Gedetailleerde beschrijving
De hoofdhypothese van deze studie is dat de toediening van autologe ex vivo geëxpandeerde HIV-specifieke T-cellen (HXTC's) geprimed om meerdere HIV-1-antigenen te herkennen bij HIV-geïnfecteerde deelnemers die ART startten tijdens acute en chronische HIV-infectie veilig zal zijn, verhogen HIV-1 antigeen-specifieke T-cel immuunresponsen en verminderen viremie op laag niveau.
Daarnaast zijn secundaire doelstellingen:
- Bepaal de haalbaarheid van de productie van HXTC van deelnemers die met cART zijn begonnen tijdens acute en chronische HIV-infectie.
- Onderzoek het vermogen van autologe ex vivo geëxpandeerde HIV-1-specifieke T-cellen (HXTC) om HIV-1-specifieke immuunresponsen te verhogen bij deelnemers die met cART zijn gestart tijdens acute en chronische HIV-infectie.
- Onderzoek het vermogen van autologe ex vivo geëxpandeerde hiv-1-specifieke T-cellen (HXTC) om latente hiv-infectie te beïnvloeden, zoals gemeten door een kwantitatieve virale uitgroei-assay (QVOA) en geïntegreerde provirale DNA-kwantificering, en viremie op laag niveau, zoals gemeten door een enkele kopie assay (SCA) bij deelnemers die zijn gestart met cART tijdens acute en chronische hiv-infectie.

Beschrijving
Inclusiecriteria:
- ≥ 18 jaar en ≤65 jaar
Bevestiging van HIV 1-infectie
- Chronische HIV-infectie (CHI) wordt gedefinieerd als het documenteren van een positief HIV-testresultaat door een goedgekeurde ELISA-testkit en bevestigd door Western blot of Multispot HIV 1/HIV 2-assay voorafgaand aan screening. HIV-kweek, HIV-antigeen, plasma-HIV-RNA of een tweede antilichaamtest met een andere methode dan ELISA is aanvaardbaar als alternatieve bevestigende test.
- Acute hiv-infectie (AHI) wordt gedefinieerd als: 1) een negatieve of onbepaalde enzymimmunoassay (EIA) plus een reproduceerbaar detecteerbaar hiv door amplificatiemethoden, of 2) een positieve 4e generatie hiv Ag/Ab-combinatieassay en ofwel een negatieve/onbepaalde hiv- sneltest of een negatieve/onbepaalde Western-blot, of 3) een negatieve hiv-RNA-test binnen 45 dagen na een positieve EIA- en ART-initiatie.
Opmerking: de bovenstaande hiv-definities zijn relevant voor het moment van diagnose en start van de behandeling.
- AHI-deelnemers worden gedefinieerd als HIV-geïnfecteerde patiënten op onderdrukkende ART die binnen 45 dagen na AHI-diagnose is gestart, zoals gedefinieerd in protocolsectie 5.1.2.
- CHI-deelnemers worden gedefinieerd als hiv-geïnfecteerde patiënten die met chronisch hiv zijn begonnen met ART, zoals gedefinieerd in protocolparagraaf 5.1.2.
Bij krachtige antiretrovirale therapie, gedefinieerd als ten minste 2 nucleoside/nucleotide reverse transcriptaseremmers plus een niet-nucleoside reverse transcriptaseremmer, integraseremmer of een proteaseremmer zonder onderbreking (gedefinieerd als ontbrekende doses gedurende meer dan twee (2) opeenvolgende dagen of meer dan vier (4) cumulatieve dagen) in de 12 weken voorafgaand aan binnenkomst.
Andere krachtige volledig onderdrukkende antiretrovirale combinaties zullen van geval tot geval worden overwogen. Voorafgaande veranderingen in of eliminatie van medicijnen voor een eenvoudiger doseringsschema, intolerantie, toxiciteit of andere redenen zijn toegestaan als een alternatief onderdrukkend regime werd gehandhaafd.
- Stabiel ART-regime gedurende minimaal 3 maanden. OPMERKING: Het ART-regime wordt bepaald door de huidige behandelingsrichtlijnen. Deelnemers hebben mogelijk een of meer wijzigingen in hun ART-regime ondergaan voor tolerantie, wijziging van richtlijnen of vereenvoudiging van de dosering.
- Vermogen en bereidheid van de deelnemer om cART gedurende het onderzoek voort te zetten.
HIV-1-RNA in plasma onder de gedetecteerde limiet met conventionele assays (detectielimiet: 75, 50, 40 of 20 kopieën/ml) gedurende ˃1 jaar.
Een enkele niet-bevestigde plasma-hiv-RNA ˃ detectielimiet maar ˂ 1000 c/ml toegestaan binnen de voorgaande 12 maanden; maar geen in de voorgaande 6 maanden.
- Plasma HIV 1 RNA ˂ 50 kopieën/ml bij screening.
- Aantal CD4+-cellen ˃ 350 cellen/mm3 bij screening.
