Herstellend plasma bij de vroege behandeling van hoogrisicopatiënten met SARS-CoV-2 (COVID-19)-infectie

Na toediening van één eenheid (ongeveer 200 ml) herstellend plasma gedurende een uur, stelt de studie voor om de therapeutische werkzaamheid (responspercentage) van herstellende plasma-infusie te bepalen bij patiënten met een hoog risico op overlijden wanneer ze zijn geïnfecteerd met SARS-CoV-2 ( COVID-19) door preventie van progressie naar ernstige of levensbedreigende COVID-19 tijdens de huidige ziekenhuisopname, zoals bepaald door te evalueren of de patiënt het volgende ervoer: ademhalingsfrequentie >30/min, bloedzuurstofverzadiging <93%, partiële druk van arteriële zuurstof tot fractie van ingeademde zuurstofrantsoen <300, of kreeg een medische diagnose van respiratoire insufficiëntie, septische shock of disfunctie/falen van meerdere organen. Dit wordt vastgelegd op basis van het dagelijkse lichamelijk onderzoek/klinische onderzoek dat wordt uitgevoerd als onderdeel van de routinezorg en bij ontslag.
De studie stelt ook voor om de immunologische effecten van herstellende plasma-infusie te bepalen, zoals gemeten door middel van seriële SARS-CoV-2 Ag-niveaus door middel van RT-PCR gemeten door CoV PCR verzameld bij inschrijving, dag 7 en ontslag.
Ten slotte is de studie bedoeld om de normalisatie van laboratoriumparameters voor risico's te meten, die elke 3 dagen zullen worden gedocumenteerd terwijl de patiënt in het ziekenhuis wordt opgenomen tot het moment dat de laboratoriumwaarde terugkeert naar binnen het normale bereik van de instelling.

Inclusiecriteria:
- Diagnose van SARS-CoV-2-infectie via RT-PCR of door de FDA goedgekeurde tests.
- Patiënten moeten ook de volgende indicaties hebben voor inschrijving:
- i. D-dimeer > 500 ng/ml FEU OR
- ii. IL-6 > 5 pg/ml
Met een van de volgende:
- iii. Lymfocyten < 0,8 103/ul OF
- iv. LDH > 700 U/L OF
- v. CK > 170 U/L OF
- vi. CRP > 1,0 mg/dl OF
- vii. Ferritine > 1000 ng/ml
EN een van de volgende:
- viii. Leeftijd ouder dan 60 jaar
- ix. Onderliggende actieve maligniteit
- X. Hart-en vaatziekte
- xi. Actief tabaksgebruik
- xiii. Geschiedenis van longvolumereductiechirurgie
- xiii. Hypertensie
- Voorafgaande behandeling: Patiënten komen nog steeds in aanmerking voor deze studie als er actieve antimicrobiële middelen worden gebruikt. Patiënten komen ook in aanmerking als ze in de loop van hun ziekte zijn behandeld in een klinische studie naar COVID-19.
- Leeftijd ≥ 18 jaar.
- De effecten van allogene plasma-infusie op de zich ontwikkelende foetus zijn niet bekend. Om deze reden komen vrouwen die zwanger zijn niet in aanmerking voor deelname.
- Stemt in met de vereiste verzamelde laboratoriumgegevens, waaronder de basislijn orgaanfunctie en regelmatige doorlopende beoordelingen die worden uitgevoerd als onderdeel van routinematige zorg.
- Het vermogen om een schriftelijk geïnformeerd toestemmingsdocument te begrijpen en de bereidheid om toestemming te krijgen van de wettelijk geautoriseerde vertegenwoordiger.
Uitsluitingscriteria:
- 4.2.1 Patiënten die niet voldoen aan bovenstaande inclusiecriteria komen niet in aanmerking.
- 4.2.2 Het is mogelijk dat patiënten geen andere onderzoeksagentia krijgen.
- 4.2.3 Geschiedenis van allergische reacties toegeschreven aan eerdere transfusiegeschiedenis.
- 4.2.4 Ademfrequentie >30/min
- 4.2.5 Bloedzuurstofverzadiging <93%
- 4.2.6 Partiële druk van arteriële zuurstof tot fractie van ingeademde zuurstofverhouding <300
- 4.2.7 Diagnose van respiratoire insufficiëntie, septische shock of disfunctie/falen van meerdere organen

Experimenteel: Behandelingsarm
Het onderzoek bestaat uit een enkele groep van alle patiënten die in het ziekenhuis zijn opgenomen en die voldoen aan de inclusie-/uitsluitingscriteria en die geïnformeerde toestemming geven voor het ontvangen van één eenheid (ongeveer 200 ml) herstellend plasma met dagelijks verzamelde gegevens over routinematige (niet-onderzoeks) klinische beoordelingen/fysieke onderzoeken en laboratoriumresultaten.

Plasma verkregen van personen die eerder gediagnosticeerd waren met SARS-CoV-2 (COVID-19) wordt toegediend aan gehospitaliseerde patiënten die voldoen aan de opname-/uitsluitingscriteria en die geïnformeerde toestemming hebben gegeven.
