Beschrijving
Inclusiecriteria:
-> 18 jaar (M/V)
-Diagnose van cirrose: i. Door leverbiopsie ii. Of een sterke verdenking van cirrose volgens geaccepteerde criteria voor de klinische diagnose van cirrose (bijv. perifeer oedeem of spataderen, palpeerbare harde linker leverkwab, kleine overspanning van de rechter kwab of palpabele splenomegalie), en/of radiologisch bewijs van cirrose (door abdominale echografie, CT of MRI, die een nodulaire lever en/of portosystemische collateralen met portale aderdoorgankelijkheid en/of ascites en/of splenomegalie en/of colloïdverschuiving op een colloïd-isotoop lever-milt scan) en/of laboratoriumvariabelen (bloedplaatjes 1.3) iii. Aanwezigheid van slokdarmvarices (zonder eerdere episode van varicesbloeding) of HVPG≥10 mmHg 12 mmHg van eerdere testen.
-Is gepland voor hemodynamische studietesten, inclusief respons op acute therapie.-
Uitsluitingscriteria:
- Patiënten die al bètablokkers krijgen
- Hepatocellulair carcinoom
- Trombose van de poortader
- Contra-indicaties voor niet-selectieve bètablokker Propanolol
- Cholestatische leverziekte
- Ernstige hart-, long- of nierziekte.
- Patiënt heeft eerder een chirurgische bypassoperatie ondergaan voor morbide obesitas
- Patiënt heeft uitgebreide dunnedarmresectie
- Elke grote operatie in de afgelopen 3 maanden.
- Patiënt is een ontvanger van een orgaantransplantatie
- Patiënt allergisch voor paracetamol.
- Patiënten die hepatotoxische geneesmiddelen gebruiken
- De patiënt mag, op basis van de mening van de onderzoeker, niet worden opgenomen in dit onderzoek.
- Patiënten die geïnformeerde toestemming niet kunnen of willen ondertekenen
- Patiënten die deelnemen aan andere klinische onderzoeken ter evaluatie van experimentele behandelingen of procedures of die in de afgelopen 3 maanden hebben deelgenomen aan een klinische studie.

Experimenteel: Methacetin (voor BT) voor/na behandeling met propanolol.
De methacetine-ademtest (MBT) wordt uitgevoerd voor en na (binnen 2 uur) injectie van de dosis Propanolol (BB) en nogmaals na 60 dagen orale toediening van Propanolol.

De methacetine is verrijkt met koolstof 13, een stabiele niet-radioactieve isotoop, en is veilig bevonden in honderden onderzoeken.
Het materiaal lijkt erg op paracetamol.
Er wordt 75 milligram (mm) van dit materiaal gebruikt, vooraf opgelost in water.
Dit materiaal ondergaat een snel metabolisme door de lever.

De veranderingen tussen MBT op baseline-niveau en na chronische behandeling met Propranolol

MBT-correlatie met klinische gebeurtenissen na 2 maanden behandeling

De MBT zal opnieuw worden uitgevoerd bij proefpersonen na 60 dagen orale bètablokkerbehandeling en de resultaten zullen worden vergeleken met eerdere MBT-waarden.
