Gedetailleerde beschrijving
Doel: Het onderzoek heeft vier doelen;
- Het primaire doel van de studie is om vast te stellen of vrouwen die wekelijks Taxol (80-100 mg/m2) of tweewekelijks Taxol (d.w.z. dosisdicht; 175 mg/m2) krijgen, tekorten vertonen in axon-reflex-gemedieerde vasodilatatie. (AMV) in de loop van zes weken van Taxol-therapie vergelijkbaar met die zijn gemeld bij patiënten met diabetische en genetisch overgeërfde neuropathieën.
- Het tweede doel van de studie is om te bepalen of vrouwen die ofwel wekelijks ofwel dosisdichte taxol krijgen, veranderingen in AMV ontwikkelen voordat ze tekenen en symptomen van chemotherapie-geïnduceerde perifere neuropathie (CIPN) ontwikkelen op een manier die het gebruik van veranderingen in AMV als een hulpmiddel ondersteunt. vroege detectiemethode voor CIPN met kleine vezels.
- Een derde (verkennend) doel van de studie is om te bepalen of tijdens de studie gedetecteerde veranderingen in AMV significant gecorreleerd zijn met zelfgerapporteerde CIPN op een manier die het gebruik van veranderingen in AMV als een bevestigende marker voor CIPN zou ondersteunen.
- Een laatste (verkennend) doel van de studie is het beschrijven van de grootte van axonreflexen en axonuitbarstingen bij vrouwen die wekelijks Taxol krijgen voordat ze beginnen met hun pre-Taxol anthracycline & cyclofosfamide (AC) therapie.

Beschrijving
Inclusiecriteria voor vrouwen met borstkanker:
- Leeftijd 18-85
- Engels kunnen lezen, schrijven en begrijpen
- Gediagnosticeerd met histologisch bevestigde, eerste keer, niet-gemetastaseerde borstkanker (stadium I-IIIB)
- Geen eerdere blootstelling aan neurotoxische chemotherapie of bestraling op het moment van inschrijving,
- Zullen wekelijks paclitaxel (Taxol® of generiek paclitaxel, 80-100 mg/m2) of tweewekelijks Taxol (d.w.z. dosisdicht; 175 mg/m2) krijgen als onderdeel van hun kankerbehandelingsregime OF
- Zal een anthracycline en cyclofosfamide (AC) krijgen, gevolgd door wekelijks paclitaxel (Taxol® of generiek paclitaxel, 80-100 mg/m2) of tweewekelijks Taxol (d.w.z. dosisdicht; 175 mg/m2) als onderdeel van hun kanker behandelingsregime;
Opnamecriteria voor gezonde controles:
- Leeftijd 18-85
- Kan Engels lezen, schrijven en begrijpen
Uitsluitingscriteria:
- Een voorgeschiedenis van hart- en vaatziekten, hypertensie of perifere arteriële/vasculaire aandoeningen;
- Huidig gebruik van (1) medicijnen/supplementen om de bloeddruk onder controle te houden (bijv. bètablokkers, nitraten, calciumkanaalblokkers, fosfodiësterase-5 (PGE5)-remmers) of (2) het gebruik van statines voor cholesterol;
- Vermoedelijke of gediagnosticeerde diabetes (met uitzondering van zwangerschapsdiabetes);
- Reeds bestaande neuropathie, neuropathische pijn of zenuwbeschadiging;
- Pijn of significante artritis in de tenen van beide voeten;
- Huidige huidziekte of schimmelinfectie van de voeten;
- Aanzienlijke schade of misvorming aan de voeten waardoor de bloedstroom zou veranderen of het onmogelijk zou worden om bevindingen te meten/interpreteren;
- Gediagnosticeerde of vermoede vasospastische ziekte zoals het syndroom van Raynaud;
- Huidig gebruik van tabak/tabaksbevattende producten;
- Diagnose van rustelozebenensyndroom of andere bewegingsstoornissen waardoor nauwkeurige gegevens niet kunnen worden verzameld.
Beperkingen tijdens de studie:
- Geen cafeïne- of alcoholhoudende producten gedurende 12 uur voorafgaand aan hun studiebezoek;
- Geen voedsel gedurende minstens een uur voorafgaand aan de controle van de bloedstroom;
- Geen niet-steroïde anti-inflammatoire geneesmiddelen (NSAID's) gedurende 24 uur voorafgaand aan studiebezoeken, tenzij voorgeschreven door een arts om dit te doen.
(Opmerking: deze beperkingen zijn bedoeld om de striktheid en kwaliteit van de gegevens te verbeteren, maar niet-naleving is geen reden voor uitsluiting van het onderzoek; naleving van deze beperkingen zal worden gecontroleerd tijdens zelfrapportage).

*Axonreflexen worden opgewekt in het palmaire oppervlak van de rechter grote teen met behulp van lokale huidverwarming met een warmtesonde van 0,33 cm2 volgens het volgende protocol:
De grootte van de axonreflex tijdens dit huidverwarmingsprotocol zal in beeld worden gebracht met behulp van Laser Doppler Flowmetry (LDF). In het geval dat deelnemers CIPN alleen aan hun linkerkant ontwikkelen, wordt de linkerteen vervangen.
Axonfakkels worden gegenereerd op het palmaire oppervlak van de rechter grote teen met behulp van hetzelfde lokale huidverwarmingsprotocol als hierboven beschreven, en afgebeeld met behulp van een laser spikkel contrast imager (LSCI) (ook bekend als full-field laser perfusion imager of FLPI).

Zelfgerapporteerde tekenen en symptomen van CIPN, zoals de aanwezigheid en locatie van (1) tintelingen, (2) gevoelloosheid en (3) neuropathische pijn, samen met informatie over (4) tekorten in trillingsdrempels en (5) verlies van diepe peesreflexen zullen bij elk studiebezoek worden beoordeeld met behulp van de 5-item, gemodificeerde Total Neuropathie Score (mTNS).
