Carboplatine en combinatiechemotherapie met of zonder veliparib bij de behandeling van patiënten met stadium IIB-IIIC borstkanker

HOOFDDOEL:
1) Het vergelijken van de pathologische complete respons (pad CR) bij patiënten met stadium IIB of stadium III triple-negatieve borstkanker behandeld met neoadjuvant paclitaxel en carboplatine met het pad CR van patiënten behandeld met paclitaxel, carboplatine en veliparib.
SECUNDAIRE DOELSTELLINGEN:
- Terugvalvrije overleving (vervolgperiode van 36 maanden).
- Algehele klinische respons op neoadjuvante therapie.
OVERZICHT: Patiënten worden gerandomiseerd naar 1 van de 2 behandelingsarmen.
ARM I: Patiënten krijgen paclitaxel intraveneus (IV) en carboplatine IV op dag 1 (alleen kuur 1) of dag 2 (kuur 2-12). De behandeling wordt om de 7 dagen herhaald gedurende 12 kuren bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit. Vanaf 21 dagen na de laatste kuur krijgen patiënten op dag 1 doxorubicinehydrochloride IV en cyclofosfamide IV. De behandeling wordt elke 21 dagen gedurende 4 kuren herhaald bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.
ARM II: Patiënten krijgen veliparib oraal (PO) tweemaal daags (BID) op dag 1-5. Patiënten krijgen ook paclitaxel IV en carboplatine IV op dag 3 (alleen kuur 1) of dag 4 (kuur 2-12). De behandeling wordt om de 7 dagen herhaald gedurende 12 kuren bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit. Vanaf 21 dagen na de laatste kuur krijgen patiënten op dag 1 doxorubicinehydrochloride IV en cyclofosfamide IV. De behandeling wordt elke 21 dagen gedurende 4 kuren herhaald bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.
Na voltooiing van de studiebehandeling worden patiënten gedurende 36 maanden elke 3 maanden opgevolgd.

