Validatie van capnografie als voorspeller van cardiale outputverandering zoals gemeten door FloTrac
Het doel van deze studie is het evalueren van de correlatie van capnografie met niet-invasieve meting van het hartminuutvolume met de FloTrac/EV1000 na een omkeerbare vloeistofprovocatie, een passieve beenhefmanoeuvre, waarbij thermodilutie als gouden standaard wordt gebruikt.
De hoofdhypothese is een correlatie van 0,8 tussen de toename van ETCO2 en de toename van het uitwerpvolume gemeten door FloTrac/EV1000 na een passieve beenhefmanoeuvre.

Er worden gegevens verzameld over medische aandoeningen in het verleden, medicatie, type operatie, afwijkingen op pre-operatief transoesofageaal echocardiogram en lokalisatie van arteriële lijn.
Voordat de anesthesie wordt geïnduceerd, wordt een arteriële lijn geïnstalleerd. De agentia van inductie van anesthesie en doses worden overgelaten aan het oordeel van de behandelend anesthesioloog. De trachea is geïntubeerd, het anesthesiecircuit en de sidestream CO2-sensor zijn aangesloten op de endotracheale tube en positieve drukbeademing begint met gestandaardiseerde parameters: gecontroleerde geassisteerde modus zonder inademingsinspanning, ademvolume van 6-8 ml/kg, ademhalingsfrequentie van 8-10 /minuut en PEEP van 5 mmHg. Wijziging van de ventilatieparameters is niet toegestaan voor en tijdens de PLR-manoeuvres. Isofluraan en propofol worden gebruikt om de anesthesie in stand te houden. Een PA-katheter (Paceport, Edwards Lifesciences, Irvine, Californië, VS) wordt ingebracht in de rechter interne halsader. Als er een femurlijn is geïnstalleerd, wordt de FloTrac/EV1000 aangesloten op de femurlijn, zo niet, dan wordt de radiale lijn gebruikt voor mini-invasieve continue bewaking van het hartminuutvolume. Het a- en v-golfaspect en de verhouding van de CVP-golfvorm worden genoteerd. De RV-golfvorm wordt geïnspecteerd op tekenen van afwijkingen, zoals een niet-horizontaal hellingsbeperkend patroon (Afbeelding xx)18. De transducers zijn allemaal op midthoracaal niveau geplaatst en voorafgaand aan de CO-metingen met de FloTrac/EV1000 wordt een "spoeltest" uitgevoerd om de afwezigheid van resonantie of demping van het systeem te garanderen.
Bij baseline, in de operatiekamer vóór de eerste PLR-manoeuvre, worden de hartslag, systolische, diastolische en gemiddelde arteriële druk, CVP-waarde, CI in drievoud gemeten met de thermodilutiemethode en de van FloTrac afgeleide variabelen geregistreerd. Deze variabelen omvatten de CO geïndexeerde CO en SVV gemeten door de FloTrac/EV1000. Dezelfde variabelen worden geregistreerd op 1, 3 en 5 minuten na het begin van PLR behalve CI door thermodilutie die slechts eenmaal opnieuw wordt gemeten, op 3 minuten en EtCO2 die elke 12 seconden wordt geregistreerd. De EtCO2-curve wordt geïnspecteerd bij baseline en tijdens de PLR-manoeuvre om er zeker van te zijn dat de veranderingen in EtCO2 geen verband houden met veranderingen in ingeademde CO2, een storing in het beademingsapparaat of het circuit of een toename in luchtwegweerstand.
Na het ontwennen van CPB worden twee keer dezelfde maatregelen genomen: met geopende borstkas, wanneer hemodynamische stabiliteit is bereikt, en CPB-canules uitgenomen en met gesloten borstkas.
Op de IC werden twee extra PLR-manoeuvres uitgevoerd met een tussenpoos van 30 minuten. De eerder beschreven reeks metingen wordt geregistreerd. Het gebruik van een epicardiale tijdelijke pacemaker, vasopressoren (doses en type), vasodilatatoren (doses en type) worden ook geregistreerd.

- 18 jaar of ouder die een hart- of aorta-operatie hebben ondergaan
Uitsluitingscriteria:
- Pacemaker
- Amputatie van de onderste ledematen of afwezigheid van de onderste ledematen
- Matige tricuspidalisinsufficiëntie
- Preoperatieve aritmie of langdurige aritmie tijdens datametingen
- Matige aortaregurgitatie
- Bekende diepe veneuze trombose
- Ballonpomp in de aorta
