Nauw gematchte niet-verwante donor Perifere bloedstamceltransplantatie met TCRαβ+ T-cel- en B-celdepletie voor patiënten met sikkelcelziekte en thalassemie major
Dit is een eenarmige pilotstudie van perifere stamceltransplantatie (PSCT) met ex vivo t-celreceptor alfa beta+(TCRaβ+) T-cel en cluster van differentiatie 19+ beta (CD19+ B) celdepletie van niet-gerelateerde donor (URD) transplantaten het gebruik van het CliniMACS-apparaat bij patiënten met sikkelcelziekte (SCD) en bèta-thalassemie major (BTM).

Dit is een eenarmige pilotstudie van perifere stamceltransplantatie (PSCT) met ex vivo TCRαβ+ T-cel- en CD19+ B-celdepletie van URD-transplantaten met behulp van het CliniMACS-apparaat bij patiënten met SCD en BTM.
Afgezien van op CliniMACS gebaseerde celverwerking, zal PSCT worden uitgevoerd volgens de huidige zorgstandaarden in de celtherapie- en transplantatieafdeling van het Children's Hospital of Philadelphia (CHOP), inclusief het gebruik van een standaard chemotherapieconditioneringsregime en standaard vervolglaboratoriumbeoordelingen.
De studie zal de werkzaamheid van deze strategie bepalen in termen van enting, percentages van acute en chronische graft-versus-host-ziekte (GvHD) en eenjarige algehele en gebeurtenisvrije overleving.

Deelname Criteria

Inclusiecriteria
Ernstige sikkelcelziekte
- Genotype: hemoglobine SS, hemoglobine SC, hemoglobine SD, SOArab of hemoglobine SBèta-thalassemie
- Moet ten minste een van de volgende ziekteverschijnselen hebben
- Klinisch symptomatisch neurologisch voorval (beroerte) of een neurologisch tekort dat langer dan 24 uur aanhoudt op enig moment voorafgaand aan inschrijving
- Geschiedenis van twee of meer episodes van vaso-occlusieve gebeurtenissen (VOE) per jaar in de 2 jaar voorafgaand aan inschrijving. Patiënten moeten refractair zijn voor hydroxyurea, gedefinieerd als het ontwikkelen van VOE ondanks het ontvangen van hydroxyurea gedurende ten minste 6 maanden. Patiënten die hydroxyurea niet verdragen, kunnen ook worden ingeschreven.
Vaso-occlusieve gebeurtenissen omvatten:
- Acuut chest syndroom
- Pijnepisodes die intraveneuze pijnbehandeling en/of ziekenhuisopname vereisen
- Priapisme
- Miltsekwestratie (gedefinieerd als een daling van 2 g/dl in hemoglobine in de setting van een acuut vergrote milt. Dit wordt bepaald als onderdeel van de klinische zorg en voorafgaand aan het onderzoek)
- Toediening van regelmatige transfusietherapie met rode bloedcellen (RBC), gedefinieerd als het ontvangen van ≥ 8 RBC-transfusies in het jaar voorafgaand aan de inschrijving om sikkelcelgerelateerde complicaties van welke aard dan ook te voorkomen volgens het oordeel van de behandelend hematoloog.
Bèta-thalassemie major
- Genotype: bevestigd bèta-thalassemie-genotype door moleculair genetische tests (kan E/Beta0- en Beta0/Beta+-genotypes omvatten)
- Moet voldoen aan de klinische diagnose van transfusie-afhankelijke thalassemie, gedefinieerd als behoefte aan ≥ 8 RBC-transfusies per jaar in de twee jaar voorafgaand aan de studie-inschrijving.
Uitsluitingscriteria
- Patiënten die niet voldoen aan de ziekte-, orgaan- of infectiecriteria.
- Vorige Hematopoëtische stamceltransplantatie (HSCT)
- Patiënten zonder geschikte niet-verwante donor beschikbaar. Patiënten met een geschikte volledig gematchte verwante donor komen ook niet in aanmerking.
- Zwangere vrouwtjes. Alle vrouwen die zwanger kunnen worden, moeten een negatieve zwangerschapstest hebben.
- Deelname aan een klinisch onderzoek waarbij de patiënt een onderzoeksgeneesmiddel krijgt, moet worden stopgezet voordat de transplantatietherapie wordt gestart. Met name transplantatiechemotherapie mag pas beginnen na ten minste 3 halfwaardetijden na het laatste gebruik van het onderzoeksgeneesmiddel.
- Ernstige RBC-alloimmunisatie, gedefinieerd als het onvermogen om verpakte RBC-transfusietherapie te ontvangen vanwege anti-RBC-antilichamen. Patiënten met een hoge titer anti-donor antilichamen tegen humaan leukocytenantigeen (HLA) die bij de screening worden gedetecteerd, kunnen worden ingeschreven als ze bereid zijn om HLA-antilichaamdesensibilisatietherapie te ondergaan.

Incidentie van acute graft-versus-hostziekte (GVHD)

Acute GvHD werd beoordeeld aan de hand van het aantal patiënten dat acute graft-versus-hostziekte ontwikkelde, ingedeeld volgens de huidige richtlijnen van het Center for International Bone Marrow Transplant Registry (CIBMTR).
De gradering volgt vastgestelde criteria op basis van de ernst van huid-, lever- en maag-darmbetrokkenheid, waaronder de omvang van huiduitslag, verhoogd bilirubine en gastro-intestinale symptomen (bijv. diarreevolume).
De evaluatie werd uitgevoerd door middel van klinische beoordeling en laboratoriumgegevens in overeenstemming met de standaardpraktijken voor het indelen van transplantatie-gerelateerde acute GvHD.