Inclusiecriteria:
- Schriftelijke geïnformeerde toestemming moet worden verkregen voorafgaand aan studiegerelateerde procedures.
- Histologisch bevestigd adenocarcinoom van de borst met de volgende markers: oestrogeenreceptor negatief (<1%), progesteronreceptor negatief (<1%) en Her-2/neu negatief (Her-2/neu 0-1+ IHC of FISH ratio <1,8 of gemiddeld aantal HER2-genkopieën van <vier signalen/kern voor testsystemen zonder interne controleprobe).
- Vrouw ≥ 18 jaar oud.
- Klinische stadium IIA (T2N0), IIB (T2N1, T3N0) of stadium IIIA (T1N2, T2N2, T3N1, T3N2), IIIB of IIIC borstkanker zonder voorafgaande behandeling.
Volledige radiologie- of tumorbeoordeling binnen 28 dagen voorafgaand aan inschrijving
- Borst MRI
- Eenzijdige echografie van de borst
- Metastatische work-up op afstand voltooid met PET/CT.
- Als vergrote oksellymfeklieren worden gevonden tijdens stadiëringsscans, moet FNA worden uitgevoerd om te bepalen of de knoop betrokken is bij kanker.
- Als de oksellymfeklieren klinisch negatief zijn tijdens de initiële opwerking, wordt een schildwachtklierbiopsie uitgevoerd voorafgaand aan de start van de chemotherapie.
- ECOG-prestatiestatus van 0 of 1
Adequate orgaan- en hematologische functie zoals blijkt uit de volgende laboratoriumonderzoeken binnen 4 weken na inschrijving voor het onderzoek:
- Cardiale ejectiefractie >/= ondergrens van normaal zoals bepaald door 2D-echo of MUGA-scan volgens institutionele normen.
- Hematologische functie, als volgt: absoluut aantal neutrofielen ≥ 1,5 x 109/l, aantal bloedplaatjes ≥ 100 x 109/l en ≤ 850 x 109/l, hemoglobine ≥ 9 g/dl, PTT en INR < 1,5 x ULN.
- Nierfunctie, als volgt: serumcreatinine </= 1,4 mg/dL).
- Leverfunctie, als volgt: aspartaataminotransferase (ASAT) ≤ 2,5 x ULN, alanineaminotransferase (ALAT) ≤ 2,5 x ULN, totaal bilirubine ≤ 2 x ULN (behalve voor patiënten met UGT1A1-promoterpolymorfisme, d.w.z. Gilbertsyndroom, bevestigd door genotypering of Invader UGT1A1 moleculaire assay voorafgaand aan inschrijving in het onderzoek. Patiënten met het syndroom van Gilbert moeten een totaal bilirubine < 3 ULN hebben).
- De patiënt moet bereid en in staat zijn om een MRI te ondergaan zoals beschreven in het protocol.
Uitsluitingscriteria:
- Bekende overgevoeligheid voor doxorubicine, cyclofosfamide, paclitaxel, cremophor of medicijnen die cremophor bevatten (miconazol, docetaxel, sandimmune, nelfinavirmesylaat, propofol, diazepam-injectie, vitamine K-injectie, ixabepilon, aci-jel) of carboplatine.
- Bekende HIV of actieve hepatitis B- of C-infectie.
- Voorafgaande behandeling voor de momenteel gediagnosticeerde borstkanker.
- Voorafgaande behandeling met doxorubicine tot 400 mg/m2.
- Reeds bestaande graad 3 of 4 sensorische neuropathie.
- Geschiedenis van bloedingsdiathese of uitgebreide bloeding die bloedtransfusie vereist binnen 14 dagen na inschrijving.
- Grote chirurgische ingreep binnen 4 weken (28 dagen) voorafgaand aan inschrijving (plaatsing van een poort wordt niet als een grote chirurgische ingreep beschouwd).
- Klinisch significante hartziekte binnen 12 maanden na deelname aan de studie, waaronder een myocardinfarct, onstabiele angina pectoris, congestief hartfalen of aanhoudende aritmieën die medicatie of een pacemaker vereisen.
- Niet-genezende wond, zweer of breuk.
- Lopende of actieve infectie.
- Zwanger (d.w.z. positieve bèta-humaan choriongonadotrofinetest) of borstvoeding geven
- Niet bereid om een zeer effectieve methode van anticonceptie te gebruiken (d.w.z. die resulteren in lage faalpercentages, minder dan 1% per jaar), gedefinieerd als intra-uteriene apparaten, barrièremethoden (condooms, anticonceptiesponsjes, diafragma's, vaginale ringen gebruikt met zaaddodende gelei of crèmes), orale anticonceptiepillen of seksuele onthouding. Tijdens het onderzoek moet anticonceptie worden gebruikt.

Algehele klinische respons

|
Het aantal en het percentage proefpersonen met elke categorie van algehele klinische respons zal worden samengevat op basis van de aanwezigheid van meetbare ziekte bij aanvang (d.w.z. volledige respons [CR], partiële respons [PR], stabiele ziekte [SD], progressieve ziekte [PD], niet in staat evalueren [UE], neurogeneratieve ziekte [ND]). Geëvalueerd volgens responsevaluatiecriteria bij solide tumoren (RECIST v1.0) zoals beoordeeld b MRI: volledige respons (CR), verdwijning van alle doellaesies; Gedeeltelijke respons (PR), >=30% afname in de som van de langste diameter van doellaesies; Algehele respons (OR) = CR + PR. Bèta zal worden gebruikt als priors voor combinatieregimes bij het berekenen van de posterieure verdeling van de pathologische complete respons [pCR] voor elke respectievelijke behandelingsgroep. Onder proefpersonen met meetbare ziekte zal een 95% geloofwaardig gebied worden berekend voor de odds ratio voor elke behandelingscombinatie ten opzichte van elkaar. |
