Het "EUropean Comparative Effectiveness Research to assessment the use of primary profylacTic Implantable Cardioverter Defibrillators (EU-CERT-ICD)" is een modulair onderzoeksproject om de effectiviteit van profylactische ICD's te bestuderen in een prospectieve studie, een retrospectief register en meta-analyses van bestaand bewijsmateriaal over dit onderwerp.

Het prospectieve studiegedeelte is een observationele, prospectieve multicenter studie die tot doel heeft 2500 patiënten met ischemische of gedilateerde cardiomyopathie en primaire profylactische ICD-indicatie voor verminderde linkerventrikelejectiefractie (LVEF) volgens de huidige richtlijnen te rekruteren.
Volgens de beslissing en voorkeur van de behandelend arts, die onafhankelijk is van deelname aan het onderzoek, worden patiënten in een van twee groepen ingedeeld: patiënten direct voorafgaand aan de eerste ICD-implantatie (ICD-groep), of patiënten die niet in aanmerking komen voor profylactische ICD-behandeling door de arts of worden overwogen, maar weigeren ICD-behandeling (controlegroep).
Patiënten ondergaan een eenvoudig 12-afleidingen elektrocardiogram (ECG) en Holter ECG-diagnostiek bij baseline, evenals genetische biobanking bij aanvang van het onderzoek.
Alle patiënten zullen vervolgens gedurende minimaal 1 jaar en maximaal 4 jaar worden gevolgd.
Co-primaire eindpunten zijn sterfte door alle oorzaken en geschikte ICD-schokken.

Patiënten die in aanmerking komen voor primaire profylactische ICD-implantatie en mogelijk in aanmerking komen voor deelname aan het onderzoek, zullen worden geïdentificeerd op basis van echocardiografische en/of 12-afleidingen ECG-resultaten.
Voor patiënten van de ICD-groep vindt de screening kort voor de geplande ICD-implantatie plaats.
Voor controlegroeppatiënten moet de tijdspanne tussen de primaire diagnose van geschiktheid voor primaire ICD-implantatie en de datum van de primaire diagnose van cardiomyopathie worden vastgelegd in het eCRF, samen met hoe en wanneer de beslissing om geen defibrillator voor te schrijven werd genomen.
De controlegroep zal worden gerekruteerd uit patiënten met ischemische of gedilateerde cardiomyopathieën en LVEF ≤ 35%.
Patiënten die zijn ingepland voor ICD-implantatie (ICD-groep) ondergaan screening en baselinebeoordelingen voorafgaand aan ICD-implantatie.
Patiënten voor de controlegroep ondergaan zo snel mogelijk na de screening basisbeoordelingen.
De basislijnbeoordelingen zijn identiek voor de ICD-groep en de controlegroep.
Beschrijving
Inclusiecriteria:
- Ischemische of gedilateerde cardiomyopathie en aanbeveling voor primaire profylactische ICD-behandeling volgens de huidige internationale behandelingsrichtlijnen
- Schriftelijke geïnformeerde toestemming
- Als ICD-implantatie gepland is, moeten de inschrijving en de basislijntests van het onderzoek worden voltooid voordat de-novo ICD-implantatie plaatsvindt
Uitsluitingscriteria:
- Permanent atriumfibrilleren of atriumfibrilleren bij baseline in het geval dat meer dan 15% van dergelijke patiënten op een bepaald moment zijn ingeschreven
- Indicatie voor secundaire profylactische ICD-behandeling
- Indicatie of kandidaat voor cardiale resynchronisatietherapie
- AV-blok II°-III° bij hartslag in rust
- Geïmplanteerde pacemaker
- Instabiele hartziekte zoals gedecompenseerd hartfalen (NYHA functionele klasse IV) of acuut coronair syndroom
- Deelname aan andere klinische onderzoeken die deelname aan andere onderzoeken uitsluiten

De eerste implantatie van een ICD-apparaat (na basislijnbeoordelingen) maakt geen deel uit van deze observationele studie en wordt uitgevoerd onder de verantwoordelijkheid van de behandelend arts. Alle ICD-apparaten zullen een unieke standaardprogrammering ondergaan om vergelijkbaarheid van ICD-schokgebeurtenissen tussen patiënten te garanderen.
