Een multicenter, gerandomiseerde, dubbelblinde, actief gecontroleerde studie met parallelle groepen om de werkzaamheid en veiligheid van LCZ696 te evalueren in vergelijking met Valsartan, over morbiditeit en mortaliteit bij patiënten met hartfalen (NYHA klasse II-IV) met behouden ejectiefractie (PARAGON- HF)
Het doel van deze studie was om het effect van LCZ696 te evalueren in vergelijking met valsartan in de vermindering van cardiovasculaire sterfte en hartfalen (HF) ziekenhuisopnames bij patiënten met HF met behouden ejectiefractie.

Dit was een multicenter, gerandomiseerde, dubbelblinde, actief gecontroleerde studie met parallelle groepen om de werkzaamheid en veiligheid van sacubitril/valsartan te evalueren in vergelijking met valsartan, op morbiditeit en mortaliteit bij patiënten met hartfalen (NYHA klasse II-IV) met geconserveerde uitwerpfractie. Specifiek evalueerde de studie het effect van sacubitril/valsartan in vergelijking met de actieve comparator valsartan in de reductie van het aantal CV sterfte en totale HF ziekenhuisopnames bij patiënten met HFpEF. De proef bestond uit twee perioden: (1) een enkelblinde inloopperiode van de behandeling die 3 tot 8 weken duurde, waarin patiënten valsartan 80 mg tweemaal daags kregen, gevolgd door sacubitril/valsartan 100 mg tweemaal daags en (2) een dubbele -blinde gerandomiseerde behandelingsperiode (sacubitril/valsartan 200 mg tweemaal daags of valsartan 160 mg tweemaal daags). In deze studie waren onderzoekers verantwoordelijk voor het beoordelen en indienen van alle gebeurtenissen die mogelijk zouden kunnen voldoen aan de criteria voor de primaire, secundaire of andere klinische eindpunten bij een Clinical Endpoint Committee (CEC). Door de onderzoeker gerapporteerde gebeurtenissen werden beoordeeld door de CEC voor berechting.
Voor angio-oedeem of angio-oedeem-achtige gebeurtenissen vulden de onderzoekers een formulier Toekenningsvragenlijst voor een angio-oedeem-achtige gebeurtenis in. Alle meldingen van angio-oedeem werden door Novartis ter beoordeling doorgestuurd naar een Angio-oedeem Adjudication Committee (AAC).

Beschrijving
Inclusiecriteria:
- Linkerventrikel-ejectiefractie (LVEF) ≥45% door echo tijdens screeningperiode of binnen 6 maanden voorafgaand aan deelname aan het onderzoek.
- Symptoom(en) van hartfalen (HF) waarvoor behandeling met diureticum(s) voor HF nodig is, ten minste 30 dagen voorafgaand aan deelname aan de studie.
- Huidige symptoom(en) van HF (NYHA klasse II-IV)
- Structurele hartziekte (vergroting van het linker atrium of hypertrofie van het linker ventrikel) gedocumenteerd door echocardiogram.
- Verhoogde NT-proBNP
Uitsluitingscriteria:
- Elke eerdere meting van LVEF < 40%.
- Acuut coronair syndroom (inclusief MI), hartchirurgie, andere grote CV chirurgie binnen 3 maanden, of dringende percutane coronaire interventie binnen 3 maanden of en electieve PCI binnen 30 dagen voorafgaand aan binnenkomst.
- Elke klinische gebeurtenis binnen de 6 maanden voorafgaand aan opname kan de LVEF hebben verlaagd (bijv. MI, CABG), tenzij een na de gebeurtenis uitgevoerde echometing een LVEF ≥45% bevestigt.
- Huidig acuut gedecompenseerd HF waarvoor therapie nodig is.
- Patiënten die behandeling nodig hebben met 2 of meer van de volgende: een angiotensineconverterende enzymremmer (ACEI), een angiotensinereceptorblokker (ARB) of een renineremmer.
- Alternatieve reden voor kortademigheid zoals: significante longziekte of ernstige COPD, hemoglobine (Hgb) <10 g/dl of body mass index (BMI) > 40 kg/m2.
- Systolische bloeddruk (SBD) ≥ 180 mmHg bij aanvang, of SBD >150 mmHg en <180 mmHg bij aanvang, tenzij de patiënt 3 of meer antihypertensiva krijgt, of SBD < 110 mmHg bij aanvang.
Andere in het protocol gedefinieerde opname-/uitsluitingscriteria kunnen van toepassing zijn.

De KCCQ is een gevalideerd instrument voor zelfbeoordeling van de kwaliteit van leven en de gezondheidsstatus bij patiënten met hartfalen (HF).
De klinische samenvattingsscore, die is afgeleid van de symptomendomeinen fysieke beperkingen en hartfalen (HF) van de KCCQ, is een geldige maatstaf voor het beoordelen van de gezondheidsaspecten van de patiënt die kunnen worden beïnvloed door CV medicatie.
Scores worden getransformeerd naar een bereik van 0-100, waarbij hogere scores een betere gezondheidstoestand weerspiegelen.
