Focale corticale atrofie na myocardiale interne capsule (ACFI)
FOCALE CORTISCHE ATROFIE NA MYOCARDIALE INTERNE CAPSULE: CORRELATIEFRACTIE ANISOTROPIE STRAALPIRAMIDE EN HANDICAP
Het primaire doel van de proef is het aantonen van het ontstaan van focale corticale atrofie, gelokaliseerd in het ipsilaterale primaire motorische gebied, gemeten in mm, drie maanden na een infarct van het interne kapsel.
De patiënt wordt tussen dag nul tot tien en drie maanden met zichzelf vergeleken.
De onderzoekshypothesen zijn:
- Een focale corticale atrofie van het ipsilesionale primaire motorgebied treedt op na een herseninfarct van het interne kapsel. Na drie maanden is het nauwkeurig en reproduceerbaar te meten met MRI. Ook andere hersengebieden binnen de vrijwillige motoriek worden onderzocht (aanvullend motorisch gebied, pre-motorisch gebied).
- Deze atrofie is gecorreleerd met het bereiken van het piramidale kanaal, beoordeeld aan de hand van de fractionele anisotropie van de vezels.
- Deze atrofie is gecorreleerd met invaliditeit na drie maanden, beoordeeld aan de hand van de Rankin-score.
Studie Overzicht
Gedetailleerde beschrijving
Tweeëntwintig patiënten met ischemische beroerte, subcorticaal, waarbij de interne capsule is betrokken, zullen worden opgenomen.
Elke patiënt wordt met zichzelf vergeleken, op dag nul tot tien en drie maanden.
Er zal een MRI worden uitgevoerd tussen dag nul en tien, daarna drie maanden van infarctsequenties met traditionele driedimensionale opname en tractografie mogelijk maken en De meting van de corticale dikte is in mm De meting van fractionele anisotropie van het piramidale kanaal Een klinische evaluatie zal worden uitgevoerd tussen Dag nul en dag tien dan drie maanden, met behulp van een neurologische score (NIHSS), een score van functionele beperkingen (Rankin) en meer analytische en gevoelige tests zoals dynamometer, de Finger Tapping-test en test nieuwe pluggen.

Beschrijving
Inclusiecriteria:
- Patiënten van 18 tot 90 jaar.
- Subcorticale beroerte
- Voltooiing van de interne capsule, volgens MRI
- Mogelijkheid om MRI uit te voeren binnen 10 dagen na het begin van de symptomen
- NIHSS groter dan of gelijk aan 2 en < 20
- Rankin Score groter dan of gelijk aan 1 en ≤ 5
- Schriftelijke geïnformeerde toestemming na informatie over het protocol, van patiënten of betrouwbare persoon als patiënt niet in staat is om te ondertekenen
- Aansluiting bij een sociale zekerheidsregeling
Uitsluitingscriteria:
- Zwangere of zogende vrouwen
- Andere cerebrale laesie, gelijktijdig of reeds bestaand
- Gelijktijdige ziekte die een ongunstige prognose veroorzaakt binnen 3 maanden na inclusie
- reeds bestaande psychiatrische aandoening
- Alcoholisme of andere chronische intoxicatie
- Corticale lokalisatie van het infarct
- Patiënt in coma, die niet kan worden onderzocht en geëvalueerd
- Patiënt geïntubeerd, beademd, verdoofd
- Hersenbloeding, intra-parenchymaal en/of subarachnoïd
- Door de wet beschermde personen (voogdij, curatoren en rechtsbescherming)
- Contra-indicaties voor onderzoek met magnetische resonantie: pacemaker, metalen implantaten, neurostimulatoren, clips neurochirurgische draadhechtingen, nietjes, metalen hartkleppen, ventriculaire bypassklep, metaalbewerkers, vreemd oog, granaatscherven, kogels, cochleaire implantaten
