Menselijke leverexplantaten voor HIF-1 alfa-analyse/vergelijking

Gedetailleerde beschrijving
In het momenteel lopende retrospectieve aspect van de studie zullen we pathologische tumor-explantatiespecimens verkrijgen uit de tumorexplantatiepopulatie van 1-8-1994 tot 31-12-2005. We zullen 5-10 objectglaasjes maken van elk tumor-explantatieblok en HIF-1α meten met behulp van immunohistochemische kleuring. We zullen ook een retrospectieve kaartbeoordeling uitvoeren voor specifieke gegevenspunten waarvan wij denken dat ze nodig zijn om ons te helpen bij onze analyse van de histopathologische monsters.
Daarnaast zullen we kijken naar gegevens in de United Network for Organ Sharing (UNOS) database: www.unos.org, wat ons in staat zal stellen een grondigere en robuustere analyse te ontwikkelen van de ervaring van de proefpersoon met leverkanker. Dit is een openbaar, verplicht meldinstrument waaraan de overheid alle transplantatiecentra de opdracht geeft zich te melden. Zelfs als de proefpersoon in Northwestern is getransplanteerd en verplaatst, kunnen we door toegang tot UNOS-gegevens zien wat de morbiditeit/sterfte voor die patiënt is.
Ten tweede gaan we op prospectieve wijze vakken inschrijven. Degenen die zijn behandeld, degenen die momenteel een behandeling ondergaan, of degenen die onlangs zijn gediagnosticeerd. Van deze proefpersonen zullen we geïnformeerde toestemming verkrijgen voor; a) de medische dossiers bekijken (huidige, toekomstige en retrospectieve gegevens), b) bloedspecimens verzamelen voor toekomstige analyse en correlatie met hun explantatieglaasjesgegevens, c) ons in staat stellen deze onderwerpen voor onbepaalde tijd te volgen om doorlopende uitkomstgegevens, morbiditeit en mortaliteit te verkrijgen informatie.

Inclusiecriteria:
- Volwassenen, elk geslacht ≥ tot 18 jaar
- Eerdere of huidige diagnose van hepatocellulair carcinoom (klinisch of biopsie bewezen)
- Resectie voor hepatoom en/of transplantatie
- Patiënten met de diagnose hepatocellulair carcinoom van 1-8-1994 tot en met 31-12-2005 (retrospectief) met biopsie, explantatie en/of levertransplantatie hier in het Northwestern Memorial Hospital (NMH).
- Patiënten die eerder zijn gediagnosticeerd of onlangs zijn gediagnosticeerd met leverkanker en die zijn behandeld, worden momenteel behandeld en zullen mogelijk een behandeling voor de ziekte ondergaan.

Het primaire doel is het vergelijken van niveaus van HIF-1alpha-expressie in HCC-tumor-explantaten die het volgende hebben ondergaan: 1) geen pre-explantaat-embolisatie; 2) pre-explantatie Transcatheter arteriële chemo-embolisatie (TACE); 3) pre-explantatie van Y90-radio-embolisatie; 4) radiofrequentie-ablatie vóór explantatie, of 5) een combinatie van pre-explantatietherapieën.
