Lokaal gevorderde borstkanker is een veel voorkomende aandoening in ontwikkelingslanden. Neoadjuvante chemotherapie biedt de mogelijkheid om genetische handtekeningen te identificeren die verband houden met objectieve klinische en pathologische volledige reacties.
Patiënten zullen doxorubicine/cyclofosfamide met paclitaxel krijgen. Tumormonsters die voor en na chemotherapie zijn verzameld, zullen worden geanalyseerd.

Inclusiecriteria:
- Vrouwen met lokaal gevorderde borstkanker bij vrouwen
- Histologie: ductale of lobulaire invasieve histologie
- Akkoord om deel te nemen aan het onderzoek en ondertekening van de geïnformeerde toestemming
- Klinische toestand om doxorubicine/cyclofosfamide en paclitaxel te krijgen
- ECOG 0 of I
Uitsluitingscriteria:
- Niet klinisch stadium III
- Inflammatoire borstkanker
- Vorige behandeling
- Eerdere diagnose van kanker (behalve basaalcel- of plaveiselcelcarcinoom van de huid en niet-invasief cervicaal carcinoom)
- Zwangerschap
- Afwezigheid van klinische conditie om chemotherapie te ontvangen

Vrouwen met invasieve borstkanker; klinisch stadium III, voorwaarde voor neoadjuvante chemotherapie op basis van doxorubicine/cyclofosfamide en paclitaxel gevolgd door chirurgie (mastectomie en okselklierdissectie)

4 cycli AC: doxorubicine 60mg/m2

De operatie wordt 30 dagen na de chemotherapie uitgevoerd. De correlatie tussen klinische, radiologische en pathologische respons zal worden gerapporteerd. Het percentage oncoplastische chirurgie zal worden gerapporteerd

Klinische en radiologische onderzoeken die moeten worden uitgevoerd vóór chemotherapie, na de 4e AC-cyclus en vóór een operatie. Pathologische evaluatie uit te voeren 30 dagen na de laatste chemokuur, b.v. na de operatie. |
