index zoals waargenomen bij hartkatheterisatie. De RV dP/dtmax steeg met 22% bij RBBB-patiënten met RV-stimulatie en QRS daalde van 163 +/- 39 naar 126 +/- 31 ms. De twee onderzoeken samen suggereren dat het grootste deel van het voordeel van BIV-stimulatie bij patiënten met RBBB en HF te danken is aan de RV-stimulatiecomponent en dat het voordeel mogelijk verminderd is vanwege de LV-stimulatiecomponent. Een betere timing van RV-stimulatie bij patiënten met RBBB zou de desynchronisatie van RV-stimulatie aanzienlijk kunnen verminderen en het voordeel van RV-stimulatie maximaliseren, zoals gezien wordt bij adaptieve LV-pacing CRT bij patiënten met LBBB en LV systolische disfunctie.
Deze onderzoeken hebben geleid tot de huidige hypothese dat adaptieve RV-stimulatie waarbij alleen RV-stimulatie wordt gebruikt, gesynchroniseerd met LV-activering wanneer intrinsieke AV-geleiding normaal is, meer fysiologisch is en de RV- en LV-functie zal verbeteren door middel van echocardiografieparameters en op Medtronic ECG-band voor activering bij patiënten met RBBB- en LV-disfunctie vanwege verbeterde synchronisatie en verkorting van de QRS-duur.
Als secundair doel wordt de levensduur van de batterij bij BIV-stimulatie vergeleken met adaptieve RV-stimulatie, ervan uitgaande dat ten minste 50% van de RV-only-stimulatie wordt bereikt met adaptieve CRT voor RBBB.

Beschrijving
Inclusiecriteria: opgenomen patiënten hebben:
- sinusritme of geïsoleerde atriale stimulatie
- LV systolische disfunctie (EF <35% op het moment van implantatie van het apparaat)
- AV-geleiding <250 ms, RBBB en
- een geïmplanteerd CRT-apparaat hebben op basis van de ACC/AHA/HRS-richtlijnen (2, 18) of een CRT-apparaat laten implanteren tijdens de studie-inschrijving volgens de ACC/AHA/HRS-richtlijnen (2).
Uitsluitingscriteria:
- atriale fibrillatie
- atriale flutter
- atriale tachycardie
- AV-vertraging > 250 ms
- sinustachycardie met hartslag in rust ten tijde van het onderzoek 100 spm
- frequente APC's (> 25% van het totale aantal slagen/min) of PVC's (>20% van het totale aantal slagen/min), of
- patiënten met EF > 40% op het moment van opname (als LV systolische functie verbeterd bleek te zijn vanaf het moment van implantatie).

Ander: CRT-optimalisatie
Elke patiënt krijgt atriale stimulatie die 10% hoger is dan de sinusfrequentie of atriale stimulatie met een frequentie van 60 spm als significante sinusbradycardie aanwezig is.
Bij elke patiënt worden in dezelfde sessie zes ECG's en echocardiografische reeksen uitgevoerd.
Daarnaast zullen ook zes elektrische activeringsevaluaties door de Medtronic ECG-riem worden uitgevoerd.
Het eerste onderzoek zal altijd plaatsvinden zonder ventriculaire stimulatie.
Eén studie zal zijn voor echocardiografie geoptimaliseerde BIV-stimulatie en vier studies voor echocardiografie-geoptimaliseerde (?) adaptieve RV-only stimulatie met verschillende AV-intervallen.
Er zal een willekeurige volgorde worden uitgevoerd om de volgorde voor de rest van de onderzoeken te bepalen.
