Vergelijking van bed omhoog hoofd verhoogde positie met snuffelende positie bij snelle sequentiële inductie: een gerandomiseerde, gecontroleerde, non-inferioriteitsstudie
Deze studie is gericht op het uitvoeren van een gerandomiseerde, gecontroleerde studie waarin endotracheale intubatie (ETI) in de BUHE-positie met opstaande kop in bed wordt vergeleken met de snuffelpositie in gesimuleerde Rapid Sequence Induction (RSI).
Het doel is om te bepalen of de tijd die nodig is voor intubatie in de stand met het hoofd omhoog, niet inferieur is aan de tijd die nodig is voor intubatie in de snuffelpositie.
De hypothesen:
- Bij patiënten die snelle sequentiële inductie ondergaan in een gesimuleerde spoedoperatie onder algemene anesthesie, is directe laryngoscopie (DL) en ETI in de BUHE-positie niet minder dan de tijd die nodig is voor DL en succesvolle ETI in de snuffelpositie.
- Bij patiënten die snelle sequentie-inductie ondergaan in een gesimuleerde spoedoperatie onder algemene anesthesie, verbeteren directe laryngoscopie (DL) en ETI in de BUHE-positie de POGO-score.
- Bij patiënten die snelle sequentiële inductie ondergaan in een gesimuleerde spoedoperatie onder algemene anesthesie, vermindert directe laryngoscopie (DL) en ETI in de BUHE-positie luchtweggerelateerde complicaties.
Terminologie:
Directe laryngoscopie (DL) en endotracheale intubatie (ETI): Is een methode waarbij een beademingsslang in de luchtpijp (trachea) wordt ingebracht zodra de patiënt onder algemene anesthesie is gebracht.
Bed up head elevatie (BUHE): Bed omhoog op 20-30 graden gericht op uitlijning tussen de uitwendige gehoorgang met sternale inkeping.
Snuffelhouding: Rugligging behouden met hoofd omhoog met hoofdsteun.
Rapid Sequence Induction (RSI): een gevestigde methode voor het induceren van anesthesie bij patiënten die het risico lopen dat maaginhoud in de longen terechtkomt. Het gaat om bewustzijnsverlies tijdens cricoid-druk gevolgd door intubatie zonder gezichtsmaskerventilatie. Het doel is om de luchtpijp zo snel en veilig mogelijk te intuberen.
POGO-score: Percentage glottisopening
Cricoid-druk (CP): manoeuvre om regurgitatie van de maaginhoud tijdens inductie van anesthesie te voorkomen door tijdelijke occlusie van het bovenste uiteinde van de slokdarm door achterwaartse druk van cricoid-kraakbeen tegen lichamen van halswervels.

Alle patiënten van 18 jaar tot 75 jaar oud die een electieve operatie ondergaan onder algemene anesthesie in de operatiekamer van het University Malaya Medical Center gedurende een periode van 15 maanden, van april 2018 tot juni 2019, zullen worden opgenomen en gerekruteerd op basis van inclusie- en exclusiecriteria.
De patiënten die aan de criteria voldeden en toestemming gaven voor het onderzoek, worden gerandomiseerd in 2 groepen.
i) BUHE-groep: bed omhoog op 20-30 graden gericht op uitlijning tussen de uitwendige gehoorgang met sternale inkeping.
ii) Snuffelgroep: rugligging behouden met hoofd omhoog met hoofdsteun (foam donut).
Inductie van anesthesie begint met:
- pre-oxygenatie met 100% zuurstof gedurende 3-5 min uitgevoerd tot eindtijd zuurstof van 85% bereikt.
- Er wordt een vooraf berekende dosis inductiemiddel toegediend, onmiddellijk gevolgd door een neuromusculair blokkerend middel.
(IV Fentanyl 2mcg/kg, IV Propofol 2-3 mg/kg, IV Rocuronium 1 mg/kg).
- Cricoid-druk van 10 Newton wordt toegepast, oplopend tot 30 Newton zodra het bewustzijn verloren is.
- Na adequate neuromusculaire blokkade zullen beide groepspatiënten door één onderzoeker worden geïntubeerd via directe laryngoscopie met behulp van Macintosh-bladmaat 3 of 4.
- De tijd die nodig is vanaf het inbrengen van het Macintosh-blad in de mondholte tot de bevestiging van de plaatsing van de endotracheale tube via detectie van CO2 op de end tidal CO2-monitor, wordt geregistreerd.

Inclusiecriteria:
- Alle patiënten die een electieve operatie onder algehele narcose ondergaan vanaf de leeftijd van 18 jaar tot 75 jaar.
Uitsluitingscriteria:
- Patiënten met luchtwegobstructie
- Patiënten met een contra-indicatie voor nekverlenging
- BMI> 35kg/m2
- Patiënt met een voorgeschiedenis van een moeilijke luchtweg uit een eerdere intubatiegeschiedenis
- Alleen enkele intubatie wordt opgenomen als patiënten tijdens hun verblijf in het ziekenhuis meerdere operaties hebben ondergaan
- Patiënten met ischemische hartaandoeningen, cerebrovasculaire aandoeningen en aandoeningen van de luchtwegen
