Onmiddellijke ART bij proefpersonen met opportunistische ziekten (TARi)
Impact van de timing van de start van antiretrovirale therapie (onmiddellijk versus vroeg) op het sterftecijfer van HIV/AIDS-patiënten die in het ziekenhuis zijn opgenomen met een opportunistische ziekte
Het doel van deze studie is het vergelijken van de klinische respons en het sterftecijfer door een opportunistische ziekte bij HIV-geïnfecteerde personen die onmiddellijk beginnen met conventionele antiretrovirale therapie.
Onmiddellijke ART (iART) wordt gedefinieerd als het starten van antiretrovirale therapie in de eerste 48 uur na de ziekenhuisopname.
Conventionele ART (cART) wordt gedefinieerd als het starten van antiretrovirale therapie zodra de opportunistische infectie onder controle is, naar goeddunken van een specialist in infectieziekten.

Steekproefgrootte werd berekend met behulp van formule, van verschil in verhoudingenformule met een bèta van 0,1, alfa van 0,05 en een verwacht verschil in mortaliteit van 20%. De steekproefomvang is 225 proefpersonen in elke groep. Geïnformeerde toestemming zal worden verkregen. Patiënten worden toegewezen aan elke groep (iART of cART) na randomisatie gestratificeerd op basis van het aantal CD4+ T-cellen (minder of meer dan 50 cellen/mm3).
De virale belasting in het plasma en het aantal CD4+ T-cellen zullen worden gemeten bij aanvang van het onderzoek en in de weken 2, 4, 12, 24 en 48 na de start van ART. Klinische uitkomsten zijn: mortaliteit na 4, 12, 24 en 48 weken, duur van ziekenhuisopname (gemeten in dagen), klinische en microbiologische genezing van de opportunistische ziekte, incidentie en ernst van immuunreconstitutie van inflammatoir syndroom (IRIS), en nadelige geneesmiddelreacties en interacties.
Zodra 50% van de steekproefomvang 30 dagen follow-up heeft voltooid, wordt een voorlopige analyse uitgevoerd om de veiligheid en werkzaamheid van iART te beoordelen; indien er verschillen in de mortaliteit worden waargenomen, wordt de studie beëindigd.

Inclusiecriteria:
- Meer dan 18 jaar oud
- Tijd van toelating tot studie ingang: minder dan 48 uur
- Bevestigde hiv-diagnose of grote kans op hiv-infectie op basis van een positieve hiv-sneltest
- Een opportunistische ziekte hebben
- Proefpersonen moeten naïef zijn voor ART of hun eerste of tweede ART-regime hebben gefaald of moeten ART gedurende meer dan 3 maanden hebben opgegeven
Uitsluitingscriteria:
- Meningitis door Cryptococcus spp. of voor M. tuberculosis
- Zwangerschap

Experimenteel: Immediate Antiretroviral Therapy (iART).
Participants randomized to this arm will initiate antiretroviral therapy (ART) within 48 hours of hospital admission.
The regimen will be selected by the treating physician based on clinical guidelines, with preference for second-generation integrase strand transfer inhibitor (INSTI)-based regimens (e.g., bictegravir/emtricitabine/tenofovir alafenamide [BIC/FTC/TAF]), unless contraindicated.

Deferred initiation of ART, with timing determined by the treating physician based on clinical stability and control of the opportunistic infection.
