Tandimplantaten bij patiënten onder orale antistollingstherapie
Werkzaamheid van lokale hemostatische behandeling bij implantaatchirurgie bij antistollingspatiënten die warfarine gebruiken: een gerandomiseerde klinische studie

Het doel van deze studie was het vergelijken van: 1) de frequentie van onmiddellijke en kortdurende postoperatieve bloedingen met behulp van tranexaminezuur (TXAg), bismutsubgallaat (BSg) of droog gaas (DGg) als lokaal hemostatisch middel 2) onderzoek naar de relatie tussen bloedingen en het optreden van hematomen, met lengte van de incisie, duur van de operatie en recontouring van de alveolaire rand. 80 chirurgische procedures uitgevoerd bij in totaal 71 patiënten (20 chirurgische procedures bij 18 patiënten die geen OAT kregen en 60 chirurgische procedures uitgevoerd bij 53 patiënten op OAT) die werden toegewezen aan een van de vier groepen, zodat elke groep 20 procedures omvatte. De controlegroep (Cg) bestond uit 20 procedures die werden uitgevoerd bij patiënten die geen OAT kregen. De 60 procedures die moesten worden uitgevoerd bij patiënten op OAT werden willekeurig toegewezen aan een van de drie volgende experimentele groepen: 1) TXAg-groep, OAT plus TXA als lokaal hemostatisch middel; 2) BSg-groep, OAT plus BS als lokaal hemostatisch middel; 3) DGg-groep, OAT plus compressie met droog gaas als lokaal hemostatisch middel. Uitkomstvariabelen waren:
Intraoperatieve variabelen: lengte van incisie uitgedrukt (Li) recontouring van de alveolaire rand (Arr), totale duur van de operatie (Ds), chirurgisch kwadrant en implantaatlocatie.
Postoperatieve variabelen: onmiddellijke postoperatieve bloeding (binnen de eerste 30 minuten na de operatie), kortdurende bloeding: binnen de eerste 5 dagen na de operatie, bepaald volgens de door Bacci beschreven index, aanwezigheid van intraorale/extraorale hematomen in huid of slijmvliezen. Beschrijvende statistische, variantieanalyse en regressielogistieke analyse werden uitgevoerd. Statistische significantie werd vastgesteld op een waarde van p<0,05.

Inclusiecriteria:
Onderwerpen:
- Systemisch gezond.
- Gezonde parodontale weefsels of goed gecontroleerde parodontale gezondheid.
- Maximaal twee tandheelkundige implantaten per hemiarcade nodig hebben,
- Toont ≥8 mm bothoogte en ≥6 horizontale botbreedte.
- Er zijn maximaal twee chirurgische ingrepen nodig.
- Die schriftelijke toestemming hadden van de verwijzende dienst om de chirurgische ingreep te ondergaan.
Uitsluitingscriteria:
Onderwerpen:
- Implantaatplaatsing onmiddellijk na extractie vereist.
- Systemische ziekte die een contra-indicatie vormt voor implantaatchirurgie.
- Geestelijke beperking waardoor ze het protocol niet kunnen naleven.
- Hematologische, metabolische, auto-immuun- of botziekten.
- Leververanderingen of het krijgen van medicatie die de leverfunctie beïnvloedt.
- Corticoïdtherapie, chemotherapie of antistollingstherapie ontvangen binnen 10 dagen voorafgaand aan de operatie.
- Antibiotica gebruiken die interageren met orale anticoagulantia.
- Vereist verticale of periosteale incisies.
- Strategie voor botregeneratie vereist.
Wanneer de behandeling het plaatsen van een implantaat in meer dan één hemiarcade inhield, werden de overeenkomstige operaties 30 dagen na elkaar gepland.

Actieve vergelijker: Controlegroep (Cg)
De controlegroep (Cg) bestond uit 20 procedures die werden uitgevoerd bij patiënten die geen OAT kregen. Buccale en palataal-linguale flappen werden opnieuw gepositioneerd en gehecht met eenvoudige hechtingen met behulp van 5/0 monofilament nylongaren, en een droog gaas werd aangebracht gedurende 30 minuten. |

Experimenteel: Tranexaminezuurgroep (TXAg)
De TXAg-groep bestond uit 20 procedures die werden uitgevoerd bij OAT-patiënten.
Buccale en palataal-linguale flappen werden verplaatst en gehecht zoals in de Cg plus compressie op de wond gedurende 30 minuten met behulp van een gaas gedrenkt in de inhoud van een ampul van 500 mg TXA, waarna een nieuw gaas gedrenkt in de inhoud van een ampul van 500 mg TXA werd gedurende twee uur aangebracht.

Experimenteel: Bismut-subgallaatgroep (BSg)
De BSg-groep bestond uit 20 procedures die werden uitgevoerd bij OAT-patiënten.
Op het moment van de operatie werd de inhoud van een anestube (1,8 ml) gemengd met een voldoende hoeveelheid BS-poeder om een pasta te verkrijgen die qua consistentie vergelijkbaar was met tandpasta (29).
Een dunne laag van de pasta werd op de botrand aangebracht en de buccale en palataal-linguale flappen werden vervolgens opnieuw gepositioneerd en gehecht zoals in de Cg.
met een droog gaasje gedurende 30 minuten op de wond gedrukt, waarna er weer twee uur lang een nieuw droog gaasje werd geplaatst.

Experimenteel: Droge gaasgroep (DGg)
De DGg-groep bestond uit 20 procedures die werden uitgevoerd bij OAT-patiënten.
Buccale en palataal-linguale flappen werden opnieuw gepositioneerd en gehecht zoals in de Cg plus compressie op de wond met een droog gaasje gedurende 30 minuten, waarna een nieuw droog gaasje werd geplaatst voor nog eens twee uur.
