SPATAX: Klinische en genetische analyse van cerebellaire ataxie en spastische dwarslaesie (Spatax)

Klinische en genetische analyse van autosomaal recessieve vormen van cerebellaire ataxie en spastische dwarslaesie
Cerebellaire ataxie (CA) en spastische paraplegie (SP) zijn genetisch en klinisch zeer heterogeen.
Er zijn al meer dan 40 loci bekend, maar het aantal fenotypes is nog groter, wat wijst op verdere genetische heterogeniteit.
Deze progressieve aandoeningen zijn vaak ernstig en fataal, vanwege het ontbreken van een specifieke therapie.
Het SPATAX-netwerk combineert de ervaring van Europese clinici en wetenschappers die aan deze groepen ziekten werken.
In het afgelopen jaar hebben ze de grootste verzameling families verzameld en een aantal taken volbracht (initiatie van een klinische en genetische database, distributie van DNA naar deelnemende laboratoria, in kaart brengen van drie nieuwe loci en verfijning van verschillende loci).
Naast clinici uit Europa en mediterrane landen, die een belangrijke rol spelen bij het verzamelen van families volgens evaluatietools die zijn ontwikkeld en gevalideerd door de SPATAX-leden, omvat de groep grote Europese laboratoria die zich bezighouden met het ophelderen van de moleculaire basis van deze aandoeningen.
Elk laboratorium zal alle families met een subtype van autosomaal recessief (AR) CA (n=116) of SP (n=207) centraliseren om de verantwoordelijke genen efficiënt in kaart te brengen en te identificeren.
In 61 families zijn al genoombrede scans aan de gang.
Gezien de expertise van de deelnemers verwachten de onderzoekers in de loop van dit project meerdere genen in kaart te brengen en te identificeren.
Het spectrum van mutaties en fenotype/genotype-correlaties zal worden geanalyseerd dankzij deze unieke reeks patiënten met verschillende fenotypes.
De opgedane kennis zal onmiddellijk toepasbaar zijn voor patiënten in termen van verbeterde positieve diagnose, follow-up en passende erfelijkheidsadvisering.
Op de lange termijn zullen modellen voor genetische entiteit worden ontwikkeld om de pathofysiologie te begrijpen en nieuwe doelen voor behandeling te identificeren.
De reeks patiënten die zijn verzameld en de precieze kennis van de natuurlijke geschiedenis zullen de implantatie van therapeutische proeven op basis van rationele benaderingen vergemakkelijken.