- Geen actieve HCV-infectie (meetbaar HCV-RNA) binnen 90 dagen na inschrijving.
- Geen actieve HBV-infectie (meetbaar HBV DNA of HBVsAg+) binnen 90 dagen na registratie.
- Alle vrouwelijke deelnemers die deelnemen aan seksuele activiteiten die tot zwangerschap kunnen leiden, moeten ermee instemmen om ten minste twee van de volgende betrouwbare anticonceptiemethoden te gebruiken (waarvan er ten minste één een barrièremethode is) gedurende ten minste 21 dagen voorafgaand aan deelname aan het onderzoek en tot 12 weken na de laatste dosis van het onderzoeksproduct: condooms (mannelijk of vrouwelijk) met of zonder zaaddodend middel, pessarium of pessarium met zaaddodend middel, spiraaltje, op hormonen gebaseerd anticonceptiemiddel, afbinden van de eileiders, NuvaRing.
- Alle mannelijke deelnemers die deelnemen aan seksuele activiteiten die tot zwangerschap kunnen leiden, moeten akkoord gaan met het gebruik van condooms gedurende ten minste 21 dagen voorafgaand aan deelname aan het onderzoek en tot 12 weken na de laatste dosis van het onderzoeksproduct.
- Vermogen en bereidheid van de proefpersoon om schriftelijke geïnformeerde toestemming te geven.
- Mogelijkheid en bereidheid om adequate locatorinformatie te verstrekken.
- Vermogen en bereidheid om effectief te communiceren met studiepersoneel; beschouwd als betrouwbaar, bereidwillig en coöperatief in termen van naleving van de protocolvereisten.
- Adequate vasculaire toegang voor HXTC-infusie en leukaferese.
- Potentiële deelnemer moet een adequate orgaanfunctie hebben, zoals blijkt uit de volgende laboratoriumwaarden:
Absoluut aantal neutrofielen (ANC): ≥ 1.500 /mcL Bloedplaatjes: ≥ 125.000 / mcL Hemoglobine: ≥ 12 g/dL
coagulatie:
Protrombinetijd of INR: ≤ 1,5 maal de bovengrens van normaal (ULN)

Serumcreatinine (of berekende creatinineklaring* voor mensen met creatinine > 1,3 ULN): ≤ 1,3 maal de bovengrens van normaal (ULN); OF Creatinineklaring* ≥ 60 ml/min voor potentiële deelnemers met creatininewaarden > 1,3 maal de institutionele ULN
Hepatisch serum totaal bilirubine: totaal bilirubine < 1,5 keer de bovengrens van het normale bereik. Als het totale bilirubine verhoogd is, moet het directe bilirubine < 2 keer het ULN-bereik zijn.
OPMERKING: Als de deelnemer atazanavir-bevattende therapie krijgt, moet een directe bilirubine worden gemeten in plaats van de totale bilirubine en moet deze ≤ 1,0 mg/dl zijn.
AST (SGOT) en ALT (SGPT): ≤ 2,0 keer ULN Alkalische fosfatase: ≤ 2,5 keer ULN
*De creatinineklaring moet worden berekend volgens de institutionele standaard.
Uitsluitingscriteria:
- Voorafgaand gebruik van een hiv-immunotherapie of vaccin binnen 12 maanden voorafgaand aan de screening.
- Gebruik van een van de volgende binnen 90 dagen voorafgaand aan opname: immunomodulerende, cytokine- of groeistimulerende factoren zoals systemische corticosteroïden, ciclosporine, methotrexaat, azathioprine, anti-CD25-antilichaam, IFN, interleukine 2 (IL 2), Coumadin, warfarine, of andere van Coumadin afgeleide anticoagulantia.
- Alle infusiebloedproducten, immunoglobuline of hematopoëtische groeifactoren ontvangen binnen 90 dagen voorafgaand aan deelname aan de studie.
- Zwangerschap of borstvoeding.
- Geschiedenis of ander bewijs van ernstige ziekte, maligniteit, immunodeficiëntie anders dan HIV, of enige andere aandoening die de proefpersoon naar de mening van de onderzoeker ongeschikt zou maken voor het onderzoek.
- Gebruik van topische steroïden over een totale oppervlakte van meer dan 0,5 mg/kg/dag binnen 30 dagen voorafgaand aan de screening.
- Elke actieve maligniteit die mogelijk chemotherapie of bestralingstherapie vereist.
- Gedwongen opsluiting (onvrijwillig opgesloten) voor de behandeling van een psychiatrische ziekte of een lichamelijke ziekte, bijvoorbeeld een besmettelijke ziekte. Het rekruteren en deelnemen van gevangenen is niet toegestaan.
- Elke goedgekeurde of experimentele niet-hiv-vaccinatie (bijv. hepatitis B, griep, pneumokokkenpolysaccharide) binnen 4 weken voorafgaand aan deelname aan het onderzoek.
- Kan geen persoon beschikbaar hebben om de deelnemer naar huis te brengen bij infuusbezoeken.
