本研究旨在进行一项随机对照试验,比较模拟快速序列诱导 (RSI) 中床头抬高 BUHE 位置的气管插管 (ETI) 与嗅探位置。
目的是确定在床头抬高位置进行插管所花费的时间是否不劣于在嗅探位置进行插管所花费的时间。
假设:
- 在全身麻醉下模拟急诊手术中接受快速序贯诱导的患者中,直接喉镜检查 (DL) 和 BUHE 位置的 ETI 并不劣于 DL 和成功的嗅探位置 ETI 所需的时间。
- 在全身麻醉下模拟急诊手术中接受快速序贯诱导的患者中,直接喉镜检查 (DL) 和 BUHE 位置的 ETI 可提高 POGO 评分。
- 在全身麻醉下模拟急诊手术中接受快速顺序诱导的患者中,直接喉镜检查 (DL) 和 BUHE 位置的 ETI 可减少气道相关并发症。
术语:
直接喉镜检查 (DL) 和气管插管 (ETI):是一种在患者接受全身麻醉后将呼吸管插入气管(气管)的方法。
仰卧头抬高 (BUHE):仰卧 20-30 度,瞄准外耳道与胸骨切迹之间的对齐。
嗅探姿势:保持仰卧位,头部抬高,头枕。
快速顺序诱导 (RSI):一种对有胃内容物吸入肺部风险的患者进行麻醉诱导的既定方法。 它涉及环状软骨压迫过程中意识丧失,随后在没有面罩通气的情况下进行插管。 目的是尽可能快速和安全地插管气管。
POGO 分数:声门开口的百分比

详细说明
从 2018 年 4 月至 2019 年 6 月,所有年龄在 18 岁至 75 岁之间在马来亚大学医学中心手术室接受全身麻醉下择期手术的患者将被纳入并根据纳入和排除标准招募。
那些符合标准并同意研究的患者将被随机分配到 2 组。
i) BUHE组:卧床20-30度瞄准外耳道与胸骨切迹对齐。
ii) 嗅探组:保持仰卧位,头部抬高,头枕(泡沫甜甜圈)。
麻醉诱导开始于:
- 进行 100% 氧气预充氧 3-5 分钟,直到达到 85% 的潮气末氧气。
- 给予预先计算剂量的诱导剂,然后立即给予神经肌肉阻滞剂。
(静脉注射芬太尼 2mcg/kg,静脉注射异丙酚 2-3mg/kg,静脉注射罗库溴铵 1mg/kg)。
- 一旦失去意识,施加 10 牛顿的环状软骨压力增加到 30 牛顿。
- 在充分的神经肌肉阻滞后,两组患者将由一名研究者使用 Macintosh 3 号或 4 号刀片通过直接喉镜进行插管。
- 将记录从将 Macintosh 刀片插入口腔到通过在呼气末 CO2 监测仪上检测 CO2 确认气管导管放置所花费的时间。

纳入标准:
- 所有在全身麻醉下接受择期手术的患者年龄从 18 岁到 75 岁。
排除标准:
- 气道阻塞患者
- 颈部伸展禁忌症患者
- 体重指数> 35kg/m2
- 既往插管史中有困难气道病史的患者
- 如果患者在住院期间进行了多次手术,则仅包括单次插管
- 缺血性心脏病、脑血管病、呼吸系统疾病患者
