Preventie van herstart van aritmie en RatE-stabilisatie bij atriumfibrilleren

Het doel van deze proef is om de therapeutische doeltreffendheid aan te tonen van de preventieve stimulatietherapieën van de Selection 9000 en Vitatron T70 DR-pacemakers, speciaal voor de behandeling van atriumfibrilleren (AF). Twee nieuwe algoritmen (post-AF-respons en ventriculaire frequentiestabilisatie [VRS]) zullen worden beoordeeld in een gecombineerd traject, vergeleken met een controlegroep. De werkzaamheid van de algemeen beschikbare therapieën om AF en de symptomen ervan te voorkomen, zal dan ook worden beoordeeld.

Doordat anti-aritmische farmacologische geneesmiddelen er niet in slagen atriumfibrilleren (AF) te onderdrukken, neemt de belangstelling voor preventieve stimulatiebehandelingen toe. Het belang van deze optie in het arsenaal aan preventieve instrumenten is volledig gerechtvaardigd, alleen door rekening te houden met de ontoereikendheid, de complexiteit of de slechte reproduceerbaarheid van de andere niet-farmaceutische benaderingen.

Preventieve stimulatietherapieën zijn afhankelijk van hun potentiële effect op verschillende aanvangsmodaliteiten die door eerdere studies werden benadrukt, en meer in het algemeen door atriumweefsel te stabiliseren wanneer potentiële triggers verschijnen.

Bovendien kan de hartstimulator deze therapieën leveren bij het identificeren van deze triggers, maar kan hij ook onvergelijkbare diagnostische hulpmiddelen bieden in termen van gevoeligheid, specificiteit en continuïteit in de monitoring.

Er zijn al vier preventieve stimulatietherapieën geëvalueerd. Het doel van deze studie is om het klinische voordeel aan te tonen van de nieuwe functies van de Selection 9000 / Vitatron T70 DR.

Beschrijving

Inclusiecriteria:

Patiënt met gedocumenteerd atriumfibrilleren: ten minste één episode van paroxismaal AF gedocumenteerd 6 maanden voorafgaand aan opname, die langer dan een minuut duurt
Patiënt met een brady-tachysyndroom of een sick-sinussyndroom, met een permanente pacing-indicatie
Atriale lead met een tip-tot-ring-interval gelijk aan of kleiner dan 12 mm
Patiënt die akkoord gaat met en de geïnformeerde toestemming heeft ondertekend

AF gerelateerd aan een omkeerbare oorzaak
Eén elektrische cardioversie 6 maanden voorafgaand aan opname
Instabiele angina
Myocardinfarct (MI) minder dan 3 maanden
