Tidig upptäckt av taxaninducerad neuropati hos kvinnor med bröstcancer

Syfte: Studien har fyra syften;
- Det primära syftet med studien är att avgöra om kvinnor som får Taxol en gång i veckan (80-100 mg/m2) eller Taxol varannan vecka (dvs. dos-tät; 175 mg/m2) visar underskott i axonreflexmedierad vasodilatation (AMV) under sex veckors behandling med Taxol liknande de som har rapporterats hos patienter med diabetiska och genetiskt ärftliga neuropatier.
- Det andra syftet med studien är att fastställa om kvinnor som får antingen veckovis eller dos-tät Taxol utvecklar förändringar i AMV innan de utvecklar tecken och symtom på kemoterapi-inducerad perifer neuropati (CIPN) på ett sätt som stödjer användning av förändringar i AMV som en tidig upptäcktsmetod för småfibrigt CIPN.
- Ett tredje (utredande) syfte med studien är att fastställa om några förändringar i AMV som upptäckts under studien är signifikant korrelerade med självrapporterad CIPN på ett sätt som skulle stödja användning av förändringar i AMV som en bekräftande markör för CIPN.
- Ett sista (utredande) syfte med studien är att beskriva storleken på axonreflexer och axonutbrott hos kvinnor som får Taxol varje vecka innan de påbörjar sin behandling med antracyklin och cyklofosfamid (AC) före Taxol.

Beskrivning
Inklusionskriterier för kvinnor med bröstcancer:
- Ålder 18-85
- Kunna läsa, skriva och förstå engelska
- Diagnostiserats med histologiskt bekräftad, första gången, icke-metastaserande bröstcancer (stadium I-IIIB)
- Ingen tidigare exponering för neurotoxisk kemoterapi eller strålning vid tidpunkten för inskrivningen,
- Kommer att få paklitaxel varje vecka (Taxol® eller generisk paklitaxel, 80-100 mg/m2) eller Taxol varannan vecka (dvs. dos-tät; 175 mg/m2) som en del av sin cancerbehandlingsregim ELLER
- Kommer att få en antracyklin och cyklofosfamid (AC) följt av paklitaxel (Taxol® eller generisk paklitaxel, 80-100 mg/m2) eller varannan vecka Taxol (dvs. dos-tät; 175 mg/m2) som en del av sin cancer behandlingsregim;
Inklusionskriterier för sunda kontroller:
- Åldern 18-85
- Kan läsa, skriva och förstå engelska
Exklusions kriterier:
- En historia av kardiovaskulär sjukdom, hypertoni eller perifer arteriell/kärlsjukdom;
- Nuvarande användning av (1) mediciner/tillskott för att kontrollera blodtrycket (t.ex. betablockerare, nitrater, kalciumkanalblockerare, fosfodiesteras-5 (PGE5)-hämmare) eller (2) användning av statiner för kolesterol;
- Misstänkt eller diagnostiserad diabetes (med undantag för graviditetsdiabetes);
- Redan existerande neuropati, neuropatisk smärta eller nervskada;
- Smärta eller betydande artrit i tårna på endera foten;
- Aktuell hudsjukdom eller svampinfektion i fötterna;
- Betydande skada eller missbildning på fötterna som skulle förändra blodflödet eller göra det omöjligt att mäta/tolka fynd;
- Diagnostiserad eller misstänkt vasospastisk sjukdom såsom Raynauds syndrom;
- Nuvarande användning av tobak/tobaksinnehållande produkter;
- Diagnos av restless leg syndrome eller andra rörelsestörningar som skulle hindra korrekta data från att kunna samlas in.
Begränsningar i studien:
- Inga koffein- eller alkoholhaltiga produkter under 12 timmar före studiebesöket;
- Ingen mat under minst en timme före blodflödesövervakning;
- Inga icke-steroida antiinflammatoriska läkemedel (NSAID) under 24 timmar före studiebesök såvida inte en läkare uppmanas att göra det.
(Obs: Dessa restriktioner är utformade för att förbättra dataernas noggrannhet och kvalitet, men bristande efterlevnad kommer inte att vara skäl för uteslutning av studien; efterlevnaden av dessa restriktioner kommer att övervakas under självrapportering).

*Axonreflexer kommer att framkallas i palmarytan på höger stortå med lokal huduppvärmning med en 0,33 cm2 värmesond med följande protokoll:
Storleken på axonreflexen under detta huduppvärmningsprotokoll kommer att avbildas med hjälp av laserdopplerflödesmetri (LDF). Om deltagarna utvecklar CIPN endast på vänster sida, kommer den vänstra tån att ersättas.
Axon-flares kommer att genereras på palmarytan på höger stortå med samma lokala huduppvärmningsprotokoll som beskrivs ovan, och avbildas med en laser speckle contrast imager (LSCI) (även känd som full-field laser perfusion imager, eller FLPI).

Självrapporterade tecken och symtom på CIPN såsom närvaron och lokaliseringen av (1) stickningar, (2) domningar och (3) neuropatisk smärta, tillsammans med information om (4) brister i vibrationströsklar och (5) förlust av djupa senreflexer kommer att bedömas vid varje studiebesök med hjälp av 5-element, modifierad total neuropati Score (mTNS).
