Zuurstof met gesloten lus om te verifiëren dat de interventies van gezondheidswerkers afnemen tijdens SARS-CoV-2-pneumonie (COVID-19)
Geautomatiseerde zuurstoftitratie - Monitoring en ontwenning bij patiënten met infectieuze pneumonie die zuurstof nodig hebben - Impact op het aantal interventies voor gezondheidswerkers. Een innovatief apparaat om patiënten met COVID-19-pneumonie te behandelen COVID-onderzoek (zuurstof met gesloten lus om te verifiëren dat de interventies van zorgverleners afnemen tijdens longontsteking)
Er is een hoog risico op overdracht van COVID-19 op gezondheidswerkers. In een recent cohort was 29% van de opgenomen patiënten gezondheidswerkers. Een van de belangrijkste strategische doelstellingen van de WHO voor de reactie op COVID-19, was de eerste om de overdracht van mens op mens te beperken, inclusief het verminderen van secundaire infecties onder nauwe contacten en gezondheidswerkers.
Geautomatiseerde zuurstoftitratie, ontwenning en monitoring (FreeO2-apparaat) kan een oplossing zijn om het aantal interventies van gezondheidswerkers in verband met zuurstoftherapie te verminderen, complicaties in verband met zuurstof te verminderen en monitoring te verbeteren.

Inclusiecriteria:
- Leeftijd > 18 jaar
- patiënten met acute respiratoire insufficiëntie in verband met vermoedelijke buiten het ziekenhuis opgelopen pneumonie (viraal en niet-viraal) die zuurstoftherapie < 6 l/min (of FiO2 < 0,50) nodig hebben (om de SpO2 tussen 90 en 94% SpO2 te houden) zonder criteria voor onmiddellijke intubatie of overplaatsing naar de IC .
- Patiënten ziekenhuisopname < 72 uur
Uitsluitingscriteria:
- schoktoestand,
- geen SpO2-signaal beschikbaar,
- geduldige agitatie,
- pH < 7,30 (indien bloedgas beschikbaar)
- PaCO2 > 50 mmHg, (indien bloedgas beschikbaar) of chronische hypercapnie voorgeschiedenis
- Niet-invasieve ademhalingsondersteuning (NIV, High Flow Nasal Therapy (HFNT)) bij opname in het onderzoek
- Intrekking van levensonderhoud of palliatie als doel van zorg
- weigering van patiënten of naasten om deel te nemen aan het onderzoek

ACTIVE_COMPARATOR: Controlegroep
De patiënten zullen de gebruikelijke zorg krijgen met betrekking tot hun medische behandeling. In de gebruikelijke controlegroep wordt zuurstof toegediend volgens de gebruikelijke lokale praktijken |
De onderzoeker adviseerde een SpO2-streefwaarde van 90-94%. De onderzoeker zal aanbevelen om de lage/hoge SpO2-alarmen in te stellen op 88% en 96% als continue oximetrie wordt gebruikt. In deze groep is de SpO2 op elk moment geregistreerd met het FreeO2-apparaat - opnamemodus |

EXPERIMENTEEL: Interventie groep
De patiënten zullen de gebruikelijke zorg krijgen met betrekking tot hun medische behandeling. In de interventiegroep wordt geautomatiseerde zuurstoftoediening geleverd met FreeO2 |
In deze groep wordt zuurstof toegediend met FreeO2 (automatische zuurstoftitratie) met een SpO2-streefwaarde ingesteld op 92% (om de oxygenatie binnen de aanbevolen SpO2-streefwaarde te houden: 90-94%)

Evaluatie van de evolutie van de NEWS 2-score (National Early Warning-score) hangt samen met de evolutie van de patiënt. De NEWS2-score wordt berekend, maar er wordt niet ingegrepen op basis van deze score. De evolutie van de patiënt zal worden vergeleken bij de interpretatie van NEWS 2. Interpretatie Een lage score (NEWS 1-4) zou moeten leiden tot beoordeling door een bekwame geregistreerde verpleegkundige die moet beslissen of een wijziging van de frequentie van klinische monitoring of een escalatie van klinische zorg nodig is. Een gemiddelde score (dwz NEWS van 5-6 of een RED-score) zou moeten overwegen of escalatie van zorg naar een team met vaardigheden op het gebied van kritieke zorg vereist is (dwz outreach-team voor kritieke zorg). Een hoge score (NEWS ≥7) zou aanleiding moeten zijn voor een noodevaluatie door een klinisch team/team voor kritieke zorg met competenties op het gebied van kritieke zorg en gewoonlijk wordt de patiënt overgebracht naar een zorggebied met een hogere afhankelijkheid. |
