Hallucinaties zijn waarnemingen zonder prikkel. 70% van de patiënten met schizofrenie lijdt aan verontrustende auditieve hallucinaties. Alleen al hun aanwezigheid verhoogt het risico op zelfmoord. De meeste bereiken remissie met D2-dopaminereceptorblokkerende geneesmiddelen na 1 jaar therapietrouw. Echter, 30% van de patiënten heeft hardnekkige hallucinaties en 50% houdt zich niet aan hun medicijnen, meestal vanwege ongunstige bijwerkingen - die hardnekkige en niet-verslaafde patiënten blijven lijden. Er is duidelijk behoefte aan een mechanistisch begrip van hallucinaties als opmaat voor een rationeel behandelontwerp.
Deze studie biedt de eerste stappen naar de ontwikkeling van een interventionele biomarker voor klinische hallucinaties, gebaseerd op computationele neurowetenschappen.
Computationele psychiatrie omvat het benutten van de kracht van computationele neurowetenschap om tegemoet te komen aan de klinische behoeften van mensen die lijden aan ernstige psychische aandoeningen. Er is veel discussie geweest over de belofte van de aanpak. Er zijn tot nu toe weinig studies geweest en ze hebben grotendeels betrekking op correlatieve methoden zoals functionele neuroimaging. Deze studie zal deze tekortkoming aanpakken door de neurale loci van computationele modelparameters persoonlijk te manipuleren bij patiënten met psychose met behulp van transcraniële magnetische stimulatie (TMS), waarbij de impact van deze manipulatie op gedragstaakprestaties wordt gevolgd. Met een dergelijke causale tussenkomst zal de waarheidsgetrouwheid van de verklaring van het model van hallucinaties gevalideerd of ontkracht worden. Indien gevalideerd, kan het model verder worden ontwikkeld als biomarker voor het voorspellen van het begin van hallucinaties, het begeleiden, ontwikkelen of volgen van de effecten van behandelingen voor hallucinaties. Als het niet wordt bevestigd, moet het model worden verworpen en moeten andere alternatieven worden nagestreefd.

Inclusiecriteria:
- Leeftijd 18 - 45 jaar
- Stem horende patiënten
- Voldoe aan de diagnostische criteria voor DSM-V schizofrenie of schizofreniforme stoornis
- Meld het horen van stemmen minstens één keer per dag
- Score > 3 op PANSS P3 (item hallucinaties)
Uitsluitingscriteria:
- Stoornis in het gebruik van middelen uit de DSM-V in de afgelopen 6 maanden
- Eerder hoofdletsel met neurologische symptomen en/of bewusteloosheid
- Verstandelijke beperking (IQ < 70)
- Niet-Engelse spreker
Contra-indicaties voor TMS, waaronder:
- Geschiedenis van aanvallen
- Metalen implantaten
- Pacemaker
- Zwangerschap
- Minder dan 6 weken van een stabiele dosis psychotrope medicatie(s)
- Diagnose van comorbide stemming of angst
- Klinische/gedragsmatige instabiliteit en niet in staat om mee te werken aan TMS-procedures
- Klinisch significante medische aandoening(en)
- Onstabiele medische toestand(en) op basis van ECG, medische geschiedenis, lichamelijk onderzoek en routinematig laboratoriumwerk
- Persoonlijke geschiedenis van een beroerte
- Familiegeschiedenis van toevallen

Actieve vergelijker: TMS naar insula
Voor deze studie zullen 30 klinische stemmenhoorders (P+H+) worden aangeworven. Ze zullen twee parallelle vormen van de geconditioneerde hallucinatiestaak (met verschillende visuele en auditieve stimuli) bij twee gelegenheden voltooien, met een tussenpoos van een week. TMS en schijnvertoning worden geleverd in een willekeurige volgorde met een tegengewicht. Hypothese: het remmen van de insula zal eerdere overgewicht verminderen. Als deze computationele verstoring verantwoordelijk is voor geconditioneerde hallucinaties, dan zal het verbeteren ervan met TMS dat de betrokkenheid van de insula vergroot, de geconditioneerde hallucinatiereacties verminderen. Bovendien zal de eerdere wegingsparameter worden verlaagd na actieve TMS in vergelijking met schijnvertoning. |
Transcraniële magnetische stimulatie (TMS) is een niet-invasieve vorm van hersenstimulatie waarbij een veranderend magnetisch veld wordt gebruikt om elektrische stroom te veroorzaken in een specifiek gebied van de hersenen door middel van elektromagnetische inductie.
Een elektrische pulsgenerator, of stimulator, is verbonden met een magnetische spoel, die op zijn beurt is verbonden met de hoofdhuid.
De stimulator genereert een veranderende elektrische stroom in de spoel die een magnetisch veld induceert; dit veld veroorzaakt dan een tweede inductantie van omgekeerde elektrische lading in de hersenen zelf.

Actieve vergelijker: TMS naar het cerebellum
Voor deze studie zullen nog eens 70 klinische stemmenhoorders worden aangeworven.
Nogmaals, ze zullen twee keer parallelle vormen van de geconditioneerde hallucinatietaak voltooien, met een tussenpoos van een week.
Ze zullen prikkelende TMS ontvangen over het cerebellum (en schijnvertoning bij de andere gelegenheid, in een willekeurige, gecompenseerde volgorde).
Hypothesen: het prikkelen van het cerebellum zal het bijwerken van overtuigingen vergroten.
Als slechte geloofsupdates bijdragen aan geconditioneerde hallucinaties, zou een toenemende betrokkenheid van het cerebellum de geconditioneerde hallucinaties moeten verminderen en de modelparameter voor geloofsupdates moeten veranderen in vergelijking met schijn-TMS.

De primaire uitkomstmaat is het aantal keren dat deelnemers hoortonen rapporteren die niet werden gepresenteerd.
Er zijn in totaal 360 proeven.
Er zijn 120 proeven zonder toon.
Mensen die stemmen horen, melden typisch hoortonen bij 30% van de geen-toonpogingen (ongeveer 36 keer, vergeleken met 12 keer bij mensen die geen stemmen horen).
De onderzoekers verwachten minder geconditioneerde hallucinaties (minder dan 36 meldingen van tonen wanneer er geen werden gepresenteerd) in de actieve TMS-condities in vergelijking met de schijnvertoning.
