Het veld van genomics is een opwindend nieuw veld dat wordt toegepast in de geneeskunde. Het gebruik ervan bij de behandeling van sommige vormen van kanker, sepsis en brandwondenpatiënten is veelbelovend. Naarmate de kennis van genomica en de toepassing van microarrays toeneemt, ontwikkelen onderzoekers intelligentere manieren om geïndividualiseerde zorg aan patiënten te bieden. Het plan van dit onderzoek is om het gebruik van genomics als instrument voor transplantatie op verschillende manieren toe te passen.
Deze onderzoeksstudie is bedoeld om methoden te ontwikkelen voor het isoleren van relevante celtypen via microarray-platen, om vervolgens mRNA-monsters van die celtypen te extraheren en hun genetisch profiel vast te leggen. Dit gebeurt met het bloed van maximaal tien gezonde donoren. Zodra dit vermogen is aangetoond, zal de volgende stap zijn om deze tests in verschillende hoedanigheden te gebruiken bij transplantatiepatiënten. Het onderzoeksplan omvat het volgen van het genetisch profiel van 26 niertransplantatiepatiënten. Eén groep zal prospectief worden gevolgd, beginnend bij de basislijn en vervolgens op verschillende vaste tijdstippen. Een tweede groep zal worden bemonsterd met het optreden van klinische gebeurtenissen. Deze gebeurtenissen omvatten virale, bacteriële en schimmelinfecties, WBC < 2,5 of door biopsie bewezen afstotingsepisoden of normale transplantaatfunctie. De bedoeling van de studie is dus om aan te tonen hoe immunosuppressie de expressie van deze genetische expressie van deze T-cellen verandert. Door dit te doen, zal de studie een beter begrip geven van de specifieke en echte immunosuppressieve toestand voor een bepaalde patiënt.
Studie Overzicht
Gedetailleerde beschrijving
De twee specifieke doelstellingen van het onderzoek zijn de volgende: 1) Proof of concept met twee delen. Ten eerste is het studieplan om te bewijzen dat T-cellen die heel vaak betrokken zijn bij de immuunrespons: CD2-, CD3-, CD4- en CD8-populaties kunnen worden gevangen. Deze cellen zullen worden gevangen, gelyseerd en hun mRNA-genomica zal in kaart worden gebracht. Het tweede deel van het proof of concept omvat het ex vivo blootstellen van de CD2-, CD3-, CD4- en CD8-populaties aan verschillende serumconcentraties van tacrolimus - de belangrijkste hoeksteen van immunosuppressieve medicatie bij orgaantransplantatie. Blootstelling van de drie celtypen aan de verschillende tacrolimusconcentraties zal aantonen dat er testen zijn die mRNA-transcripten kunnen detecteren die overeenkomen met onder, optimale en over immunosuppressie toestanden. Vervolgens zal de kwantificering van de IL2-productie van de drie celtypen onder de verschillende immunosuppressieve omstandigheden worden gemeten. Met deze 2 delen van het eerste specifieke doel van de studie, is een techniek gedemonstreerd die gemakkelijk en gemakkelijk de celpopulaties van belang kan isoleren van de transplantatiepatiënten en kan vastleggen hoe deze celpopulaties reageren op immunosuppressie bij verschillende concentraties. 2) Het tweede specifieke doel van het onderzoek is het uitvoeren van deze genomische profilering op ontvangers van een niertransplantatie. Voor dit deel van het onderzoek zullen er twee cohorten zijn. Een cohort geïdentificeerd als de "toekomstige" groep en een cohort gelabeld als de "gebeurtenis"-groep. Voor het toekomstige cohort zal op specifieke tijdstippen bloed van de patiënten worden afgenomen. Concreet zullen bloedmonsters worden genomen in de pre-transplantatiesetting wanneer de patiënten zich voor hun transplantatie in het ziekenhuis melden, vervolgens op dag 0, 1, 2, 3 en 7 (+/- 1 dag) na de niertransplantatie en op dag 14, 21 en 28 (+/-3 dagen). Na 2, 3, 6, 9 en 12 maanden (+/- 1 week) worden opnieuw bloedmonsters afgenomen. Voor de tweede groep zullen bloedmonsters worden genomen op het moment van "gebeurtenissen", zoals afstoting, infectie, enz., en dezelfde reeks tests op hun bloed toepassen. Voor deze groep wordt bloed afgenomen op tijdstip 0(+/-1 dag), vervolgens op één week (+/-2 dagen), en op één, twee en drie maanden (+/-1 week) na tijd 0. Per patiënt worden vijf bloedmonsters afgenomen. Voor alle patiënten zal er een vergelijking zijn van de standaard verzamelde klinische informatie over de patiënt - transplantaatfunctie, afstoting, infectie, immunosuppressieniveaus - met de resultaten van onze transcriptomics. Het hoogtepunt van het project zal zijn om te onthullen hoe de genomica van de specifieke typen T-cellen nauwkeuriger de werkelijke immunosuppressieve toestand van een patiënt kan weerspiegelen door te corresponderen met hoge, optimale en lage immunosuppressieve klinische fenotypes, en zo transplantatieartsen in staat te stellen op intelligentere en nauwkeurigere wijze immunosuppressieve middelen toe te dienen. medicijnen.
Deze studie omvat het verkrijgen van bloedmonsters van de niertransplantatiepatiënten op het moment dat ze anders bloed zouden laten afnemen voor standaard klinische zorg. Standaard verzamelde klinische informatie van patiënten zal over, optimaal en onder immunosuppressieve toestanden (IS) identificeren bij ontvangers van een niertransplantatie (txp).
In txp-zorg wordt de definitie van IS bepaald door het ziektebeeld van de patiënt.
Patiënten met infecties hebben te maken met immunosuppressie, patiënten met afstoting hebben immunosuppressie.
Patiënten met geen van beide en een goede transplantaatfunctie met normale Cr & goede urineproductie hebben een optimale IS.
De onderzoekers hopen een genomisch patroon van het mRNA van T-cellen te identificeren dat correleert met het klinische beeld van over-, onder- of optimale IS bij een niertxp-patiënt.
Bloed zal worden verzameld op het moment van de gebeurtenissen en genomics zal worden uitgevoerd op het T-cel-mRNA van dat bloed van elk type patiënt.
De specifieke gebruikte T-cel (CD3, CD4 of CD8) zal worden bepaald door het proof of concept-gedeelte van de studie.
De onderzoekers zullen dit deel van het onderzoek bijwerken aangezien deze informatie wordt verkregen nadat de proof of concept is voltooid.
