Het is onze ervaring dat het plaatsen van postpylorische buisjes moeilijk kan zijn en dat deze buisjes vaak verstopt raken en moeilijk te vervangen zijn, wat resulteert in een verloren calorie-inname voor de patiënt. De populatie waar dit het meest relevant is, zijn kinderen die niet-invasieve positieve drukbeademing (NPPV) krijgen. NPPV is steeds populairder geworden in de populatie van de Pediatrische Intensive Care Unit (PICU), omdat het wordt gezien als een veilig en effectief alternatief voor mechanische beademing via een endotracheale tube. Historisch gezien is NPPV gebruikt bij kinderen met chronische respiratoire insufficiëntie, maar de toepassing ervan voor acuut respiratoir compromis neemt toe. Het is aangetoond dat het effectief is bij ziektetoestanden die verband houden met hypoventilatie en wordt nu ook toegepast bij ademhalingsproblemen die leiden tot verminderde zuurstoftoevoer. Baby's en kinderen die niet-invasieve beademing krijgen voor respiratoire insufficiëntie, die vaak van infectieuze oorsprong is, hebben een aanzienlijke caloriebehoefte. Het is echter onze gewoonte om bij deze patiënten geen maagvoeding toe te staan vanwege de angst voor maagzwelling en braken, wat een risico op aspiratie met zich meebrengt.
Er zijn geen gegevens beschikbaar over de werkzaamheid en veiligheid van maagvoeding bij ernstig zieke kinderen ondersteund door niet-invasieve beademing. Gezien het gemak van (ver)plaatsing en het potentiële voedingsvoordeel van eerdere voeding via maagvoeding, lijkt het klinisch belangrijk om vraagtekens te plaatsen bij de vooringenomenheid tegen deze route van enterale voeding bij niet-invasief beademde patiënten. De onderzoekers stellen daarom een gerandomiseerde pilotstudie voor van maag- versus postpylorische voedingen bij patiënten op NPPV.

Beschrijving
Inclusiecriteria:
- Leeftijd onder de 18 jaar
- Acuut ademhalingsfalen als indicatie voor niet-invasieve beademingsondersteuning
- Ondertekende toestemming van ouder of voogd
- Patiënten met een geplaatste voedingssonde die > 12 uur niet zijn gevoed
Uitsluitingscriteria:
- Onmiddellijke postoperatieve hartchirurgie
- Chronische beademingsondersteuning
- Opnamediagnose aspiratiepneumonie
- Bekende geschiedenis van frequente aspiratie (meer dan 2 eerdere opnames voor deze diagnose)
- Contra-indicatie voor het plaatsen van een voedingssonde (bijv. basale schedelfractuur)
- Dringende behoefte aan endotracheale intubatie
- Percutane maagsonde op zijn plaats
- Geschiedenis van Nissen-fundoplicatie
- Contra-indicatie voor het bestuderen van voedingsformules (bijv. galactosemie)
- Allergie voor metoclopramide
- Geen ondertekende toestemming van ouder of voogd
- Continue positieve luchtwegdruk (CPAP) met of zonder drukondersteuning toegediend via een endotracheale tube
- Gecorrigeerde zwangerschapsduur onder 38 weken
- Patiënten met een geplaatste voedingssonde die in de afgelopen 12 uur zijn gevoed
- Patiënten met een geplaatste voedingssonde bij wie het PICU-personeel de positie van de voedingssonde niet wil veranderen (d.w.z. niet akkoord gaan met het randomiseren van de positie van de voedingssonde van de patiënt)
