STUDIE DOELSTELLINGEN:
Primair:
Het primaire doel is het evalueren van het volledige responspercentage na 4 cycli van FCR-Lite plus lenalidomide bij niet eerder behandelde patiënten met CLL.
Ondergeschikt:
Het eerste secundaire doel is het evalueren van de toxiciteit van patiënten met niet eerder behandelde CLL die werden behandeld met FCR-Lite plus lenalidomide, gevolgd door lenalidomide. De tweede is het evalueren van het algehele responspercentage en de algehele overleving van patiënten met niet eerder behandelde CLL die werden behandeld met FCR-Lite plus lenalidomide gevolgd door lenalidomide. De derde is om te bepalen of het toevoegen van lenalidomide als consolidatie-/onderhoudstherapie minimale restziekte van het beenmerg bij CR-patiënten zal elimineren en of patiënten die een PR hebben na 6 cycli van FCR-Lite plus lenalidomide zullen reageren op 12 maanden lenalidomide. Het uiteindelijke secundaire doel is om te bepalen of de expressie van ZAP-70, CD38 en chromosomen correleert met responspercentage, responsduur en overleving voor niet eerder behandelde patiënten met CLL.
STUDIE ONTWERP:
2-traps fase 2 onderzoeksopzet. 19 proefpersonen worden in stadium 1 behandeld met FCR-Lite plus 5 mg lenalidomide, oplopend tot 10 mg en 15 mg in volgende cycli, afhankelijk van de toxiciteit. Als er ten minste 5 CR's zijn, zal het onderzoek nog eens 35 proefpersonen opleveren (zie statistische sectie). Een secundair doel van deze studie zal zijn om te bepalen of MRD-positieve patiënten MRD-negatief zullen worden met lenalidomideconsolidatie/-onderhoud en of PR-patiënten zullen overgaan op CR's. Lenalidomide begint 2 maanden na de laatste dosis FCR-Lite bij alle proefpersonen met CR. Het kan al 1 maand na FCR-Lite plus lenalidomide beginnen bij proefpersonen met PR. Lenalidomide wordt gegeven in cycli van 28 dagen waarbij de dosis wordt verhoogd van 5 mg/d tot 10 mg/d in cyclus 2 en tot 15 mg in cycli 3-6 indien goed verdragen (geen graad 3 of -4 toxiciteit). Patiënten met een creatinineklaring ≥ 30 ml/min en < 60 ml/min starten met 2,5 mg per dag, oplopend tot 5 en 10 mg in volgende cycli. Verlaging tot de eerdere dosis is toegestaan voor graad 3/-4 toxiciteit. MRD zal worden bestudeerd door middel van flowcytometrie van beenmerg- en perifere bloedmonsters na 4 en 6 cycli van FCR-Lite en na 6 en 12 maanden lenalidomide bij CR-patiënten.

Inclusiecriteria:
Patiënten moeten de diagnose CLL hebben (zoals gedefinieerd door de onderstaande NCI-criteria:
- Patiënten moeten een absoluut aantal lymfocyten in het perifere bloed hebben van >5.000/mm3 verkregen binnen 2 weken voorafgaand aan de start van het onderzoek.
- De lymfocytose moet bestaan uit kleine, rijpe lymfocyten, met ≤55% (niet meer dan 55%) prolymfocyten.
Patiënten moeten fenotypisch gekarakteriseerde CLL hebben zoals gedefinieerd als:
- De overheersende celpopulatie deelt B-celantigenen met CD5 in afwezigheid van andere pan-T-celmarkers (CD3, CD2, enz.);
- Oppervlakte-immunoglobuline (slg) en CD20 met expressie met een lage celoppervlakdichtheid.
- Als oppervlakte-immunoglobuline kan worden aangetoond, zijn de leukemische cellen beperkt tot expressie van kappa of lambda.
- Splenomegalie, hepatomegalie of lymfadenopathie zijn niet vereist voor de diagnose van CLL
- Patiënten moeten chemotherapie nodig hebben
- Patiënten mogen geen eerdere behandeling met cytotoxische middelen, immunotherapie of experimentele therapie hebben ondergaan.
- Patiënten mogen geen voorgeschiedenis hebben van behandeling met corticosteroïden voor CLL, auto-immune trombocytopenie of auto-immuun hemolytische anemie.
- Berekende creatinineklaring ≥30 ml/min volgens de Cockcroft-Gault-formule
- Bilirubine moet ≤1,5 mg/dl zijn, tenzij secundair aan tumor, verkregen binnen 2 weken voorafgaand aan registratie
- Bloedplaatjes ≥75x109/L, tenzij door CLL-betrokkenheid van het beenmerg
- Neutrofielen ≥1,5x109/L, tenzij door CLL-betrokkenheid van beenmerg
- AST of ALT < 2x bovengrens van normaal, tenzij gerelateerd aan CLL
- Leeftijd ≥18 jaar
- ECOG-prestatiestatus 0-2
- Vrouwtjes die zwanger kunnen worden (FCBP) moeten een negatieve serum- of urinezwangerschapstest hebben
- Mannen moeten ermee instemmen een latexcondoom te gebruiken tijdens seksueel contact met een FCBP, zelfs als ze een succesvolle vasectomie hebben gehad
- In staat om dagelijks aspirine (81 mg of 325 mg) te nemen als profylactische antistolling
- De proefpersoon moet schriftelijke geïnformeerde toestemming geven
- Alle deelnemers aan de studie moeten geregistreerd zijn in het verplichte RevAssist®-programma en bereid en in staat zijn om te voldoen aan de vereisten van RevAssist®
Uitsluitingscriteria:
- Patiënten met auto-immuun hemolytische anemie of auto-immuuntrombocytopenie komen niet in aanmerking
- Geen eerdere immunotherapie, experimentele of cytotoxische chemotherapie
- Patiënten met een voorgeschiedenis van behandeling met steroïden voor CLL/SLL auto-immune hemolytische anemie of auto-immune trombocytopenie komen niet in aanmerking
- Patiënten met actieve infecties die orale of intraveneuze (IV) antibiotica nodig hebben totdat de infectie is verdwenen en de therapeutische antibiotica zijn voltooid
- Vrouwen die zwanger kunnen worden en seksueel actieve mannen die beiden weigeren een geaccepteerde en effectieve anticonceptiemethode te gebruiken of vrouwen die borstvoeding geven
- Patiënten met een tweede maligniteit anders dan basaalcelcarcinoom of plaveiselcelcarcinoom van de huid of in situ cervixcarcinoom komen niet in aanmerking tenzij de tumor minimaal twee jaar eerder curatief is behandeld
- Geschiedenis van bekende HIV
- Geschiedenis of aanwezigheid CZS-aandoening
- Bewijs van laboratorium-TLS door Cairo-Bishop-definitie van tumorlysissyndroom
- Geschiedenis van behandeling met corticosteroïden voor CLL, auto-immune trombocytopenie of auto-immuun hemolytische anemie.

Ander: FCR met Lenalidomide
Fludarabine, Cyclofosfamide, Rituximab, Lenalidomide - 19 proefpersonen werden in stadium 1 behandeld met FCR plus 5 mg lenalidomide, oplopend tot 10 mg en 15 mg in volgende cycli, afhankelijk van de toxiciteit.
Als er ten minste 5 CR's zijn na 4 cycli van FCR plus lenalidomide, zal het onderzoek nog eens 35 proefpersonen opleveren.

19 proefpersonen worden in stadium 1 behandeld met FCR plus 5 mg lenalidomide, oplopend tot 10 mg en 15 mg in volgende cycli, afhankelijk van de toxiciteit.
