Managementsamenvatting
Doelen en hypothesen van de studie:
Het doel van de studie is het evalueren van de veiligheid, effectiviteit en vroege uitkomst van de StomaphyX-procedure voor het verkleinen van de vergrote maagzak en stoma bij post-bariatrische patiënten. De studie zal een hypothese testen dat de StomaphyX-procedure effectief zal zijn bij het bevorderen van het verlies van herwonnen gewicht, het verminderen van het risico op comorbiditeiten en het verbeteren van de kwaliteit van leven van patiënten.

Inclusiecriteria:
18-70 jaar, BMI ≥ 35, ≥ 2 jaar na primaire Roux-en-Y gastric bypass met bewijs van vergroot maagzakje of vergrote stomadiameter van ≥ 20 mm via endoscopie of fluoroscopie. Pts vertonen ook een gewichtstoename van 15% van het overtollige lichaamsgewichtverlies.
Uitsluitingscriteria:
Slokdarmvernauwing, of andere anatomische aandoeningen die de doorgang van transorale endoluminale instrumenten verhinderen, heeft een andere oorzakelijke factor voor gewichtstoename dan stoma- of opvangzakjedilatatie, portale hypertensie, stollingsstoornissen of chronisch gebruik van anticoagulantia, elke actieve medische aandoening waardoor de patiënt niet het voltooien van het onderzoek of zou leiden tot een onredelijk risico voor de patiënt.
Interventiebehandeling: creatie van meerdere plicaties van volledige dikte in de maagzak met behulp van het StomaphyX-apparaat (EndoGastric Solutions, Inc., Redmond, WA, VS)
Beoordelingscriteria Primair resultaat: verandering in lichaamsgewicht na 6 en 12 maanden Secundair resultaat: beoordeling van het volume van de maagzak, bepaling van de stomadiameter, scores voor kwaliteit van leven, middelomtrek, oplossen van comorbiditeit Beoordeling van effectiviteit en veiligheid Klinisch: gewicht, lengte, middelomtrek vragenlijsten over kwaliteit van leven (impact van gewicht QOL-Lite, eet- en gewichtspatronen, drie-factor eten, emotionele eetschaal, GI QoL, GSRS en/of PGWB) Anatomisch: endoscopie van het bovenste deel van het maagdarmkanaal, computertomografie of bariumradiografie Biochemisch: bloed- en urinetesten (glucose, cholesterol, triglyceride, HDL, LDL, urinezuur) Veiligheid: bijwerkingen, complicaties, comorbiditeiten, ziekenhuisopname

Beschrijving
Inclusiecriteria:
- Leeftijd 18-70 jaar
- BMI 35 of hoger met een of meer comorbiditeiten
- Minstens 2 jaar post-Roux-en-Y gastric bypass-operatie
- Heeft bij inschrijving ten minste 15% van het overtollige lichaamsgewicht verloren
- Vergrote stomadiameter
- Vergrote maagzak
- Succesvolle voedingsscreening afgerond en in overeenstemming met voedingsprogramma's
- Voltooide succesvolle cardiopulmonale evaluatie
- Patiënt bereid mee te werken aan vervolgonderzoeken
- Ondertekende geïnformeerde toestemming
Uitsluitingscriteria:
- Slokdarmvernauwing of andere anatomische aandoeningen die de doorgang van transorale endoluminale instrumenten verhinderen
- Heeft een andere oorzakelijke factor voor gewichtstoename dan stoma- of opvangzakjedilatatie, zoals het niet volgen van voedings- en bewegingsprogramma's
- Portale hypertensie
- Stollingsstoornissen of chronisch gebruik van antistollingsmiddelen
- Elke actieve medische aandoening waardoor de patiënt het onderzoek niet kan voltooien of die een onredelijk risico voor de patiënt met zich meebrengt

StomaphyX met SerosaFuse-bevestigingsmiddelen (EndoGastric Solutions, Inc., Redmond, WA, VS) is een transoraal incisieloos bevestigingsapparaat dat GI-weefselbenadering en het creëren van volledige dikte (serosa-naar-serosa) plicaties mogelijk maakt.
