Studie van continue dosering van Sunitinib bij niet-GIST-sarcomen met gelijktijdige radiotherapie
Fase I-onderzoek naar continue dosering van sunitinib bij niet-GIST-sarcomen met gelijktijdige radiotherapie
Het doel van deze studie is om de veiligheid te bepalen van continue dosering van sunitinib in combinatie met radiotherapie bij patiënten met niet-GIST (gastro intestinale stromale tumor) sarcomen die niet chirurgisch kunnen worden behandeld.
Het primaire doel van de studie is het bepalen van de maximaal getolereerde dosis (MTD) van continue dosering van sunitinib in combinatie met radiotherapie bij patiënten met niet-GIST-sarcomen die niet chirurgisch kunnen worden behandeld.
Deze studie is een multicenter, open-label fase I met dosisescalatie: 2 dosisniveaus.
Op elk dosisniveau worden 3-6 patiënten opgenomen.3-18 patiënten zullen worden opgenomen in de studie.

Stap 1: 25 mg eenmaal daags Stap 2: 37,5 mg eenmaal daags
Er zullen 3-6 patiënten worden geïncludeerd op elk van de sunitinib-dosisniveaus, afhankelijk van het aantal DLT's (dosisbeperkende toxiciteit) dat optreedt binnen 14 weken na start van de behandeling
DLT wordt gedefinieerd als:
elke graad 3 of 4 musculoskeletale of cutane toxiciteit binnen het stralingsveld elke andere toxiciteit > of = 4
Secundaire doelstellingen zijn:
- om de veiligheid met late toxiciteiten te evalueren
- om het responspercentage op 6 maanden te schatten
- om de progressievrije overleving te schatten
- om het percentage patiënten met een operabele tumor na behandeling te evalueren
Verkennende doelstellingen zijn:
- om de evolutie tijdens de behandeling van neo-angiogenese te bestuderen, gemeten met dynamische contrastversterkte echografie (DCE-US)
- om de correlatie te bestuderen tussen klinische respons en veranderingen van tumorperfusie gemeten door DCE-US

Beschrijving
Inclusiecriteria:
- Mannelijke of vrouwelijke patiënten ouder dan 18 jaar
Histologisch of cytologisch (in geval van recidief) bevestigd bindweefselneoplasma, inclusief een van de volgende subtypes:
- liposarcomen
- Fibrosarcoom, myxofibrosarcoom
- Ongedifferentieerd pleomorf sarcoom
- Leiomyosarcomen
- Alleen pleomorfe rabdomyosarcomen
- angiosarcomen
- Onzekere gedifferentieerde tumoren: synoviale sarcomen, epithelioïde sarcomen, alveolaire sarcomen, sarcomen met heldere cellen.
of diagnose osteosarcoom, chondrosarcoom of chordoom.
- Lokaal gevorderde of lokaal terugkerende niet-operabele tumor zonder voorafgaande bestraling [de status van niet-operabele moet worden beoordeeld door personeel, waaronder een chirurg die gespecialiseerd is in sarcoom].
- Geen voorafgaande behandeling met sunitinibmalaat
- Levensverwachting > 6 maanden
- ECOG-prestatiestatus ≤ 2
Bloedonderzoek, nier- en leverfuncties in het normale bereik met, in de 7 dagen voorafgaand aan het begin van het onderzoek, bloed- of serumwaarden als volgt:
- Absoluut aantal neutrofielen ≥ 1,5 G/L
- Aantal bloedplaatjes ≥ 100 G/L
- Bilirubine ≤ 1,5 mg/dl
- PT en INR ≤ 1,5 maal de bovengrens van normaal [Patiënten onder preventieve antistollingstherapie mogen deelnemen]
- ASAT en ALAT ≤ 2,5 maal de bovengrens van normaal
- Creatinine ≤ 150 umol/L
- Calcium ≤ 12 mg/dL
- Bloedglucose < 150 mg/dL
- Vruchtbare patiënten moeten effectieve anticonceptie gebruiken vóór, tijdens en gedurende 28 dagen na voltooiing van de onderzoekstherapie
- Mogelijkheid om orale medicatie in te slikken
- Verplichte aansluiting bij een zorgverzekeraar
- Ondertekende schriftelijke geïnformeerde toestemming.
Uitsluitingscriteria:
- GIST, Ewing-sarcoom of embryonale rabdomyosarcomen
- Stralingsveld inclusief long, darm of centraal zenuwstelsel
- Reeds bestaande schildklierafwijking, gedefinieerd als abnormale schildklierfunctietests ondanks medicatie
- NCI graad ≥ 3 bloeding in de afgelopen 4 weken voorafgaand aan de toediening van het onderzoeksgeneesmiddel
- Significante cardiovasculaire ziekte (New York Heart Association (NYHA) > graad 2 congestief hartfalen, myocardinfarct binnen 6 maanden voorafgaand aan opname, instabiele angina pectoris, ernstige hartritmestoornissen, ernstig cerebrovasculair accident binnen 6 maanden voorafgaand aan opname, voorgeschiedenis van ernstige trombo-embolie (pulmonale embolie of diepe veneuze trombose DVT) binnen 6 maanden voorafgaand aan opname (patiënten met een recente voorgeschiedenis van DVT behandeld met anticoagulantia (behalve therapeutische warfarine) gedurende ten minste 6 weken komen in aanmerking), verlengd QTc-interval (QTc > 480 msec met Bazett), bradycardie (hartslag < 45 bpm), elektrolytische problemen (hyponatriëmie < 120 mmol/l, kaliëmie ≥ 6 mmol/l) of ongecontroleerde hypertensie terwijl u de juiste medicatie krijgt (≥ 160 mm Hg systolisch en/of ≥ 90 mm Hg diastolisch).
- Minder dan 6 weken tussen eerdere neoplastische behandeling met tyrosinekinaseremmer en opname en minder dan 4 weken voor andere neoplastische behandelingen
- Grote chirurgische ingreep, open biopsie of ernstige niet-genezende wond binnen 28 dagen voorafgaand aan de eerste behandelingsdag
- Gelijktijdige deelname aan een ander klinisch onderzoek
Andere ziekten of aandoeningen in de afgelopen 6 maanden voorafgaand aan de toediening van het onderzoeksgeneesmiddel, waaronder de volgende:
- Psychiatrische ziekte of sociale situatie die studietrouw in de weg zou staan
- Bekende aan het humaan immunodeficiëntievirus (hiv) of verworven immunodeficiëntiesyndroom (aids) gerelateerde ziekte of andere actieve infectie
- Bekende hersenmetastasen, compressie van het ruggenmerg of carcinomateuze meningitis, of tekenen van symptomatische hersen- of leptomeningeale ziekte
- peritoneale carcinose
- aantal metastatische sites > 2
- Vrijheidsbeperking door rechterlijke of administratieve beslissing
- Zwangere of zogende vrouwen

EXPERIMENTEEL: vereniging sunitinib radiotherapie
|
Alle patiënten zullen worden behandeld met sunitinib (2 dosisniveaus) eenmaal daags ('s ochtends) gedurende 6 weken in combinatie met radiotherapie. Radiotherapie zal 1-4 uur na inname van sunitinib worden uitgevoerd. Dosisniveau 1: 25 mg eenmaal daags Dosisniveau 2: 37,5 mg eenmaal daags Er wordt alleen toestemming gegeven om een patiënt in de bovenste trede op te nemen als de deadline van 14 weken na de start van de behandeling van de laatste opgenomen patiënt strikt werd gerespecteerd en afhankelijk van aantal optredende DLT. |
