Het is meer dan 80 jaar geleden dat Gibbs et al. lieten zien hoe het elektro-encefalogram (EEG) systematisch veranderde bij toenemende doses slaapmiddelen zoals penthobarbital en ether. De studie concludeerde dat "elektro-encefalografie daarom van waarde kan zijn bij het beheersen van de diepte van anesthesie en sedatie". Ondanks een solide documentatie van het systematische verband tussen de dosering van anesthetica, EEG-patronen en het niveau van sedatie/anesthesie, is op EEG gebaseerde DoA geen onderdeel geworden van de standaardbehandeling bij anesthesiemanagement. Er is overvloedig bewijs van hoe verschillende anesthetica leiden tot karakteristieke fluctuaties in de elektrische activiteit van het menselijk brein, met betrekking tot de diepte van de anesthesie, het gekozen anestheticum en de leeftijd. Deze door anesthesie veroorzaakte fluctuaties zijn gemakkelijk zichtbaar als veranderingen in het EEG van de patiënt.
Anesthetica worden meestal toegediend in farmacologische modellen op basis van een populatie, rekening houdend met hun leeftijd, gewicht en lengte. Er is echter een significant verschil in hoe patiënten op deze modellen reageren. Bij volwassenen zijn er aanwijzingen dat de doses die nodig zijn om bij bewustzijn te komen, variëren met een factor 2 boven en onder de aanbevolen doses. Bij onderdosering van anesthetica bestaat het risico van peroperatief bewustzijn. Anderzijds zijn er ook aanwijzingen dat overdosering van anesthetica schadelijke effecten heeft; kinderen die meer dan 4% sevofluraan krijgen, kunnen epileptiforme activiteit vertonen, en volwassenen die een overdosis nemen tot "burst-onderdrukking" tijdens anesthesie hebben een hoger risico op postoperatief delirium (POD) en een verhoogd optreden van postoperatieve cognitieve disfunctie (POCD).
Bispectral Index (BIS) is een algoritme ontwikkeld door Aspect Medical Systems in 1994, dat is gebaseerd op gewogen sommen van EEG-subparameters om een index van 0 tot 100 weer te geven voor de diepte van de anesthesie, waarbij 100 klaarwakker is en 0 een iso-elektrisch EEG . Het BIS-doel voor een anesthesie die diep genoeg is, ligt tussen de 40 en 60. Het BIS-nummer komt vaak overeen met andere klinische observaties met betrekking tot anesthesiediepte, maar er is ook een ervaring van divergentie. BIS en andere op EEG gebaseerde indices zijn geprogrammeerd vanuit volwassen cohorten en kunnen niet direct worden vertrouwd bij kinderen of ouderen. Er is ook een onvermogen in deze voorgeprogrammeerde indices (BIS en andere) om te integreren hoe specifieke anesthetica het EEG beïnvloeden, en daarna de BIS-waarde. Een voorbeeld hiervan is hoe het medicijn ketamine een specifieke gamma-frequentie in het EEG induceert, wat de BIS-index vertaalt als een lichtere anesthesie, ook al wordt het medicijn "bovenop" een toch al diepe anesthesie toegediend.
